Breaking barriers: navigating cardiovascular health in the transgender community.

Transgender individuals, whose gender identity differs from their assigned sex at birth, experience unique health disparities, including an increased risk of cardiovascular diseases (CVDs). This review examines how gender identity influences cardiovascular health among the transgender population, with a particular focus on emerging risk factors, biological mechanisms, and psychosocial determinants, and the role of gender-affirming hormone therapy (GAHT).A comprehensive literature review was performed on PubMed, Scopus, and Embase to search for literature focused on cardiovascular diseases amon

[Management of recurrent urolithiasis: indications for therapy escalation and discontinuation].

Urolithiasis is a common condition characterized by a high recurrence rate and substantial clinical and economic impact. Stone formers are stratified into low- and high-risk groups based on stone analysis and basic diagnostic evaluation. Following a stone event (spontaneous passage or interventional treatment), all patients should receive general preventive measures, including increased fluid intake, dietary modification, and lifestyle changes. High-risk patients should additionally undergo extended metabolic evaluation (24 h urine analysis) and, if indicated, targeted pharmacological therapy.

Assessment and Protection of Cerebral Function in Patients with Acute Liver Failure.

Hepatic encephalopathy (HE) is a frequent and life-threatening neurological complication of acute liver failure (ALF) encountered in the intensive care unit and remains a major determinant of short-term mortality and long-term neurological outcomes. Increasing evidence indicates that HE in ALF is primarily driven by hyperammonemia, with synergistic contributions from systemic inflammation, cerebral hemodynamic dysregulation, metabolic failure, and osmotic imbalance. These interacting mechanisms promote astrocytic swelling, disruption of the blood-brain barrier, cerebral edema, and intracranial

[Lifestyle pattern and therapeutic adherence in patients on hemodialysis].

BACKGROUND: Chronic kidney disease (CKD) is a common complication of hypertension and diabetes mellitus. Hemodialysis (HD) is a renal replacement therapy that requires self-care and therapeutic adherence (TA), which if not performed increase the risk of associated complications. Therefore, it is essential to adopt self-care practices and implement modifications in lifestyle patterns (LP). OBJECTIVE: To determine changes in LP and TA in patients with CKD undergoing HD following educational interventions (EIs). MATERIAL AND METHODS: Quasi-experimental, longitudinal, non-randomized study. Socio

Risk Stratification and Fetal Therapies for Congenital Diaphragmatic Hernia.

Congenital diaphragmatic hernia (CDH) remains one of the most challenging congenital anomalies encountered by neonatologists and pediatric surgeons, with outcomes largely determined before birth. Although surgical repair of the diaphragmatic defect is essential, postnatal morbidity and mortality are primarily driven by antenatal pulmonary hypoplasia, abnormal pulmonary vascular development, and associated cardiac dysfunction that culminate in respiratory failure and pulmonary hypertension. Advances in prenatal imaging have enabled increasingly refined risk stratification, allowing for the iden

Infant with Complex Congenital Cardiac Anomalies: A Case Report and Review of Literature.

The coexistence of multiple complex congenital cardiac anomalies in a single infant - particularly unbalanced Atrioventricular Septal Defect; Double Outlet Right Ventricle; Infracardiac Total Anomalous Pulmonary Venous Connection, situs inversus, and pulmonary hypertension - is exceptionally rare and presents significant surgical and physiological challenges. A full-term male neonate was diagnosed at 25 days of age with this rare combination of defects. Despite low-flow oxygen, the baseline oxygen saturation remained at 60%-65%, and growth was severely impaired. Initial management at a tertiar

Crossing the blood-brain barrier: neurotrauma in disasters and resource-limited settings.

Traumatic brain injury (TBI) is a major cause of morbidity and mortality in natural disasters, armed conflict, and other resource-limited settings, where advanced imaging, invasive neuromonitoring, and specialized neurosurgical care, common in high-resource environments, are often unavailable. This narrative review summarizes the global health impact of TBI in austere conditions and outlines key management strategies and ethical considerations relevant to these contexts. Evidence from civilian disaster responses, military conflicts, and global health guidelines highlight the need for adaptable

Targeting Neuropeptide Y: Novel Approaches to the Treatment of Cardiovascular and Haematological Disorders.

BACKGROUND: The 36-amino acid peptide known as neuropeptide Y (NPY) is widely expressed in both the central and peripheral nerve systems and is essential for regulating energy balance, stress responses, cardiovascular function, and immunological regulation through Gprotein- coupled Y receptors (Y1, Y2, Y4, Y5). OBJECTIVE: To explore the various functions of NPY in hematological and cardiovascular disorders and investigate potential therapeutic approaches that target NPY signaling networks. METHODS: A comprehensive literature analysis focused on NPY-mediated mechanisms in cardiovascular disea

Transport of the Open Abdomen: A Review.

OBJECTIVE: An open abdomen (OA) is a critical surgical condition in which the abdominal wall is intentionally left open to prevent or manage intra-abdominal hypertension or abdominal compartment syndrome or facilitate staged re-exploration. Although OA management is well described in surgical literature, the safe transport of patients with an OA-particularly in air medical and critical care transport settings-presents unique challenges that are less frequently addressed. METHODS: This review summarizes current evidence and best practices for the transport of patients with an OA, focusing on p

Tubular retractor approach for resection of a subependymal giant cell astrocytoma (SEGA) in tuberous sclerosis complex (TSC): a case report.

BACKGROUND: Subependymal giant cell astrocytoma (SEGA) is a rare pediatric tumor that most commonly occurs in the setting of tuberous sclerosis complex (TSC). The deep anatomical location of these lesions makes the selection of an appropriate neurosurgical approach critical. However, minimally invasive techniques have emerged as a safe and effective alternative for the surgical resection of tumors located in deep regions. OBSERVATIONS: A 7-year-old male with a history of TSC presented with a large intraventricular mass located in the frontal horn of the right lateral ventricle, associated wit

Mad honey poisoning in Nepal: ecology, epidemiology, and clinical spectrum of grayanotoxin intoxication (2009-2026).

Wild honey from the Himalayan cliff bee Apis laboriosa, commonly known as mad honey, has been consumed across Nepal's mid-Himalayan belt for centuries for its perceived medicinal and psychoactive properties, yet its capacity to cause grayanotoxin poisoning has never been systematically documented. The bee forages on Rhododendron species whose nectar contains grayanotoxins, which become concentrated in the honey it produces. We reviewed 27 publications describing approximately 68 cases of grayanotoxin poisoning spanning five decades, situating the clinical epidemiology within the ecological fra

Diagnostic accuracy of ultrasonography for lower extremity arterial disease in patients with diabetes.

Lower extremity arterial disease (LEAD) is a major macrovascular complication in patients with diabetes mellitus, and accurate noninvasive imaging is essential for early recognition and risk stratification. This retrospective diagnostic accuracy study included 59 adult patients with diabetes who underwent both lower extremity arterial ultrasonography and contrast-enhanced computed tomography angiography (CTA) within a predefined interval of ≤ 30 days. Baseline data showed that patients with CTA-confirmed LEAD were older, had longer diabetes duration, higher glycated hemoglobin (HbA1c), a highe

Adrenergic receptors on immune cells in cardiovascular disease: signaling plasticity, biased agonism, and therapeutic opportunities.

Sustained sympathetic nervous system activation is a hallmark of hypertension, atherosclerosis, ischemic injury, stroke, and heart failure. While adrenergic receptor (AR) biology is often discussed in cardiomyocytes, vascular smooth muscle, and endothelium, immune cells also express α1-, α2-, and β-ARs, enabling leukocytes to directly sense catecholamine tone and translate it into inflammatory or pro-resolving programs. In this review, we integrate subtype-specific signaling logic (α1AR-Gq/PLC/Ca2+; α2AR-Gi with reduced cAMP; βAR-Gs with cAMP-PKA/EPAC) with key regulatory features that determi

Atypical Teratoid/Rhabdoid Tumor of the Posterior Fossa Mimicking Medulloblastoma in an Infant: A Case Report.

Atypical teratoid/rhabdoid tumor (AT/RT) is a rare and highly aggressive central nervous system (CNS) neoplasm that predominantly affects children under three years of age. In the posterior fossa, AT/RT frequently mimics medulloblastoma (MB) on both radiological and histopathological evaluation. Owing to significant morphological overlap, a definitive diagnosis cannot be established on histology alone and requires immunohistochemical (IHC) confirmation, particularly demonstration of loss of nuclear integrase interactor 1 (INI1) expression. We report the case of a seven-month-old male infant wh

New Frontiers in the Management of Aortic Disease.

The aorta has multiple functions within the cardiovascular system and disease progression remains incompletely understood. Diseases of the aorta cause substantial morbidity and mortality. This review summarizes the management of thoracic aortic disease, current knowledge gaps, and future opportunities. There is an increasing effort to incorporate aortic function, beyond diameter, into risk stratification for aortic disease. Medical therapies focus on slowing growth of aortic aneurysm, minimizing perioperative risk, and reducing overall cardiovascular mortality, focusing on treating hypertensio

Platypnea-Orthodeoxia Syndrome Caused by a Patent Foramen Ovale and Interatrial Septal Aneurysm: A Case Report.

Platypnea-orthodeoxia syndrome (POS) is a rare condition characterized by positional hypoxemia, often due to right-to-left shunting across a patent foramen ovale (PFO) despite normal right-sided pressures. Early recognition is essential as symptoms may be subtle and imaging is often unrevealing. A 78-year-old female patient with a known PFO presented with one week of lower abdominal pain and mild exertional dyspnea without chest pain. She was profoundly hypoxic to 76% on room air, with improvement when supine and worsening when upright. Initial labs were unremarkable, and arterial blood gas re
