A rare lethal combination of premature closure of the foramen ovale and d-transposition of the great arteries with intact ventricular septum.

We report an infant who presented with cyanosis after birth and was unresponsive to all resuscitative efforts. Emergent echocardiogram showed d-transposition of the great arteries (d-TGA) with an intact ventricular septum. Attempts to perform a bedside balloon atrial septostomy failed due to inability to traverse the atrial septum. Autopsy revealed d-TGA with intact atrial and ventricular septa and patent ductus arteriosus. Though no survivors have been reported for this rare lethal combination of lesions thus far in the literature, we speculate that recent advances in fetal treatment may soon

A rare case of biventricular non-compaction associated with ventricular septal defect and descendent aortic stenosis in a young man.

Non-compaction of the ventricular myocardium is a cardiomyopathy caused by the arrest of normal embryogenesis of the ventricles. It is classified in isolated non-compaction of the ventricles (most frequently of the left one) and in ventricular non-compaction associated with other congenital anomalies of the endocardium and myocardium, such as obstruction of the right or left ventricular outflow tracts, complex cyanotic congenital heart disease, and coronary artery anomalies. There are controversies regarding the right ventricle non-compaction due to the normally trabeculated shape of its walls

Phenytoin metabolite renal calculus: an index case.

INTRODUCTION: Drugs and their metabolites are known factors in 1% to 2% of all kidney stones. Certain antiepileptic drugs are known to cause stone formation. Phenytoin is used as a first line antiepileptic therapy for many seizure disorders. We present what we believe to be the first report of a phenytoin metabolite urinary stone. METHODS: A 79-year-old woman with a fever and seizure disorder was found to have a right pelvic kidney with hydronephrosis and multiple large calcifications. She had been taking the antiepileptic medication phenytoin for the past 10 years. Average total serum phenyt

Cysteine, sulfite, and glutamate toxicity: a cause of ALS?

BACKGROUND: Amyotrophic lateral sclerosis (ALS) of nonmutant superoxide dismutase (SOD) type may be caused by toxicity of the reduced glutathione (GSH) precursors glutamate and cysteine, and sulfite (a metabolite of cysteine), which accumulate when one or more of the enzymes needed for GSH synthesis are defective. OBJECTIVES: A case is examined where the patient exhibited elevated sulfur on a hair mineral analysis, elevated blood cysteine, positive urine sulfite, elevated urine glutamate, and low whole blood GSH. During the time when strict dietary and supplement measures normalized the patie

Partial mole with fetal triploidy as a cause for imminent HELLP-syndrome at 16 weeks of gestation.

A woman at 16 weeks of gestation was admitted to our perinatal center with unspecific abdominal pain. The results from blood samples 12 h after admission revealed a fulminant HELLP-syndrome. After starting i.v. corticosteroid therapy, the woman recovered quickly. CVS was performed because of abnormal findings by ultrasound and a fetal triploidy (69, XXX) was diagnosed. Pregnancy was terminated and histopathological examination of the placental tissue confirmed a partial mole.

A special case of anencephaly in an early-born baby with an exagerated prognastic face: further example for human devolution.

A 7-month-old baby was born in a village near Iskenderun (Turkey) where "Unertan Syndrome" with quadrupedality and primitive cognitive abilities was discovered. The clinical diagnosis was anencephaly. However, his head did not show the classical symptoms of anencephaly because it was covered with bony structures. The baby has an ape-like, prognasthic head with low-set ears and flapped ear flaps. The other parts of the body were similar to humans with broad shoulders and a short neck. This may be a further example of human devolution, which was first reported by Tan (2005, 2006a,b,c). A genetic

Poorly differentiated leiomyosarcoma of the urogenital tract in a horse.

CASE DESCRIPTION: A 2-year-old Thoroughbred filly was evaluated because of hemorrhage from the vulva and suspected hematuria of 5 days' duration. CLINICAL FINDINGS: A primary coagulopathy was ruled out on the basis of results of hematologic testing. Vaginoscopy and cystoscopy revealed a large bleeding mass in the bladder that extended into the vagina, causing marked obliteration of normal urogenital structures and difficulty in urination. Histologic examination of endoscopic and surgical biopsy specimens revealed a poorly differentiated neoplasia likely of mesenchymal origin. Chronic suppurat

[Acute anterior myocardial infarction due to left main coronary artery thrombosis].

Acute anterior myocardial infarction (MI) due to left main coronary artery thrombosis is a rare entity with a very high mortality rate. A 72-year-old male patient was admitted with chest pain of two-hour onset that appeared during syncope. Electrocardiography showed first-degree AV block, right bundle branch block, left anterior fascicular block, ST-segment elevation of 5 mm in lead aVR, and significant ST depression in anterior derivations, suggesting acute anterior MI. Coronary angiography showed total occlusion of the left main coronary artery. During consultation for emergency operation, h

[A histologic study of brain in fatal cerebral malaria].

Plasmodium falciparum infection is an emergency because of the risk of cerebral malaria, that is the most severe complication. Malaria diagnosis is usually made on blood samples of feverish patients coming from endemic area. The pathologist is rarely confronted with malaria lesions. If the occasion arises, the diagnosis of malaria is often already known or suspected and the pathologist has to confirm it. Rarely, malaria is unknown before histological examination and the diagnosis is based on the identification of specific histological features. We report a case of sudden death due to cerebral

Fever, abdominal pain, and leukopenia in a 13-year-old: a case-based review of meningococcemia.

The presenting symptoms of meningococcemia are protean, and the illness is rapidly progressive and often fatal, making it simultaneously one of the most dangerous and most important illnesses the Emergency Physician can encounter. It attacks the young and it is highly contagious. This report uses one of the many unusual presentations of meningococcemia as a framework for discussing the epidemiology, presentation, diagnosis, and treatment of meningococcal disease.

Metastatic thymoma and acquired generalized myasthenia gravis in a beagle.

A 16-year-old, spayed female beagle was diagnosed with metastatic thymoma causing a probable paraneoplastic syndrome of generalized acquired myasthenia gravis. Anticholinesterase treatment was initiated; however, 5 days later the dog died.

Hemorrhagic small intestine cancer with solitary pulmonary metastasis initially presented as suspected primary lung cancer: an autopsy report.

Cancer of the small intestine presenting with a solitary pulmonary metastasis is rare. Diagnosis and treatment of hemorrhagic small intestinal disease is clinically problematic due to its anatomic aspect, especially after multiple laparotomies. The case that we present here was a 79-year-old man who was initially diagnosed with suspected T2N2M0 lung cancer. After non-diagnostic results on two bronchoscopic biopsies and computed tomography-guided needle biopsy, he was admitted for thoracoscopic biopsy and possible curative operation. The patient had a history of multiple laparotomies for gastri

Calciphylaxis: a pseudo-vasculitis syndrome.

OBJECTIVES: To report a case of fatal calciphylaxis, an uncommon condition affecting patients with chronic renal disease and calcium metabolism abnormalities, that can mimic vasculitis. METHODS: We reviewed the English literature using the Medline and Embase databases and keywords "calciphylaxis" and "calcific uremic arteriolopathy." RESULTS: A patient with end-stage renal disease and no known calcium metabolism abnormalities presented with intractable lower extremity ulcers and skin findings suggestive of small-vessel disease of the upper extremities. Biopsy of the lesions showed classic ca
