Clinical Outcomes of Hyperplantarflexion Variant Compared With Supination External Rotation Ankle Fractures: A Matched-Cohort Analysis.

Recent literature has reported an uncategorized hyperplantarflexion variant ankle fracture characterized by a posteromedial fragment separate from the posterior or medial malleolar fragments. The current study sought to determine whether the outcomes for surgically treated hyperplantarflexion variant fractures are similar to the more common supination external rotation (SER) IV fractures. A prospective registry of operatively treated ankle fractures was queried to create 2 age- and gender-matched cohorts: hyperplantarflexion variant and SER IV fractures. Each cohort had 23 patients (18 females

Rotational stability in screw-fixed scaphoid fractures compared to plate-fixed scaphoid fractures.

BACKGROUND: The literature describes the treatment of scaphoid fractures comparing the volar and dorsal approaches, the advantages and disadvantages of percutaneous screw fixation, as well as the treatment of scaphoid nonunions using different types of cancellous or corticocancellous bone grafts. Yet, to date no studies are available comparing the outcome of rotational stability in screw-fixed scaphoid fractures to angular stable systems. The purpose of this study is to provide reliable data about rotational stability in stabilised scaphoid fractures and to gain information about the rigidity

Total hip arthroplasty in a patient with congenital insensitivity to pain: a case report.

INTRODUCTION: Congenital insensitivity to pain, a rare neurological entity, is characterized by varying degrees of sensory loss and autonomic dysfunction. Orthopedic manifestations of congenital insensitivity to pain include delayed diagnosis of fractures, nonunions, malunions, Charcot arthropathy, acro-osteolysis, avascular necrosis, osteomyelitis, heterotopic ossification and joint dislocations. We here report the case of a patient with congenital insensitivity to pain who had multiple lower extremity fractures at varying intervals, the most recent being a femoral neck fracture managed by to

[Early results of treatment of proximal humerus fractures with the PHILOS locking plate].

OBJECTIVES: The aim of this study was to evaluate early results of proximal humerus fractures treated with the PHILOS locking plate. METHODS: Proximal humerus fractures of 22 patients (13 males, 9 females; mean age 57 years; range 35 to 83 years) were treated with the PHILOS locking plate. According to the Neer classification, 13 patients had comminuted fractures, four patients had valgus impact injuries, two patients had fracture-dislocations, and three patients had fractures involving the proximal 1/3 of the humerus. The fractures were reduced by the transdeltoid lateral approach (n=8) usin

Chronic anterior monteggia lesions in children: report of 4 cases treated with closed reduction by ulnar osteotomy and external fixation.

BACKGROUND: Chronic, subacute, and chronic Monteggia fracture dislocations of the elbow are challenging problems. The literature generally recommends complex open reconstructive procedures. We present an alternative, minimally invasive, percutaneous method with external fixation. METHODS: This was a retrospective review of 4 cases presenting with chronic Monteggia lesions with anterior dislocation of the radial head. RESULTS: Reduction of the radiocapitellar joint was successfully achieved in all patients after ulnar osteotomy and gradual correction using the Ilizarov external fixation. Open…

[Traumatic spondylolisthesis of L5-S1].

Pure traumatic bilateral lumbosacral dislocation is a rare injury with just eight cases reported in the literature. This condition occurred also in 36-year-old man, who was struck into the lower back by a falling tree, during a woodcutting, at the moment when he was kneeling and his spine was flexed. Neurological examination showed no signs of spinal nerves injury. X-ray examination of the lumbosacral spine revealed the presence of a 40% anterior dislocation of L5 over S1 with locked facets and multiple fractures of transverse processes. Computer tomography confirmed these findings and also re

Radiographic evaluation of the acromioclavicular and sternoclavicular joints.

Plain radiography is useful for the initial assessment of suspected disorders of the sternoclavicular and acromioclavicular joints. Other modalities are often required to further assess more complex pathologies involving these joints, however. Ultrasound has been described as a screening tool to assess possible sternoclavicular joint dislocation; however, it is usually used only if CT and MRI are not readily available. It has also been used to confirm intraoperative relocation. Ultrasound has a limited role in the evaluation of the AC joint, where it is most useful to exclude the presence of j

Distal tibial and fibular epiphyseal fractures in children: prognostic criteria and long-term results in 158 patients.

From 1974 to 1993, 158 injuries of the ankle joint were treated at the Department of Orthopedics and Traumatology of Santobono Children's Hospital. One hundred thirty-two patients were treated conservatively; 26 patients underwent surgical treatment. Fibular fractures of the malleolus without epiphyseal separation or dislocation (68 patients) were excluded from this review. Of the 158 patients, 113 (70%) were available for follow-up. The mean age of patients was 11 years. The mean follow-up was 6 years (range 3-15 years). In this review, the authors compare, based on the degree of epiphyseal s

[Posterior shoulder joint instability. Classification, pathomechanism, diagnosis, conservative and surgical management].

The posterior instability of the shoulder is a more difficult diagnostic and therapeutic challenge than the anterior instability. There are many etiologies and causes of posterior instability. Most studies in the literature are retrospective and yield a great variation in therapeutic recommendations. Generally it has to be separated in traumatic and atraumatic instabilities. Most of the traumatic dislocations are impaction fractures of the humeral head against the dorsal glenoid. Therapy is depending on the size of the humeral defect, the duration of dislocation and the functional demand of th

Inferior capsular shift procedure for anterior-inferior shoulder instability in athletes.

Sixty-eight shoulders in 63 athletic patients with anterior-inferior glenohumeral instability underwent an anterior-inferior capsular shift procedure. Shoulders with glenoid fractures, predominantly posterior instability, or routine, unidirectional anterior instability were not included in this study. There were 42 men and 21 women, with an average age of 23 years. Forty-two repairs were performed on the dominant arm. All 31 overhead throwing athletes had their dominant arms repaired. Forty-six shoulders had histories of recurrent anterior dislocations, while 22 shoulders had recurrent subluxa

A retrospective study of Moore and Thompson hemiarthroplasty. A review of 599 surgical cases and an analysis of the technical complications.

In a retrospective review of 580 patients with 599 hemiarthroplasties of the hip, 454 operations (75.8%) were carried out as primary procedures for fresh intracapsular fractures, and 145 operations (24.2%) were performed as salvage procedures. The three-month mortality rate was 5.0%, and the dislocation rate was 5.3%. The early results were assessed by roentgenograms alone; head size, neck length, stem-shaft angle, and calcar seating were measured on preoperative and postoperative films. Anteversion and retroversion could not be accurately assessed. Using an arbitrary rating system, the result

A novel figure-of-eight polymeric cerclage wiring technique for greater trochanter fixation in revision arthroplasty and conversion of failed proximal femur fracture fixations to total hip arthroplasty.

INTRODUCTION: Periprosthetic femoral fractures occur in 0.8-10% of primary total hip arthroplasties (THA) and up to 17.5% for revision THA (revTHA), with isolated greater trochanter (GT) fractures being the most predominant pattern. This study aimed to evaluate a novel figure-of-eight cerclage wiring technique for greater trochanter fixation in hip replacement cases. METHODS: These retrospectively collected data included 36 hips (36 patients) undergoing revision THA or conversion of failed intertrochanteric fracture to THA with figure-of-eight cerclage wiring between April 2021 and June 2023.

Non-neoplastic indications and outcomes of the proximal and distal femur megaprosthesis: a critical review.

PURPOSE: Megaprosthesis or endoprosthetic replacement of the proximal and distal femur is a well-established modality for treatment of tumors. The indications for megaprosthesis have been expanded to the treatment of some non-neoplastic conditions of the knee and hip, with the severe bone loss associated with failed arthroplasty, communited fractures in the elderly with poor bone quality, and resistant non-union. Th aim of this study is to find out whether megaprosthesis of the knee and hip is successful in the treatment of non-neoplastic condtions. The study comprises a review of the indicati

Shoulder problems of adolescents. How they differ from those of adults.

The adolescent athlete with an open epiphyseal plate presents with different shoulder problems than does the adult. Most shoulder injuries in the adolescent athlete can be treated conservatively with a good prognosis for full return of function. The exception is a glenohumeral dislocation, which will probably recur and need a surgical reconstruction.

Polyethylene wear in metal-backed tibial components in unicompartmental knee prostheses.

The utilization of unicompartmental knee arthroplasty (UKA) has increased worldwide and has become a favourable alternative to total knee arthroplasty (TKA) in patients with isolated single-compartment knee osteoarthritis. With the increase in UKA utilization, there has been an increase in the number of revisions. The two most common causes of revision are the progression of osteoarthritis in other compartments and aseptic loosening with polyethylene wear, with other common causes for revision including periprosthetic fracture, bearing dislocation, infection, instability and unexplained pain.
