BACKGROUND: The randomized clinical trial of implantable drug delivery systems (IDDS) plus comprehensive medical management (CMM) versus CMM alone showed better clinical success at 4 weeks for IDDS patients. This 'as treated' analysis assessed if improvements in pain control, drug toxicity and survival were maintained over time.

OBJECTIVES: This report seeks to highlight a pitfall that may be encountered in the management of patients with trigeminal neuralgia with imaging showing both neurovascular conflict as well as tumors.
CASE PRESENTATION: A case of a 53 year old male with simultaneous neurovascular conflict and a vestibular schwannoma with trigeminal neuralgia is presented and the management is discussed with reference to managing this particular subset. Pain was noted to be likely generated by neurovascular conflict and not by the tumor, which is usually not the case.

The Danish Health Authority has recently published a new guideline on the treatment of lumbar disc herniation with radiculopathy. The main features of multiple myeloma bone disease (MBD) are hyperactivity of systemic bone destruction and inhibition of new bone formation. Its clinical manifestations include pain, osteoporosis, pathologic fractures and associated nerve compression symptoms etc. MBD is one of the most common complications of multiple myeloma (MM). Its misdiagnosis rate is higher, and the patients' quality of life and prognosis are poor. This review discusses the most recent advancement on its pathogenesis and novel therapies, so as to provide reference for clinical treatment of MBD.

We report the case of a Thai male patient with neuralgic amyotrophy, presenting with acute and severe pain in the upper extremities, followed by patchy muscle weakness, associated with hepatitis E virus (HEV) infection. The entire HEV genome was sequenced and subjected to phylogenetic analysis and found to be genotype 3. Data on the most common HEV genotypes circulating in Thailand are scarce. The origin of infection and the mode of HEV transmission in this patient remained inconclusive; however, this study provides essential baseline data as a reference for further genetic analysis of HEV.

A 56-year-old man was referred to the neurological outpatients' department suffering from problems with walking and painful, burning feet after having been struck by lightning 6 months previously. He also experienced orthostatic symptoms and episodes of 'flushing' and was unable to tolerate contact with clothing or bed sheets on his lower legs or feet. After excluding other possible causes, the patient was diagnosed with 'polyneuropathy due to lightning strike'. Gabapentine had a favourable effect on the sensory symptoms. Lightening strikes are a rare cause of polyneuropathy.

In double-blind conditions, ST-679 (a single oral 1200 mg dose in 20 patients), or paracetamol (a single oral dose of 1000 mg in 20 patients) was administered to forty patients who presented pain as a result of obstetrical-gynecological surgery. The study proposed to evaluate the analgesic efficacy and tolerability of ST-679, a new non-steroid antiphlogistic drug, as compared to a reference drug amply utilized in clinical practice such as paracetamol.
