Improved control of asthma in the office setting. A large-scale study of once-daily evening doses of theophylline.

A large-scale, multi-investigator open evaluation compared a once-daily regimen of controlled-release theophylline (Uniphyl tablets) with previous twice- or thrice-daily methylxanthine regimens. Three hundred asthmatic patients, 78 percent prone to nocturnal episodes during prior therapy, completed the investigation. Eighty-two percent of the patients were treated for moderate or severe disease. After a one-week evaluation of baseline theophylline therapy (with adjunctive medication), the patients substituted evening doses of the once-daily drug in approximate milligram-for-milligram equivalen

Effects of phosphatidylcholine on the topical bioavailability of corticosteroids assessed by the human skin blanching assay.

A non-occluded multiple application skin blanching assay has been used to determine the effect of applied phosphatidylcholine (PC) on the bioavailability of corticosteroids. One forearm of each of ten volunteers was treated twice daily for one week with PC presented as a liposomal suspension in Sørensen's (pH 5.0) phosphate buffer (2.5 mg PC/0.5 mL) while the other arm was treated with 0.5 mL of phosphate buffer. For the following two weeks this treatment regimen was continued but in addition, each of four corticosteroid formulations (containing (i) hydrocortisone 0.1%, (ii) clobetasone butyra

Topical minoxidil. A survey of use and complications.

To obtain data on clinicians' experiences with topical minoxidil, we performed a mail survey of one in six members of the American Academy of Dermatology. Sixty-one percent of this random sample of dermatologists responded. More than 70% of these reported they had prescribed topical minoxidil for hair loss. The frequency of prescribing varied substantially among dermatologists, with less than 10% of respondents being responsible for over half the reported prescriptions. Only 2% of prescribers reported complications other than local irritation that they associated with topical minoxidil therapy

Use of topical minoxidil as a possible adjunct to hair transplant surgery. A pilot study.

Twelve patients, aged 21 to 60 years, with varying Hamilton classifications of androgenetic alopecia (male pattern baldness) were treated with a 3% solution of topical minoxidil. Therapy began 48 to 72 hours after hair transplant surgery. Two patients demonstrated hair growth in the grafts without the shedding that usually occurs 2 to 4 weeks after surgery. (In untreated patients after hair transplant surgery, regrowth begins 3 to 5 months after surgery, after the shedding period.) A review of the literature--and personal discussions with surgeons whose hair transplant experience spans 25 year

Histopathologic changes in condylomata acuminata after application of podophyllum.

The histologic changes of podophyllin-treated condylomata acuminata were studied at 24-hour, 48-hour, and 7-day intervals. The 24-hour specimens had scattered individual atypical cells, whereas 48-hour specimens demonstrated small focal areas of atypia. Seven-day specimens exhibited very mild atypia of cells. All treated specimens had inflammation-necrosis ranging from scattered inflammatory cells to degeneration of the squamous epithelium.

Effect of topically applied flurbiprofen on ultraviolet-induced erythema.

The effect of flurbiprofen, a nonsteroidal antiinflammatory agent, on ultraviolet B-induced erythema was studied in normal volunteers. The effect of various concentrations as well as the effect of multiple applications were evaluated at 4, 8, and 24 hours after irradiation with three MEDs of ultraviolet B. Repeated applications of flurbiprofen during the four- and eight-hour periods following ultraviolet B exposure did not increase the blanching response obtained following a single treatment. A concentration dependent effect of flurbiprofen on blanching was observed with suppression of erythem

Dermatophytosis. A review of diagnosis and current therapy.

Inhibition of fungal growth and accelerated keratolysis are the necessary ingredients of dermatophyte therapy. Often, the primary fungus is destroyed by secondary bacterial invasion or the body's immune response. Clinicians must recognize this possibility and, in such instances, treat the bacterial infection or immune response rather than the suspected dermatophyte.

A phase I trial of topically applied trans-retinoic acid in cervical dysplasia-clinical efficacy.

Forty-two patients were entered into a phase I trial to evaluate the vitamin A derivative, trans-retinoic acid, in cervical intraepithelial neoplasia. Treatment consisted of four consecutive 24-h applications of retinoids via an inert collagen sponge in a cervical cap. Patients were followed for response at 3-month intervals using cytology, colposcopy, and selected biopsies. Thirty-six patients were evaluable (mild dysplasia, 13; moderate dysplasia, 17; severe dysplasia, 6) with follow-up from 5 to 18 months. Complete regression was seen in 2/14 (14%) patients treated with concentrations of 0.

Randomized trial of oral naproxen or local injection of betamethasone in lateral epicondylitis of the humerus.

A randomized pilot study, comparing oral naproxen and a single betamethasone injection, was carried out in 21 patients suffering from lateral epicondylitis of the humerus ("tennis elbow"). The naproxen dosage was 250 mg per day for two weeks. Six milligrams of betamethasone in a long-acting form was given as a local injection. To achieve "blindness," the patients receiving naproxen were also given an injection of saline into the area of maximal tenderness at the epicondyle, while the patients getting the betamethasone-injection were given oral placebo tablets. At a clinical control after two w

Lymphomatoid papulosis associated with plaque-stage and granulomatous mycosis fungoides.

A 47-year-old black man had typical papulonodular lesions of lymphomatoid papulosis (LyP) with concurrent plaque-stage mycosis fungoides (MF). Both diagnoses were confirmed histologically. This supports the concept that LyP is part of the spectrum of cutaneous T-cell lymphoproliferative disorders. The patient also had a large nodule and a deeply infiltrated plaque, each of which exhibited a deep granulomatous reaction. These were interpreted as representing granulomatous MF. Clinically, there was an evolution from a predominance of LyP lesions to a predominance of MF plaques. Topical carmustin

Topical carmustine therapy for lymphomatoid papulosis.

Seven patients with lymphomatoid papulosis were treated with solutions of topical carmustine, a nitrosourea compound. Recently used schedules have employed 10 mg of carmustine in dilute alcohol applied to the total skin surface daily for four to 17 weeks (total dosage, 280 to 1,180 mg). All patients experienced a rapid reduction in the number and size of lesions. Maintenance therapy consisted of local applications of carmustine (2 to 4 mg/mL of 95% ethanol) to individual new papules. This method was effective in suppressing disease activity and reduced by half the average life cycle of individ

Comparison of the immediate effects on the sub-gingival microflora of acrylic strips containing 40% chlorhexidine, metronidazole or tetracycline.

In the management of chronic periodontitis, there has been a renewed interest in the local delivery of antimicrobial drugs into periodontal pockets. This study assessed the effects of the acrylic strip delivery system containing chlorhexidine, metronidazole or tetracycline on subgingival microflora assessed by dark field microscopy. Strips containing 40% chlorhexidine, metronidazole or tetracycline were placed for 2 to 3 days into pockets greater than 6 mm which bled on probing. Plaque samples were obtained before and after treatment and counts of morphological and motile groups of organisms w
