AIMS AND METHODS: To assess the clinical features of septic arthritis and characterize therapeutic strategies and outcome in a prospective study of 75 patients selected by positive synovial fluid culture.

Four laboratory-grown, low-passage isolates of Borrelia burgdorferi sensu stricto, B31, JD-1, 910255, and N40, were incorporated into Ixodes scapularis ticks to examine the pathogenesis of these isolates in mice after tick transmission. Septic arthritis commonly occurs in the rheumatoid arthritis population. The diagnosis is frequently delayed and the associated mortality is high. Between 1950 and 1991 the authors have treated 45 adults with primary septic arthritis of the hip, not including tuberculosis. The mortality rate was 13%. There is an accompanying septicaemia, or, more commonly, a bacteraemia, and other general and local factors are often present. The diagnosis may be obvious, but in 50% of cases the presentation may be atypical and the clinical diagnosis uncertain. UNLABELLED: Based on the results of a retrospective study of 67 patients who had diagnostic and surgical arthroscopy of the elbow from 1977 until 1990, we present the technique, results, complications, and indications for elbow arthroscopy. The average age of the patients was 26 years (range: 11-59). At follow-up, the patients were examined both clinically and radiologically. A method is described which provides standardised reproducible radiographic images of the lower limb. Anteroposterior and lateral radiographs are digitised and processed by computer to provide graphic/numeric displays of angles and linear measurements, relating the centre points of the hip, knee, and ankle. Two cases illustrate how surgical planning is facilitated when standardised data are available. These data confirm the close relationship between postoperative limb alignment and positioning of prosthetic elements.

Osteonecrosis is not well documented as a predisposing factor of septic arthritis despite such a relationship having obvious clinical significance. We report 4 patients with involvement of 5 hips with septic arthritis in established osteonecrotic joints. The etiologies of the osteonecrosis in our study included corticosteroid therapy, sickle cell disease and one case of idiopathic osteonecrosis. Osteonecrosis appears to render the hip more susceptible to hematogenously derived bacterial infection. In some cases, removal of the necrotic tissue may be necessary to cure the infection.

Lyme disease is capable of producing a wide variety of clinical pathologic conditions and lesions having in common histologic features of collagen-vascular disease. The symphysis pubis is a nonsynovial amphiarthrodial joint that is situated at the confluence of the two pubic bones. A thick intrapubic fibrocartilaginous disc is sandwiched between thin layers of hyaline cartilage. The inferior pubic ligament provides most of the joint's stability. Anatomic sections demonstrate a symphysis by the end of the second month of gestation. Thick cartilaginous end-plates are present at birth but become thin by the time of skeletal maturity. Congenital diseases resulting in failure of symphysis formation include exstrophy of the bladder and cleidocranial dysostosis.

Twelve patients were treated with high-dose intravenous penicillin for neurologic abnormalities of Lyme disease. Headache, stiff neck, and radicular pain usually began to subside by the second day of therapy and were often gone by 7 to 10 days. Nongonococcal septic arthritis, particularly if caused by Staphylococcus aureus or Gram-negative bacilli, presents major dilemmas to the primary care physician. Patients with this disorder require prolonged regimens of parenteral antibiotics in high doses and intensive serial radiographic investigation for coexistent osteomyelitis. For patients with pyoarthrosis caused by Gram-negative bacilli, use of combination synergistic chemotherapy is recommended.
