Cancer screening with digital mammography for women at average risk for breast cancer, magnetic resonance imaging (MRI) for women at high risk: an evidence-based analysis.

OBJECTIVE: The purpose of this review is to determine the effectiveness of 2 separate modalities, digital mammography (DM) and magnetic resonance imaging (MRI), relative to film mammography (FM), in the screening of women asymptomatic for breast cancer. A third analysis assesses the effectiveness and safety of the combination of MRI plus mammography (MRI plus FM) in screening of women at high risk. An economic analysis was also conducted. RESEARCH QUESTIONS: How does the sensitivity and specificity of DM compare to FM?How does the sensitivity and specificity of MRI compare to FM?How do the re

Feasibility and impact of a mental health chatbot on postpartum mental health: a randomized controlled trial.

BACKGROUND: Perinatal mood disorders are common yet underdiagnosed and un- or undertreated. Barriers exist to accessing perinatal mental health services, including limited availability, time, and cost. Automated conversational agents (chatbots) can deliver evidence-based cognitive behavioral therapy content through text message-based conversations and reduce depression and anxiety symptoms in select populations. Such digital mental health technologies are poised to overcome barriers to mental health care access but need to be evaluated for efficacy, as well as for preliminary feasibility and a

Recent advances in the detection and management of motor dysfunction in Alzheimer's disease.

Alzheimer's disease (AD) is a progressive neurodegenerative disorder primarily characterized by cognitive decline. However, there is growing recognition of the significant impact of motor dysfunction in individuals affected by AD. These motor impairments contribute substantially to functional decline, reduced quality of life, and increased caregiver burden in AD patients.1 Current research efforts are increasingly focused on identifying motor dysfunction as a potential early marker in the progression of AD. The temporal relationship between motor and cognitive decline is under intense investig

Computationally efficient x-ray simulation framework using parameterized material attenuation models in anatomically detailed imaging.

BACKGROUND: Virtual clinical trials provide an efficient alternative to clinical imaging trials for evaluating imaging technologies. In x-ray simulations, however, modeling material-specific attenuation becomes computationally intensive as anatomical complexity and material heterogeneity in digital phantoms increase. Parameterization models offer a potential solution by representing material properties with a compact set of coefficients. PURPOSE: To develop and validate an x-ray simulation framework that models material attenuation using parameterization models, reducing computational cost wh

Prior information-based subtraction radiography for enhanced tumor visibility in image-guided radiotherapy.

PURPOSE: Two-dimensional (2D) X-ray imaging is routinely used in radiotherapy for patient alignment but offers limited tumor visibility due to anatomical overlap. We propose a novel technique, prior information-based subtraction (PIBS) radiography, to enhance tumor visualization on 2D X-ray and facilitate more accurate patient alignment. MATERIALS AND METHODS: PIBS X-ray improves tumor visualization on 2D X-ray by subtracting signal contributions from non-essential (NE) tissues, which can be obtained from the planning CT (prior information). To demonstrate the feasibility, we acquired four CT

Smart Devices and Multimodal Systems for Mental Health Monitoring: From Theory to Application.

Smart devices and multimodal biosignal systems, including electroencephalography (EEG/MEG), ECG-derived heart rate variability (HRV), and electromyography (EMG), increasingly supported by artificial intelligence (AI), are being explored to improve the assessment and longitudinal monitoring of mental health conditions. Despite rapid growth, the available evidence remains heterogeneous, and clinical translation is limited by variability in acquisition protocols, analytical pipelines, and validation quality. This systematic review synthesizes current applications, signal-processing approaches, an

Video-Based Motion Capture Smartphone Apps for Testing Human Motor Performance Skills: Scoping Review.

BACKGROUND: Good motor performance skills (MPS) are relevant in all stages of life. Higher MPS are associated with enhanced cognitive abilities and physical and mental health. The assessment of MPS is important to identify deficits in MPS at an early stage and to implement interventions to address these deficits. One method to assess MPS is through marker-based motion capture in a laboratory setting with multiple cameras. However, this approach is expensive and time-consuming, making it impractical, for example, in large-scale studies for MPS assessment. Recent advancements (eg, artificial int

The ADAPT model: integrating employer insights into a framework for practice-ready health professions graduates.

INTRODUCTION: Healthcare systems rely on a skilled and diverse workforce to deliver patient-centred care across settings. Rising patient acuity, rapid technological change, and interprofessional collaboration have heightened expectations for graduates to be "practice-ready" on entry to the workforce. Health professions education (HPE) must therefore extend beyond discipline-specific knowledge to prepare graduates for real-world practice. Employers, as key stakeholders, are uniquely positioned to assess this readiness, yet their perspectives are seldom explored. This study explores employer vie

Application of Quality by Design (QbD) as Artificial Intelligence in Formulation for Diabetes: A Practical Review.

INTRODUCTION: The increasing incidence of diabetes has made it essential to create more efficient, customized, and reproducible drug delivery systems. Quality by Design (QbD) is viewed as a science-driven, disciplined approach to drug development that relies more on ensuring consistent product quality through the determination of the Critical Quality Attributes (CQA), Critical Process Parameters (CPP), and development of a Design Space. Nonetheless, the sophistication of contemporary drugs and volumes of data involved tend to confine the independent effectiveness of QbD. Drug development has b

Cost-effectiveness of three screening strategies for atrial fibrillation in Sri Lanka: a decision-tree modelling analysis using community-based prevalence data.

INTRODUCTION: Early diagnosis and treatment of atrial fibrillation (AF) are crucial to reduce AF-related complications and associated healthcare costs. In low-resource settings, digital health technologies could help achieve this; however, costs of different screening strategies are key for policy change. METHODS: This decision-tree model representing the Sri Lankan public health system perspective used prevalence data from a community-based cross-sectional study of 10 000 individuals aged ≥50 years in Northern Province, Sri Lanka. Participants were screened for AF using AliveCor, a handheld

Modifiable lifestyle factors and sudden cardiac death: mechanisms, evidence, and prevention strategies.

Sudden cardiac death (SCD) accounts for an estimated 4-5 million deaths annually worldwide and remains a major unresolved challenge in cardiovascular medicine. Survival from out-of-hospital cardiac arrest remains below 10% in most regions, and approximately half of SCD events occur as the first clinical manifestation of cardiovascular disease, often in individuals not identified by conventional risk stratification. We conducted a narrative review of the literature in PubMed/MEDLINE, Web of Science, and EMBASE (January 2003 to April 2026), prioritizing prospective cohorts, randomized trials, me

Beyond Blood Pressure: Salt Sensitivity as a Cardiorenal Phenotype-A Narrative Review.

Background: Salt-sensitive blood pressure (SSBP) represents a prevalent yet underrecognized hypertensive phenotype, in which blood pressure (BP) and volume status are disproportionately influenced by dietary sodium intake. Beyond BP elevation alone, salt sensitivity reflects a convergence of renal sodium handling abnormalities, neurohormonal activation, vascular dysfunction, and inflammatory pathways that link excessive sodium exposure to progressive kidney injury and adverse cardiac remodeling. Given its association with chronic kidney disease (CKD) and the association of heart failure with p

Development and validation of a duplex droplet digital PCR assay for the simultaneous detection of cytomegalovirus and Epstein-Barr virus in plasma.

BACKGROUND: Human cytomegalovirus (CMV) and Epstein-Barr virus (EBV) are globally prevalent herpesviruses. While typically self-limiting in immunocompetent individuals, infections can cause severe consequences even in this population. In immunocompromised groups, such as transplant recipients and HIV-infected individuals, viral reactivation or coinfection frequently triggers graft rejection, multi-organ invasion, and malignancies, often exhibiting synergistic pathogenicity. Current serological assays are limited by "window periods" and delayed immune responses, while traditional quantitative P

Social, Digital and Community Capital Facilitated COVID-19 Pandemic Resilience in a Qualitative Survey of Older Adults With Mild Cognitive Concerns.

The COVID-19 pandemic negatively affected known dementia risk factors and cognition in older adults. Our objective was to explore how older adults with cognitive concerns (ie, MCI or SCD) were able, or not able to engage in lifestyle activities associated with dementia prevention and maintain their wellbeing. We invited adults with mild cognitive concerns without dementia, aged ≥60 years participating in a randomised controlled trial of a psychosocial, secondary dementia prevention intervention, to complete a semi-structured survey, regarding how the pandemic impacted their lifestyle and wellb

Pattern and structural detection in grayscale images through the application of quantile graphs in higher-dimensional spaces.

Deep Learning (DL) and Machine Learning (ML) algorithms are adept at managing and classifying a wide range of data formats, including time series, text, and images, addressing challenges in both supervised and unsupervised learning. However, the practical applications of specific algorithms-particularly convolutional neural networks (CNNs) and vision transformers (VTs)-are often constrained by the need for large datasets, extensive training, and complex parameter tuning, which frequently relies on a trial-and-error approach. Other approaches, such as visibility graphs (VGs), often produce netw

Design considerations for C9orf72 disease prevention trials.

The idea that it might be possible to prevent some forms of amyotrophic lateral sclerosis and frontotemporal dementia has finally come of age. The hexanucleotide repeat expansion in the C9orf72 gene accounts for ∼10% of all amyotrophic lateral sclerosis and 10%-15% of all frontotemporal dementia diagnoses, with the two clinical syndromes co-manifesting in a significant number of patients. As a result, clinically unaffected carriers of pathogenic C9orf72 repeat expansions are currently the largest identifiable population at significantly elevated risk for both amyotrophic lateral sclerosis and
