Trapezium nonunion: A rare case report.

Trapezium fracture is a rare clinical situation that several authors have described through clinical case reports. Nevertheless, there is currently no consensus on the best treatment. We report a case of trapezium nonunion, a situation which, to our knowledge, has never been described before.

Biomechanical comparison of modified Calcanail system with plating fixation in intra-articular calcaneal fracture: A finite element analysis.

The calcanail system is a novel intramedullary approach for calcaneal fractures but is believed to be insufficient to treat complex fractures. We propose a modified Calcanail technique by adding a transfixation screw to improve stability. The aim of this study was to evaluate the biomechanical stability of the modified Calcanail system and compare it with the traditional Calcanail system and plate fixation. A Sanders type-IIIAB calcaneal fracture model was built and simulated fixation with the three implants. A vertical loading of 700 N was applied to the subtalar joint surfaces, and the poste

Which additional augmented fixation procedure decreases surgical failure after proximal humeral fracture with medial comminution: fibular allograft or inferomedial screws?

BACKGROUND: The purpose of this study was to compare clinical and radiologic results between treatment with locking plate fixation and fibular allograft augmentation (FA) and treatment with locking plate fixation and additional inferomedial screws (IMSs) in 3- or 4-part proximal humeral fractures with medial comminution in geriatric patients. METHODS: We enrolled 164 patients with 3- or 4-part proximal humeral fractures with medial comminution who were treated with locking plates. The patients were divided into 2 groups according to additional augmented fixation procedures. The IMS group was

Increased wound complication with intramedullary screw fixation of clavicle fractures: Is it thermal necrosis?

PURPOSE: Adult mid-shaft clavicle fractures are common injuries. For displaced fractures, open reduction with plate or intramedullary (IM) fixation is the widely used techniques. All methods have their own potential drawbacks, especially related to local soft tissue complications. There is little information about outcome and management of local wound complications after clavicle fracture fixations. METHODS: Ninety-seven patients underwent open reduction and internal fixation, 17 were treated with IM screw fixation and 80 with plate fixation. Wound complication occurred in eight patients (8.2

Current concepts for the treatment of acute scaphoid fractures.

Fractures of the scaphoid are common injuries, accounting for approximately 80 % of carpal fractures. Differentiation between stable and unstable fractures (Herbert classification) cannot be made with conventional X-rays, so evaluation by computed tomography should additionally be performed. Under most circumstances, minimally invasive surgery with cannulated screws is the treatment of choice. A longer cast immobilization after minimal-invasive surgery is not necessary. Conservative treatment still has a place if the fracture is not dislocated nor unstable, but operative treatment can be offer

Beneficial effects of turnover orbicularis oculi muscle suspension flap for treating facial fractures via subciliary incision.

In patients with moderate lower lid laxity, the lower orbicularis oculi muscle becomes atonic or ptotic. Hence, in such patient populations, with periorbital fractures, additional vertical support endorsement either by lateral canthopexy or orbicularis oculi muscle suspension flap must accompany plate and screw fixations. In this report, we shared our experience in applying prophylactic suspension to the lower lid with turnover orbicularis oculi transposition muscle flap in zygomatic fractures treated by subciliary approach in 98 patients. Our results show that turnover orbicularis oculi muscl

Fracture site augmentation with calcium phosphate cement reduces screw penetration after open reduction-internal fixation of proximal humeral fractures.

BACKGROUND: We sought to examine fracture settling and screw penetration after open reduction-internal fixation of 2-, 3-, and 4-part proximal humeral fractures and determine whether the use of calcium phosphate cement reduced these unwanted complications. METHODS: We performed a retrospective study of prospective data. Inclusion criteria included patient age of 18 years or older and an acute traumatic fracture of the proximal humerus that was treated with open reduction-internal fixation with a locked plate. Metaphyseal defects were treated with 1 of 3 strategies: no augmentation, augmentati

Biomechanical evaluation of a locking and nonlocking reconstruction plate in an osteoporotic calcaneal fracture model.

BACKGROUND: This study evaluated the effects of locking vs nonlocking configuration on the biomechanical performance of a calcaneal reconstruction plate in an osteoporotic cadaveric model. MATERIALS AND METHODS: A Saunders II B type calcaneal fracture was created in ten matched pair of cadaveric calcanei. Each pair was fixed with the Ascension calcaneal reconstruction plate using either locking or nonlocking screws in the same hole pattern. Specimens were axially loaded for 1000 cycles through the talus followed by load to failure. Statistical comparisons were made between the locking and non

The short-term outcome of the modified Sauvé-Kapandji procedure regarding range of motion, carpal bone translation and bony shelf size.

The Sauvé-Kapandji (S-K) procedure is a common treatment for rheumatoid wrists, but in some cases severe bone destruction makes this operative modality difficult to perform, while also resulting in a poor outcome. A modified S-K procedure for these wrists has been reported, but the clinical outcomes of the modified procedure are unclear. This study evaluated 24 wrists in 20 patients who underwent the modified S-K procedure. The mean follow-up period was 34.5 months. The clinical assessments were range of motion, carpal bone translation and bony shelf size. The range of motion and carpal bone t

The percutaneous antegrade scaphoid fracture fixation--a safe method?

INTRODUCTION: The object of this study was to assess the risk of injury to tendons, nerves and vessels in percutaneous antegrade scaphoid fracture fixation. METHODS: Forty cadaveric forearms were used in this study. A guide wire for cannulated headless compression screws was inserted percutaneously in each scaphoid according to established surgical technique. RESULTS: No nerve or vessel injuries were observed. Tendons however were injured in 5 out of the 40 specimens. This included the extensor pollicis longus tendon in two specimens, the extensor carpi radialis tendon in two specimens and t

[Osteotomies of the distal tibia and hindfoot for ankle realignment].

Asymmetric load of the ankle joint often results in degenerative disease. Although reconstructive surgery, including osteotomies above and beneath the tibiotalar joint, is possible, there are little data with respect to its evidence. This article presents general considerations for osteotomies around the osteoarthritic ankle and elaborates a rationale for the technical procedure. Additional measures for correcting the deformed and malaligned hindfoot are also elucidated. As a principle, opening-wedge and closing-wedge osteotomies are possible in one or more planes. In some instances, inframall

Sub-trochanteric fractures of the femur: results of ORIF at KMCTH.

Sub-trochanteric fractures of the femur are not commonly encountered as compared to trochanteric or neck of femur fractures. Relatively younger persons are injured more and a considerable violence is required for this fracture to occur. These fractures, treated conservatively by methods like traction, splints or casts, are often associated with complications like non-union, mal-union, shortening of the limb etc. Thus, we undertook this study of 10 consecutive cases of sub-trochanteric fractures of the femur treated by open reduction and internal fixation and bone grafting in Kathmandu Medical

Ultrasound-guided percutaneous scaphoid pinning: operator variability and comparison with traditional fluoroscopic procedure.

This paper reports on pilot laboratory experiments with a recently proposed surgical procedure for percutaneous screw insertion into fractured scaphoid bones using ultrasound guidance. The experiments were intended to determine the operator variability of the procedure and its performance in comparison with a traditional pinning procedure using fluoroscopy. In the proposed procedure, a three-dimensional surface model is created from pre-operative computed tomography images and intra-operatively registered to the patient using ultrasound images. A graphical interface that communicates with an o

[Actual relevance of minimal invasive surgery in fracture treatment].

Minimal invasive osteosynthesis (MIO) should belong nowadays in the armentarium of each trauma surgeon. The tendency to minimize the invasivity of every operation is a logical development considering the goal of each surgeon to reduce the iatrogenic damage caused by the operation. The term 'MIO" stands for this criteria: Fracture zone not opened, reduction by indirect manoeuvres or percutaneously by joy-sticks, small approaches for the application of the implants, intraoperative assessment of the reduction by imaging (fluoroscopy, arthroscopy, endoscopy, etc.). Long time before the definition

Use of a distraction plate for distal radial fractures with metaphyseal and diaphyseal comminution.

BACKGROUND: Distal radial fractures with extensive comminution involving the metaphyseal-diaphyseal junction present a major treatment dilemma. Of particular difficulty are those fractures involving the articular surface. One approach is to apply a dorsal 3.5-mm plate extra-articularly from the radius to the third metacarpal, stabilizing the diaphysis and maintaining distraction across the radiocarpal joint. METHODS: Twenty-two patients treated with a distraction plate for a comminuted distal radial fracture were included in the study. With use of three limited incisions, a 3.5-mm ASIF plate
