Synergistic Effect between the APOE ε4 Allele with Genetic Variants of GSK3B and MAPT: Differential Profile between Refractory Epilepsy and Alzheimer Disease.

Temporal Lobe Epilepsy (TLE) is a chronic neurological disorder characterized by recurrent focal seizures originating in the temporal lobe. Despite the variety of antiseizure drugs currently available to treat TLE, about 30% of cases continue to have seizures. The etiology of TLE is complex and multifactorial. Increasing evidence indicates that Alzheimer's disease (AD) and drug-resistant TLE present common pathological features that may induce hyperexcitability, especially aberrant hyperphosphorylation of tau protein. Genetic polymorphic variants located in genes of the microtubule-associated …

Multivariate analysis of seizure outcomes after resective surgery for focal epilepsy: a single-center study on 833 patients.

The predictors of seizure outcomes after resective surgery for focal epilepsy, for an update on the features of good and poor outcomes, are … ed epilepsy duration < 5 years, localized discharge, no. of antiepileptic drugs at surgery < 3, and temporal lobe resection. However, predictors of worse outcomes included intracranial hemorrhage in infancy, interictal abnormal discharge, intracranial electrode monitoring, and acute postoperative seizure. Our study suggests that resective surgery for focal epil …

Overlapping and unique brain responses to cognitive and response inhibition.

Although cognitive inhibition and response inhibition fall under the umbrella term of inhibition, the question remains whether the two aspec … sponse inhibition recruited a group of overlapping brain regions (inferior frontal cortex, inferior temporal lobe, precentral cortex, parietal cortex). However, a direct comparison of cognitive and response inhibition revealed that the two aspects of inhibition also engaged distinct, task-specific brain regions (voxel-wise FWE corrected p < 0.05). Cognitive inh …

Cerebral atrophy as a cause of aphasia: From Pick to the modern era.

In his epoch-making monograph, Wernicke (1874) claimed that atrophy of the brain cannot cause aphasia. Refuting this claim, Pick (1892, 1898, 1901, 1904a) documented in increasing detail several cases of aphasia with circumscribed atrophy of the left temporal lobe, frontal lobe, or both, which persuaded Wernicke (1906). To explain why the atrophy is circumscribed and leads to focal symptoms, Pick (1908a) advanced a functional network account. Behavioral, neuroanatomical, and histopathological studies by Dejerine and Sérieux, Fischer, Alzheimer, Altman, Gans, Onari and Spatz, and Stertz further…

Conceptual Combination in the LATL With and Without Syntactic Composition.

The relationship among syntactic, semantic, and conceptual processes in language comprehension is a central question to the neurobiology of language. Several studies have suggested that conceptual combination in particular can be localized to the left anterior temporal lobe (LATL), while syntactic processes are more often associated with the posterior temporal lobe or inferior frontal gyrus. However, LATL activity can also correlate with syntactic computations, particularly in narrative comprehension. Here we investigated the degree to which LATL conceptual combination is dependent on syntax, …

Neurosyphilis in disguise.

PURPOSE: Neurosyphilis can mimic different diseases, not only in its clinical presentation but also on imaging. Treponema pallidum is also known as the "great imitator." Having an ultimate diagnosis of neurosyphilis is quite critical as this can affect management drastically. Herein, we discuss the case of a 69-year-old female who was treated for neurosyphilis, while having an atypical imaging finding of anterior temporal lobe enhancement that simulated an infection with herpes simplex virus (HSV); we also review the available literature on different imaging findings in both the early and late…

Predicting Development of Alzheimer's Disease in Patients with Shunted Idiopathic Normal Pressure Hydrocephalus.

BACKGROUND: Idiopathic normal pressure hydrocephalus (iNPH) patients often develop Alzheimer's disease (AD) related brain pathology. Disease State Index (DSI) is a method to combine data from various sources for differential diagnosis and progression of neurodegenerative disorders. OBJECTIVE: To apply DSI to predict clinical AD in shunted iNPH-patients in a defined population. METHODS: 335 shunted iNPH-patients (median 74 years) were followed until death (n = 185) or 6/2015 (n = 150). DSI model (including symptom profile, onset age of NPH symptoms, atrophy of medial temporal lobe in CT/MRI, …

Cardiovascular symptoms and longitudinal declines in processing speed differentially predict cerebral white matter lesions in older adults.

It is well established that cerebral white matter lesions (WML), present in the majority of older adults, are associated with cardiovascular … burden was associated both with elevated cardiovascular symptoms and declines in processing speed, temporal lobe WML burden was only significantly associated with declines in processing speed. These latter outcomes suggest that age-related WML-cognition associations may be etiologically heterogeneous across fronto-temporal cerebral regions.

Delayed presentation of cerebral schistosomiasis presenting as a tumor-like brain lesion.

Schistosomiasis is the second most common parasitic infection worldwide. North America is a nonendemic area. However, there are occasional case reports among travelers and immigrants from endemic regions. We describe a case of a 55-year-old Canadian woman who presented with first episode of seizure. Her magnetic resonance imaging scan revealed a mass-like lesion involving the left anterior temporal lobe. The lesion showed T1 hypo- and T2 hyperintense with perilesional brain edema. On post-gadolinium-enhanced T1-weighted sequence, the lesion showed multiple small nodular and linear enhancements…

Dissociated signals in human dentate gyrus and CA3 predict different facets of recognition memory.

A wealth of evidence has implicated the hippocampus and surrounding medial temporal lobe cortices in support of recognition memory. However, the roles of the various subfields of the hippocampus are poorly understood. In this study, we concurrently varied stimulus familiarization and repetition to engage different facets of recognition memory. Using high-resolution fMRI (1.5 mm isotropic), we observed distinct familiarity and repetition-related recognition signal profiles in the dentate gyrus (DG)/CA3 subfield in human subjects. The DG/CA3 demonstrated robust response suppression with repetiti…

Spatial patterns of brain atrophy in MCI patients, identified via high-dimensional pattern classification, predict subsequent cognitive decline.

Spatial patterns of brain atrophy in mild cognitive impairment (MCI) and Alzheimer's disease (AD) were measured via methods of computational neuroanatomy. These patterns were spatially complex and involved many brain regions. In addition to the hippocampus and the medial temporal lobe gray matter, a number of other regions displayed significant atrophy, including orbitofrontal and medial-prefrontal grey matter, cingulate (mainly posterior), insula, uncus, and temporal lobe white matter. Approximately 2/3 of the MCI group presented patterns of atrophy that overlapped with AD, whereas the remain…

Bilateral deep discharges involving the hippocampal-mesial temporal lobe regions may explain EAA, which is probably a postictal phenomenon. The interictal memory problems may be due to subclinical discharges causing difficulty in codification or consolidation of the amnesic trace. EAS patients having uniform anamnestic, …
