Ventral Cervical Spinal Schwannoma Presenting as Progressive Myelopathy.

PRESENTATION: A male patient in his 20s presented with a two-year history of progressive paraparesis and new-onset urinary incontinence. Neurological examination revealed bilateral ankle clonus, reduced lower-limb power, spastic gait and a positive Hoffmann's sign. DIAGNOSIS: MRI of the cervical spine demonstrated an intradural extramedullary lesion extending from C4 to C5-C6 disc level, causing severe cord compression. Histopathology confirmed schwannoma. TREATMENT: The patient underwent surgical resection via anterior cervical corpectomy of C4 and C5. Postoperatively, his lower-limb streng

Interactive associations of sagittal and vertical craniofacial skeletal patterns with upper cervical spine morphology and posture in growing patients.

OBJECTIVES: To investigate the independent and interactive associations of sagittal and vertical craniofacial skeletal dimensions with upper cervical spine posture and morphology in growing patients. MATERIALS AND METHODS: Pretreatment lateral cephalograms of 726 children aged 7-14 years were retrospectively analyzed. Sagittal (ANB) and vertical (SN-MP) skeletal dimensions were treated as continuous variables. General linear models were constructed to evaluate their associations with cervical spine parameters while adjusting for sex and skeletal maturation. Significant ANB × SN-MP interaction

Association Between Cervical Range of Motion and Swallowing Speed in Older Women After Total Hip Arthroplasty: A Cross-Sectional Study.

BACKGROUND AND PURPOSE: Total hip arthroplasty (THA) may induce acute alterations in spinopelvic alignment and compel the cervical spine to compensate for postural balance. Although these changes may biomechanically affect swallowing speed, the relationship between cervical range of motion (ROM) and swallowing speed in older adults after THA remains unclear. Therefore, this study aimed to investigate the relationship between cervical ROM and swallowing speed in older women after THA. METHODS: This cross-sectional study included 60 older women (mean age 71.6 ± 5.5 years) without clinically sig

Effect of muscle deactivation in the kinematic responses of cervical spine under emergency ejection - Finite element simulation.

The cervical spine is highly vulnerable to injury during high-acceleration emergency ejection because of its flexibility, head-torso inertial lag, and dependence on coordinated muscle function. This study investigated the biomechanical effects of selective neck muscle deactivation on cervical spine kinematics under vertical acceleration loading using a validated C0-T1 finite element model based on a Hill-type muscle model. A 10-G vertical acceleration was applied, and individual muscle deactivation was simulated by reducing muscle activation to 0.005 at 80 ms. The results showed all muscular e

Treatment of iatrogenic vertebral artery injury during C1-C2 arthrodesis with a covered stent.

Iatrogenic vertebral artery injury (VAI) has been described in the context of both anterior and posterior approaches to the cervical spine.1 2 Endovascular treatment of VAI primarily entails vessel sacrifice or vascular reconstruction. The latter option offers the advantage of achieving hemostasis while maintaining parent vessel patency.3 In this video (video 1) we describe the case of a patient in their 20s undergoing C1-C2 arthrodesis with Harms technique. A V3 segment VAI was successfully treated with two overlapping covered stents which provided hemostasis and parent vessel reconstruction.

Radiograph-based intervertebral sagittal translation-to-canal diameter ratio as a quantitative imaging marker for suspected lower cervical instability.

OBJECTIVE: To compare radiographic differences between clinically suspected symptomatic lower cervical instability segments and asymptomatic normal segments and to evaluate the value of the radiograph-based intervertebral sagittal translation-to-canal diameter ratio (TCDR) as a quantitative imaging marker for lower cervical instability. METHODS: This single-center retrospective study screened 197 clinically suspected cases of lower cervical instability. Seventeen cases with multilevel abnormalities but without a clearly identifiable clinically dominant responsible segment were excluded, leavi

C2 Slope as an Independent Predictor of Cervical Lordosis Loss Following Laminoplasty.

OBJECTIVE: Postoperative loss of cervical lordosis remains a common and clinically relevant complication following laminoplasty, negatively affecting neck pain, neurological recovery, and long-term sagittal balance. However, reliable and easily applicable preoperative predictors for identifying patients at high risk of cervical lordosis deterioration remain limited. This study aims to investigate whether preoperative C2 slope (C2S) independently predicts cervical lordosis deterioration following laminoplasty. METHODS: This retrospective cohort study included 179 patients who underwent cervica

A call to arms: the absence of standardized return to play guidelines for professional rugby union players after cervical spine surgery.

Cervical spine injuries in elite rugby union, while infrequent, carry a disproportionately high risk for players. The nature of collisions in rugby creates unique biomechanical stresses on the cervical spine, heightening the risk of catastrophic injury. Injuries vary across a spectrum from transient neuropraxia ('stingers') to structural injuries requiring surgical intervention. Despite the gravity of these injuries, there remains no standardized, evidence-based return to play (RTP) protocol for rugby players after cervical spine surgery. A thorough literature review was undertaken to systemat

Technical evolution and short-term outcomes of minimally invasive versus conventional open anterior cervical discectomy and fusion: a systematic review and meta-analysis.

BACKGROUND: Conventional open anterior cervical discectomy and fusion (ACDF) is an established treatment for degenerative cervical disease, but approach-related morbidity remains a concern. Minimally invasive anterior cervical techniques have evolved from microscope-assisted mini-open procedures to endoscope-assisted and selected full-endoscopic approaches. This study systematically reviewed the technical evolution of minimally invasive ACDF-related procedures and compared perioperative and short-term outcomes with conventional open ACDF. METHODS: This systematic review and meta-analysis was

Evaluation of a novel C2 pedicle screw insertion technique: a retrospective comparative clinical study and finite element analysis.

C2 pedicle screws (C2PS) are extensively used in the treatment of upper cervical spine disorders owing to their superior mechanical properties. However, incorrect screw insertion in the C2 pedicle can lead to catastrophic consequences due to the proximity to critical structures such as the vertebral artery groove and spinal cord. There is currently no universally accepted method for precise C2PS placement. In this study, we evaluated a novel free-hand C2PS insertion technique that uses the superior margin of the C2 lamina and the medial margin of the C2 pedicle as landmarks, thereby avoiding r

Is a central cavity necessary for bioactive glass-ceramic spacers in plated ACDF? A retrospective comparison of solid versus cavity designs.

PURPOSE: Traditionally, interbody cages feature a central cavity for graft packing. However, for bioactive glass-ceramic (BGS-7) spacers achieving fusion via surface-mediated osseointegration, this cavity may be biologically redundant and structurally disadvantageous. We compared radiological and clinical outcomes of plated anterior cervical discectomy and fusion (ACDF) using BGS-7 spacers with or without a central cavity. METHODS: We retrospectively reviewed patients undergoing plated ACDF using solid non-cavity (17 patients, 24 levels) or central-cavity (17 patients, 33 levels) BGS-7 spacer

Cervical implant fixation: a topical review of techniques and their importance.

Cervical implant fixation is a critical surgical intervention for stabilizing the cervical spine, often necessitated by trauma, degenerative diseases, or spinal deformities. While spinal disc disease has historically been treated with fusion-based procedures, there has been a recent surge of interest in motion-preserving disc arthroplasties. The present study provides a topical narrative review of selected and recent literature on cervical implant fixation techniques, including anterior and posterior approaches, implant materials, biomechanical considerations, and reported clinical outcomes. A Fresh Look at Post-ACDF Dysphagia and SLP Role in Acute Management.

Anterior cervical discectomy and fusion (ACDF) is a commonly performed procedure for the treatment of cervical spine pathologies, such as disc disorders and myelopathy. Patients with cervical spine disorders suffer neck pain, numbness, and in severe cases, weakness of the extremities. When patients fail conservative management, surgical treatment can be beneficial. An anterior approach to the cervical spine is preferential due to relatively low complication rates; however, dysphagia is a common and troublesome complication of this approach. The pathophysiology of dysphagia is multifactorial an
