We discuss the use of magnetic resonance imaging of the pelvis in a patient with the UroLume urethral stent. This case demonstrates that magnetic resonance imaging is a safe and effective modality for imaging tissue surrounding the stent.

PURPOSE: We describe glanuloplasty and in situ tubularization of the urethral plate, and report indications, results and complications.
MATERIALS AND METHODS: We evaluated our surgical results using glanuloplasty and in situ tubularization of the urethral plate in 166 boys 5 months to 13.5 years old (mean age 20 months).

Intraurethral collagen injection has become a recognized modality to treat stress urinary incontinence. Transrectal ultrasound was used to assess anatomic support of the urethrovesical junction (UVJ) in continent and stress incontinent women. UVJ drop on straining of less than 1 cm as assessed by transrectal ultrasound correlated well with good support to the UVJ. Drop of UVJ of more than 1 cm on straining correlated with poor support to bladder neck and stress urinary incontinence. The transrectal technique is quick and easy to perform and interpret.

Primary closure of classical exstrophy was performed with paraexstrophy skin flaps for urethral lengthening in 4 female neonates. In all cases more than 2.0 cm. of urethral length were achieved. Efficacy of bladder closure and subsequent bladder neck revision were enhanced by the use of these flaps. The technique of urethral construction is described and our results are discussed.

To straighten the penis in truncal, scrotal and perineal forms of hypospadias is expedient to use the proposed method. Fixation of the penis should be performed with the help of a correcting splint. An analysis of results of urethroplasty by different methods has shown an indisputable advantage of the Landerer method modified by Rusakov.

Perirenal urinomas may develop when obstruction to urinary flow creates sufficient back pressure to produce extravasation of urine. Such fluid collections have been described as mainly sonolucent. However, in the last 2 years, we have imaged five neonates with multiple, marked septations of the urinoma, four with posterior urethral valves and one with a ureteropelvic junction obstruction. Diagnostic confusion may arise when the kidney is not identified, when the septated nature of the urinoma is not recognized, and when urinary tract obstruction is not seen.

Epidermolysis bullosa is a rare skin disorder that may at times involve the respiratory, gastrointestinal and genitourinary systems. We report 3 cases of epidermolysis bullosa involving the urinary tract. The radiographic evaluation of this entity and its relationship to other forms of obstructive uropathy are discussed along with a management plan for patients with genitourinary involvement owing to epidermolysis bullosa.

We present a new method for a one-stage phalloplasty. In forming the penile body and its rigid supporting tissue, a composite flap, including the lateral groin skin flap (11 cm long and 10 cm wide) and the iliac crest bone in its entire length, is designed based on the superficial circumflex iliac artery and its accompanying veins. A new technique for the treatment of female stress urinary incontinence is described. It consists of colposuspension obtained by suprapubic placement of two double monofilament sutures in a spiral fashion on the lateral vaginal walls, which are fixed proximally to a Cooper's ligament and tied distally one to the other over the rectus fascia. This technique has been successfully employed in a preliminary series of 26 patients and it appears to be easy to perform and with a wide application range.

A reliable axial flap from the adductor region has been used for many years to repair a series of urethral defects formerly regarded as difficult problems. In particular the flap has proved invaluable in the treatment of epispadias "cripples". Since Duckett's introduction of hypospadias repair by meatoplasty and glanuloplasty we have seen a rekindled interest in the correction of the subcoronal hypospadias. However, we have not found this procedure to be widely applicable or the cosmetic results to be universally acceptable. We describe a repair for distal hypospadias that incorporates a more vigorous vertical glans incision plus more aggressive distal urethral mobilization. Reconstruction has been successful in all 28 patients who have undergone this procedure.
