Novel classification and pathogenetic analysis of hypoganglionosis and adult-onset Hirschsprung's disease.

BACKGROUND AND AIMS: Researchers have not clearly described the clinical and pathogenetic features of hypoganglionosis and adult-onset Hirschsprung's disease, which cause pseudo-obstruction or intractable constipation. We conducted this study to explore these features of hypoganglionosis and adult-onset Hirschsprung's disease in Korean patients. METHODS: We enrolled 24 patients pathologically confirmed as having hypoganglionosis and 11 as having adult-onset Hirschsprung's disease. We recruited 26 subjects who had undergone operation for nonobstructive colon cancer and 45 healthy volunteers as

Distribution of estrogen receptor alpha and progesterone receptor, and leukocyte infiltration in the cervix of cyclic bitches and those with pyometra.

The objectives were to localize estrogen receptor alpha (ERα) and progesterone receptor (PR), and enumerate leukocyte infiltration in cervical tissue of normal bitches during various stages of the estrous cycle (n = 35), as well as in those developing open (n = 22) or closed-cervix pyometra (n = 19). Each pyometra group was subdivided into anestrus and diestrus. Cervical tissues were collected after ovariohysterectomy. Receptor expressions were determined by immunohistochemistry and leukocyte infiltration was evaluated in histological sections stained with haematoxylin-eosin. The assessment wa

Assessment of the thoracic aorta by multidetector computed tomography: age- and sex-specific reference values in adults without evident cardiovascular disease.

BACKGROUND: Dilatation of the aortic root and other segments of the thoracic aorta is important in the pathogenesis of aortic regurgitation and of aortic dissection. Although echocardiographic criteria exist to detect aortic root dilation, comparably standardized methods have not been developed to detect enlargement of the remainder of the thoracic aorta. Nongated axial chest computed tomography (CT), traditionally used to evaluate aortic size, does not account for the obliquity, systolic expansion, and nonaxial motion of the aorta during the cardiac cycle. Reference values for aortic diameter

Impaired emptying of the retained distal stomach causes delayed gastric emptying after pylorus-preserving pancreaticoduodenectomy.

OBJECTIVE: Delayed gastric emptying (DGE) is the major morbidity after pylorus-preserving pancreaticoduodenectomy (PPPD). Gastroscintigraphy was used to characterize food distribution in the proximal and distal stomach during emptying. PATIENTS AND METHODS: Between October 2000 and June 2003, 20 healthy volunteers and 23 PPPD patients underwent single-phase gastric emptying scintigraphy 14 days after surgery. Scintigraphic studies of the stomach were divided into proximal and distal regions, and the ratio of proximal to distal radiation counts (P/DR) was plotted. Momentary monitor-displayed i

Left atrial size is a poor predictor of pathology in the older patient: preliminary observations.

OBJECTIVE: The goal of this study was to determine the relationship between left atrial (LA) size and pathology in humans. METHODS: We evaluated 14 patients (age 60 +/- 14 years, range 25-77, 9 males) who had died at our hospital. Eight patients were in sinus rhythm, 3 had paroxysmal atrial fibrillation (PAF), and 3 were in established atrial fibrillation (AF). LA size was calculated at transthoracic echocardiography (TTE) prior to death. At autopsy, histology of the dissected LA was examined at the appendage (APP), transverse sinus (TS), oblique sinus (OS), crux, and oval fossa. The severity

Ross Procedure After Valve-Sparing Root Replacement With Marfan Syndrome.

BACKGROUND: The Ross procedure is one of the optimal surgical options for aortic valve pathologies in relatively young patients. However, its application in patients with Marfan syndrome (MFS) is not only debatable but may even be considered a contraindication. CASE SUMMARY: A 31-year-old woman with MFS had undergone valve-sparing root replacement 2.5 years earlier and now presented for treatment of severe aortic insufficiency (AI). A Ross procedure with stabilization of the aortic root was performed. Two years after the procedure, mild AI was observed; however, the patient was asymptomatic,

Exercise recommendations for patients with Marfan syndrome: an updated review.

Marfan syndrome (MFS) is a genetic connective tissue disorder which is inherited in an autosomal dominant manner. It is a multisystem disorder affecting the skeletal, ocular, pulmonary and cardiovascular systems. The cardiovascular complications such as aortic root dilatation and aortic dissection represent the most life-threatening of these and are the major causes of a reduced life-expectancy in Marfan patients compared to the general population. While exercise offers the general population several health benefits, particularly in reducing cardiovascular risks, in MFS it has long been consid

Aortic Root Enlargement in a Young Athlete.

BACKGROUND: Aortic enlargement in athletes presents multiple clinical challenges including defining abnormal aortic size, the role of genetic testing, and counseling on sports participation. CASE SUMMARY: A 23-year-old former collegiate volleyball player was incidentally discovered to have an aortic root diameter of 4.4 cm. Genetic testing was negative for any variants associated with heritable thoracic aortic disease (HTAD). Concern remains for HTAD, and corresponding physical activity modifications and a surveillance plan have been established for this athlete. DISCUSSION: Genetic testing

MTR4 deficiency attenuates abdominal aortic aneurysm progression through suppressing VSMC senescence via the MTR4-RAP1a axis.

Abdominal aortic aneurysm (AAA) is a life-threatening vascular disease characterized by progressive aortic dilation, with insidious onset but high mortality after rupture and limited treatment options. Vascular smooth muscle cell (VSMC) senescence is a key driver of AAA progression. RNA helicase MTR4 has been reported to function in cardiovascular disorders, laying a foundation for exploring its aortic pathological functions. Our research findings indicate that MTR4 was significantly upregulated in both human and murine AAA tissues. Mendelian randomization indicated a suggestive causal associa

Ascending Aorta Aneurysm: What the Cardiologist Needs to Know.

A thoracic aortic aneurysm (TAA) is characterized by dilation of the aortic root or ascending aorta; it is a heritable disease in 10% to 20% and of unknown origin in the remainder; it is an increasingly recognized condition that is often diagnosed incidentally. It could be potentially life-threatening if left untreated due to its catastrophic rupture. The pooled incidence and prevalence of TAAs is about 5.3 per 100 000 individuals/year and 0.16%, respectively. The pooled incidence of ruptured aneurysms was estimated at 1.6 per 100 000 individuals/year. Genetic studies indicate that the somatic

Transient neurovascular coupling impairment in brain penetrating arterioles of streptozotocin treated mice post recurrent nonsevere hypoglycemia.

Hypoglycemia is a major complication of insulin therapy, particularly in type 1 diabetes mellitus. During hypoglycemia, the brain initiates vascular adaptations to maintain adequate blood flow. Here, we examined the effects of non-severe recurrent hypoglycemia (RH) on brain vascular diameter and neurovascular coupling (NVC)-the functional mechanism adapting local blood flow to neuronal energetic demand-in a streptozotocin (STZ)-induced hyperglycemia mouse model. We assessed stimulus-induced dilation of penetrating arterioles (PAs) in the somatosensory cortex of awake mice using in vivo two-pho

An unusual cecal submucosal mass in an elderly gentleman.

A 60-year-old male with previous hypertension was hospitalized because colonoscopy revealed a cecal submucosal mass during a community health examination 1 month previously. The patient has no symptoms such as poor appetite, fatigue, abdominal pain, diarrhea, nausea and vomiting. Laboratory tests and physical examination was unremarkable. A protrusion with multiple superficial small ulcers on the smooth surface measuring 1.5 × 2.0 cm was found beside the appendiceal orifice by colonoscopy. Abdominal computed tomography demonstrated a high-density mass without enhancement protruding towards the

SOX9-dependent fibrosis drives renal function in nephronophthisis.

Fibrosis is a key feature of a broad spectrum of cystic kidney diseases, especially autosomal recessive kidney disorders such as nephronophthisis (NPHP). However, its contribution to kidney function decline and the underlying molecular mechanism(s) remains unclear. Here, we show that kidney-specific deletion of Fbxw7, the recognition receptor of the SCFFBW7 E3 ubiquitin ligase, results in a juvenile-adult NPHP-like pathology characterized by slow-progressing corticomedullary cysts, tubular degeneration, severe fibrosis, and gradual loss of kidney function. Expression levels of SOX9, a known su

Surgical treatment of giant seminal vesicle cyst with ureteral compression: a case report.

BACKGROUND: Seminal vesicle cysts (SVCs) are rare benign diseases in men and are commonly asymptomatic. Giant SVCs with complications there are no standard treatments for SVCs, however surgical intervention is required for giant SVCs accompanied with complications. CASE SUMMARY: We present one case of a 49-year-old male patient diagnosed with giant SVC. Chief complaint of this patient was persistent gross hematuria for 1 week. Both a computed tomography (CT) and magnetic resonance imaging (MRI) scans indicated the presence of a cystic mass in left seminal vesicle with hemorrhage, of which the

Atypical Lesions in Virchow-Robin Spaces: A Case Report.

The differentiation between benign and malignant brain lesions remains a fundamental challenge in modern neuroimaging. This case highlights a rare presentation of ectatic Virchow-Robin spaces (VRS), which mimicked tumefactive brain lesions and required a comprehensive diagnostic evaluation to exclude neoplastic, infectious, and inflammatory processes. A 37-year-old female presented with progressive headache, cognitive impairment, and facial pain. Magnetic resonance imaging revealed a multiloculated subcortical cystic lesion, initially raising suspicions of a dysembryoplastic neuroepithelial tu

A mathematical model of human oesophageal motility function.

Recent advances in various observation methods revealed several unique characteristics of oesophageal peristalsis and its disorders. However, a framework for understanding the oesophageal motility pattern is lacking. Here, we propose a simple mathematical model of the human oesophageal motility function. The model comprises central nervous system signals, enteric nervous system neurons (interneurons and motoneurons) and oesophageal smooth muscles. The neural function implements excitable dynamics at the oesophageal body and toggle-switch dynamics at the lower oesophageal sphincter. The local s
