Venous insufficiency of the legs is a frequent and insidious disorder which, through its consequences' cascade, lead to some cutaneous microangiopathy with relative ischemia. Chronic leg ulcer is the late issue of the process accompanied by varied medical and socio-economic aspects. The pathogeny of the cutaneous lesions is directly related to alterations in hemorrheology and leukocyte adhesion inside capillaries. Elastic contention and some venotropic drugs exert preventive effects on the development of leg ulcers. They also bring some therapeutic improvement of the symptoms of leg heaviness.

An 18-year-old woman with renovascular hypertension had stenosis due to fibromuscular dysplasia (FMD) of the bilateral renal arteries, and this did not respond to conventional balloon angioplasty owing to marked elastic recoil. Implantation of Palmaz-Schatz stents resulted in successful dilation of the stenosis and remission of hypertension. Transstenotic pressure gradients were measured by use of a pressure-monitoring guidewire. It is suggested that stenting is a very useful adjunct to balloon angioplasty in the treatment of renal artery stenosis due to FMD.

We studied collagen expressed by skin fibroblasts in culture, and performed immunocytochemical, ultrastructural and stereological analysis of dermis from a child with signs reminiscent of a mild Ehlers-Danlos syndrome (EDS) type IV and severe periodontitis. No alterations in types I and III [corrected] collagen synthesis and secretion, or serum levels of type III procollagen aminoterminal propeptide were found, and morphological studies revealed non-specific alterations of collagen and elastic components observed in many connective tissue disorders.

This project investigated the potential spread of cancer within the larynx by an analysis of the permeability and resistance of the subepithelial spaces. Permeation and diffusion of dye injections in the loose connective tissues below the epithelium was analyzed by histologic preparations in the sagittal plane. A rare case of lipoma of the spermatic cord is reported. A 53-year-old man was admitted with painless mass in the left scrotum during the preceding one year. Since an elastic soft, tenderless, smooth surface, fist-sized and low echoic solid pattern mass in the left scrotum was suspected of tumor of the spermatic cord, resection of the mass with left orchiectomy was performed. Histological finding showed lipoma of the spermatic cord. Including the present case, 61 cases of intrascrotal lipoma in the Japanese literature are reviewed.

A 46-year-old man visited our clinic with the chief complaint of indolent swelling of the left scrotal contents. An elastic hard and fist-sized mass with negative transillumination was palpable in left scrotum, but testis and epididymis were not discriminated. Left high orchiectomy was performed. Tumor was 90 X 55 mm in size and the testis and epididymis at the lower pole of the tumor were intact. Histological diagnosis was leiomyoma and we diagnosed leiomyoma of the spermatic cord. To our knowledge, this is the 5th case reported in Japan.

Following endarterectomy, stenosis can recur in 0.2-3.6% of carotid arteries due to neointimal fibromuscular hyperplasia (NFH). Angiographically, NFH is manifested as concentric, alternating areas of normal tissue and smooth narrowing which are seen as pale, glistening, nonulcerated plaques on gross examination. Histological study demonstrates proliferation of smooth muscle cells, disruption of the internal elastic lamina, and often discontinuity of the endothelium. Calcium and cholesterol deposits are absent. The cause of the lesion is unknown, and surgical management is frequently difficult.

Dissection of the basilar artery caused sudden coma and death in a 40-year-old man. Atypical clinical features were explained at necropsy. A ventral dissection of the artery within its outer layers resulted in destruction of the pontine tegmentum with sparing of the basis pontis. An unsuspected defect in the internal elastic lamina in the left internal carotid artery was also found suggesting a more generalised disorder of arterial walls. Basilar artery dissection should be considered in the diagnosis of coma in young people.

In a group of 100 patients with ulcus cruris, the incidence of contact allergies was determined. Patch tests were applied with 18 products that might have come into contact with the skin. These products comprised drugs and excipients (preservatives and ointment vehicles) potential ingredients of tapes or of elastic bandages. Peru balsam (14%), wool-wax alcohols (10%), Nestosyl owing to its para-aminobenzoate component (7%), neomycin (5%) and Thyme oil (5%) were the allergens most frequently encountered.

Four cases of dissecting aneurysm of the aorta after lateral aortic clamping for aorto-coronary bypass are described. These accidents are rare but very serious. They often occur in hypertensive patients, and sometimes an aortic wall with a low elastic fibre content is found on histology. Surgery is that of dissecting aneurysm of the ascending aorta. However the severity of these accidents should lead to routine preventive measures during all aortocoronary bypass operations, in particular aortic anastomosis of the grafts under extra-corporeal circulation.

A male patient is presented is clinical and histopathological characteristics are typical of Ehlers-Danlos Syndrome (cutaneous and articular hyperelasticity and cutaneous fragility with the presence of atrophic scars). Looking into the family history we think that other relatives had the same disease and they had died during their childhood due to internal hemorrhages. The patient we present did not have significant hemorrhages in any moment. The elastic fibers in the dermal papilla are reduced giving rise to the appearance of being increased in the middle and deep dermis.
