OBJECTIVES: We undertook this report to underline the risks of lesions of the internal carotid artery after lateral oropharyngeal trauma in children and to discuss the diagnosis and treatment of this complication.

Genioplasty has been a useful and frequently employed technique in the aesthetic facial surgeon's armamentarium. However, as the prevalence of obstructive sleep apnea becomes more apparent and its diagnosis more frequent, genioplasty has also evolved to a commonly performed procedure for reconstruction of the upper airway. This article discusses patient evaluation and surgical techniques used in aesthetic and functional surgery of the chin. A case of fatal, inadvertent, intraspinal placement of a Foley urinary catheter is presented. The patient suffered a severe craniofacial injury with life-threatening epistaxis requiring emergent tamponade with two Foley catheters plus gauze packing. There was an associated atlanto-occipital dislocation. The mechanism of production is discussed, and guidelines to avoid this complication are outlined.

Midfacial trauma is often complicated by ocular disorder. A scoring system has been devised to help the maxillofacial surgeon identify patients who warrant referral to an ophthalmologist. A prospective pilot study was carried out on 100 patients with midfacial fractures to evaluate the effectiveness of this system in clinical practice. Computed tomography (CT) is ideally suited for detection of intracranial pathology, whereas plain radiography is limited to the bony structures. Distinguishing surgical and nonsurgical lesions is quite feasible with CT. Follow-up examinations are non-traumatic and carry minimal risk to the critically ill patient. In the great majority of head trauma cases, the recognition of pathology is unequivocal, thus facilitating proper management.
