Congenital dislocation of the radial head: spectrum and natural history.

A retrospective review of 24 patients with congenital radial head dislocation and three patients with congenital elbow dysplasias was performed. Eight patients had only congenital radial head dislocations and no other anomalies. In these eight patients, a noted limitation of elbow and forearm motion was greatest in those with anterior dislocations. Limitation of wrist motion also was noted in all affected extremities. Unilateral congenital radial head dislocation as well as progressive subluxation to dislocation were documented. There was no increased loss of motion as growth occurred. Signifi…

Traumatic injuries to the brachial plexus.

Transient lesions of the brachial plexus are fairly common but are difficult to recognize early because of other associated severe injuries of the brain and skeleton. They often cause hand edema, producing stiffness. In recent years, the understanding of the role of afferent sensory impulses and the application of microsurgical technique has helped in development of newer methods for surgical rehabilitation of the arm and hand. However, repair of the brachial plexus has to be supplemented by sequential reconstruction designed to restore the function of the shoulder, elbow, and hand. Although t…

[Nerve lesions following injuries of the upper limbs in childhood (author's transl)].

A total of 93 cases of nerve lesions in the limbs in children is reported. After blunt trauma (n = 53), there was most often damage to the ulnar nerve (n = 28), especially in the area of the elbow joint, followed by the radial nerve (n = 14) and the median nerve (n = 11). Causes were primary pressure and scar compression following fractures of the elbow and forearm, where ventral re-siting and/or neurolysis were necessary in serious denervation processes. Among the cutting injuries (n = 40), the lesions in the area of the underside of the wrist severing the ulnar and median nerves, more rarely…

Radial head replacement with intact lateral ulnar collateral ligament vs. repair and augmentation using nonabsorbable tape and anchors: a pragmatic comparative study.

BACKGROUND: Rupture of the lateral ulnar collateral ligament (LUCL), particularly in cases of complex radial head fractures, can occur in up to 80% of cases. The outcomes of radial head replacement in such scenarios may not be optimal despite ligament augmentation. The objective was to compare the clinical outcomes of radial head replacement in elbows with an intact LUCL to those with a complete LUCL rupture managed with repair and augmentation. METHODS: From January 2021 to June 2023, 27 patients underwent radial head replacement surgery using the Antea modular mobile radial head prosthesis …

Effects of Biomechanical Testing Using a Synthetic Ligament Fabricated from Polyhydroxyalkanoate Biopolyesters for Lateral Ulnar Collateral Ligament Reconstruction in Cadaver.

An injury to the elbow's lateral ulnar collateral ligament (LUCL) is an orthopedic emergency that can impair joint stability and functional biomechanics throughout the upper extremity. The development and application of synthetic ligament substitutes, particularly short-chain-length and medium-chain-length polyhydroxyalkanoate (SCL-PHA and MCL-PHA) co-polymers, represent a promising innovation for lateral elbow stabilization. This experimental cadaveric study aimed to (1) compare biomechanical parameters of torque and angular rotation among control, damage, repair, and reconstruction groups an…

Ulnar Collateral Ligament Reconstruction with Hamstring Autograft.

BACKGROUND: Ulnar collateral ligament (UCL) reconstructions have become increasingly common, particularly in youth overhead throwing athletes. These injuries are most commonly due to overuse and repetitive trauma on the inner elbow. Throwers with a deficient UCL often report decreased pitching speeds in addition to elbow pain and instability. INDICATION: The indications for this procedure include symptomatic valgus elbow instability during overhead throwing motions and a verified rupture of the UCL on advanced imaging. TECHNIQUE DESCRIPTION: The ipsilateral gracilis tendon is harvested and p…

Functional Outcome of Distal Humerus Fractures Treated With Open Reduction and Internal Fixation With Bicolumnar Plating in a Tertiary Care Setting.

Introduction Fractures of the distal humerus in the adult comprise approximately one-third of all humeral fractures. Over the past 20 years, nonoperative treatment for these fractures has been substituted by anatomic reduction and internal fixation based on the Association for Osteosynthesis (AO)/Association for the Study of Internal Fixation (ASIF) philosophy of plate fixation which resulted in early mobilization and superior performance. Pre-contoured, anatomically designed locking plates are anticipated to offer sufficient stability, permit early elbow range of motion, and safeguard the sof…

Suture anchor reinsertions of distal biceps rupture: a histologic analysis of a torn tendon and clinical results at short- and long-term follow-up.

BACKGROUND: Distal biceps brachii tendon (DBBT) rupture is a relatively rare injury. Nonsurgical treatment determines 30%-40% power loss of elbow flexion and up to 50% of forearm supination. Therefore, refixation of the DBBT is recommended. The DBBT is exposed to tension and compression loading. It is known that the tendon under compression might develop fibrocartilaginous metaplasia that improves the resistance to compression but reduces the resistance to tension. To test this hypothesis, the present study evaluated the presence of cartilage in DBBT samples. Furthermore, the present study eva…

Are the Current Little League Pitching Guidelines Adequate? A Single-Season Prospective MRI Study.

BACKGROUND: Little League throwing guidelines have recently been implemented in an attempt to lessen the growing number of elbow injuries occurring in youth baseball players. HYPOTHESIS/PURPOSE: The purpose of this study was to examine pre- and postseason changes seen on magnetic resonance imaging (MRI) in youth baseball players' elbows in an attempt to identify risk factors for pain and MRI abnormalities, with a particular focus on the current Little League guidelines. We hypothesized that MRI abnormalities would be common in pitchers with high pitch counts and poor guideline compliance. ST…

Diagnosis of occult radial head and neck fracture in adults.

The purpose of this study was to compare imaging modalities in the diagnosis of occult radial head and neck fractures and to assess the diagnostic value of ultrasound in diagnosing occult fractures of the radial head and neck. The study included 193 patients (101 male, 92 female) who were referred by trauma surgeons from January 2011 to July 2014 and presented with history of acute elbow trauma. The mean age of the patients was 37 years (range 15-82 years); 95 right and 98 left elbows were included in the study. Clinical examinations and standard radiograms were conducted. The anteroposterior …

Let them move: Early range of motion and rehabilitation after fixation of medial epicondyle fractures in adolescent athletes.

BACKGROUND: Medial epicondyle fractures account for 11%-20% of pediatric elbow fractures. While nonoperative treatment is effective, surgery is often recommended for overhead athletes to enhance union rates, valgus stability, and expedited return to sporting activities. Postoperative recovery protocols vary, with traditionally up to 2 weeks of strict immobilization and rehabilitation lasting 6-12 months. This study evaluates the early range of motion (ROM) and accelerated rehabilitation in high-performing youth athletes undergoing surgical fixation. METHODS: A retrospective review included pa…

Scapulothoracic joint function and its association with throwing-related injuries in collegiate baseball pitchers: a prospective cohort study.

BACKGROUND: Throwing-related shoulder and elbow injuries are common among collegiate baseball pitchers; however, prospective evidence on scapulothoracic (ST) joint function as a risk factor is limited. This study prospectively investigated whether ST joint stability was associated with throwing-related injuries. METHODS: Twenty-seven collegiate pitchers were prospectively categorized into injury and noninjury groups during the 12-month follow-up. Scapular and trunk positional coordinates were recorded using a 3D scanner during preseason screening under 2 conditions: (1) 90° scapularplane abdu…
