A study on the correlation between albumin-to-alkaline phosphatase ratio and the severity of pelvic inflammatory disease.

OBJECTIVE: To evaluate albumin-to-alkaline phosphatase ratio (AAPR) as a pelvic inflammatory disease (PID) severity biomarker and determine PID predictors. METHODS: This retrospective study included ninety-nine patients with PID, as the experimental group, further classified by severity into mild and severe PID groups. The control group included forty-nine patients who underwent elective surgery for non-inflammatory tubal-ovarian masses in the same period. Relationships between PID severity and general information, vital signs, laboratory indices, and imaging characteristics among the three g

A Retrospective Review of Cardiogenic Shock Development in Patients With ST-Elevation Myocardial Infarction and Percutaneous Coronary Intervention Receiving Early Beta-Blockers.

Beta-blockers (BBs) have proven to improve morbidity and mortality in patients after an ST elevation myocardial infarction (STEMI). Guidelines suggest initiating a BB within 24 hours, except in those with risk factors for developing cardiogenic shock, although published literature is conflicting regarding the true association of these risk factors with shock. This retrospective cohort study aimed to assess whether the presence of defined risk factors was associated with cardiogenic shock after early BB administration in patients with a STEMI and percutaneous coronary intervention. The primary

Hydrogen therapy after resuscitation improves myocardial injury involving inhibition of autophagy in an asphyxial rat model of cardiac arrest.

Hydrogen (H2) therapy is a therapeutic strategy using molecular H2. Due to its ability to regulate cell homeostasis, H2 therapy has exhibited marked therapeutic effects on a number of oxidative stress-associated diseases. The present study investigated the effectiveness of H2 therapy in protecting against myocardial injury in a rat model of asphyxial cardiac arrest and cardiopulmonary resuscitation. Rats underwent 10-min asphyxia-induced cardiac arrest (CA) and cardiopulmonary resuscitation (CPR), and were randomly divided into control and H2 therapy groups. After resuscitation, the H2 therapy

Risk of coronary artery disease in adults with congenital heart disease: A comparison with the general population.

BACKGROUND: Coronary artery disease (CAD) will increasingly determine outcome in the aging adult congenital heart disease (CHD) population. We aimed to determine sex-specific incidence of CAD in adult CHD patients throughout adulthood, compared to the general population. METHODS AND RESULTS: We followed 11,723 adult CHD patients (median age 33 years; 49% male; 57% mild, 34% moderate, 9% severe CHD) from the Dutch CONCOR registry, and two age-sex-matched persons per patient from the general population for first CAD event in national registers (period 2002-2012). Incidence rates were estimated

Electrical activity of the diaphragm (Edi) values and Edi catheter placement in non-ventilated preterm neonates.

OBJECTIVE: The electrical activity of the diaphragm (Edi) reflects neural respiratory drive. Edi peak correlates with inspiratory drive and Edi minimum (Edi min) correlates with the tonic activity of the diaphragm. Edi data in non-ventilated preterm neonates have not been determined. The primary aim of this study was to determine Edi values in non-ventilated preterm neonates throughout postnatal maturation and with various types of noninvasive respiratory support. The secondary aim was to evaluate the success or complications of placement of the Edi catheter in premature neonates. STUDY DESIGN

[The utility of quantitative 99mTc-GSA liver scintigraphy in the evaluation of hepatic functional reserve: comparison with 99mTc-PMT and 99mTc-Sn colloid].

Using data from 17 patients with liver cirrhosis and 3 patients with fatty liver, we have compared the utility of 3 hepatic imaging agents in the evaluation of hepatic functional reserve. Evaluated here were 99mTc-galactosyl human serum albumin (GSA) which is a new ligand for hepatic binding protein, 99mTc-N-pyridoxyl-5-methyl tryptophan (PMT) of a hepatobiliary agent, and 99mTc-Sn colloid. In each patient, we performed these 3 imaging studies within a week and also examined hepatic function tests (indocyanine green test, hepaplastin test, choline-esterase, etc). In each imaging study, serial

Physiological correlates and predictors of vasovagal responses following dry needling in myofascial pain syndrome: a controlled observational study.

BACKGROUND: Vasovagal responses (VVR) are common transient autonomic reactions to invasive procedures such as injections or needling, characterized by bradycardia, hypotension, and transient loss of consciousness; however, their occurrence and underlying autonomic mechanisms during dry needling (DN) for myofascial pain syndrome (MPS) remain poorly understood. OBJECTIVES: This study aimed to determine the frequency of VVR during DN in patients with MPS and to compare autonomic parameters-heart rate variability (HRV), pupil diameter, and skin perfusion-between those with and without VVR. DESIG

Predicting low oxygen in patients with acute COVID-19 infection isolating at home: a clinical prediction model.

RATIONALE: Identifying patients who could be safely managed at home versus those needing hospitalisation was a particular concern during early COVID-19. Respiratory viruses remain a concern, including new COVID-19 variants, influenza and respiratory syncytial virus. We developed COVIDFree@Home, a mobile application and clinician dashboard for remote monitoring, to determine if remotely collected measures could predict low oxygen saturation in home-isolating patients with COVID-19. METHODS: We conducted a prospective cohort study of patients newly diagnosed with COVID-19 from three Toronto hos

Complex interplay between disease and aerobic exercise training on vascular function in an exacerbated emphysema rat model.

Chronic obstructive pulmonary disease (COPD) is a respiratory disease with systemic complications, including vascular dysfunction. Aerobic exercise improves blood pressure, endothelial function, and muscle angiogenesis in health and disease, but these effects in COPD are inconsistent. The elastase and lipopolysaccharide (LPS)-induced emphysema (ELA-LPS) model with exacerbation replicates COPD cardiovascular comorbidities. We aimed to assess the impact of aerobic exercise on blood pressure and endothelial determinants in this model. Male Wistar rats were assigned to four groups: controls (Ctrl)

The SAVE FGR study: Short term variation Analysis versus Visual Evaluation of cardiotocography in early-onset Fetal Growth Restriction to trigger expedited birth - study protocol for a stepped wedge cluster randomized trial.

BACKGROUND: Early-onset fetal growth restriction (eoFGR) is associated with high risks of adverse outcomes and a key dilemma in clinical obstetrical practice in determining the optimal timing of birth. Cardiotocography (CTG) is an important tool for antepartum fetal surveillance and typically assessed visually. Computerised CTG including short term variation (STV) analysis offers an objective analysis by quantifying the beat-to-beat variation in the fetal heart rate. Currently, there is a lack of randomized studies of sufficient power that examine whether birth intervention based on such quant

Impact of Diabetes Mellitus on Left Ventricular Mechanics and Long-Term Outcome in Patients with Hypertrophic Cardiomyopathy.

BACKGROUND: Type 2 diabetes mellitus (T2DM) intensifies the clinical symptoms of heart diseases and leads to a worse prognosis in heart failure patients. Although hypertrophic cardiomyopathy (HCM) and DM rarely co-occur, particularly in older individuals, the impact of DM on cardiac function and outcomes in individuals with HCM remains insufficiently understood. METHODS: A total of 421 HCM patients were included and followed up in a prospective cohort study (mean 68.7 months). In the diabetic HCM group (n = 47), patients had a mean age of 47 ± 17 years, and 31 (66%) were male, while the non-d

Application of intranasal dexmedetomidine in magnetic resonance imaging of preterm infants: The ED50, efficacy and safety analysis.

BACKGROUND: Infants undergoing magnetic resonance imaging (MRI) often require pharmacological sedation. Dexmedetomidine serves as a novel sedative agent that induces a unique unconsciousness similar to natural sleep, and therefore has currently been used as the first choice for sedation in infants and young children. OBJECTIVE: To determine the 50% effective dose (ED50) and 95% confidence interval (95%CI) of intranasal dexmedetomidine for MRI in preterm and term infants, and to observe the incidence of adverse events. To explore whether there were differences in ED50 and 95%CI, heart rate (HR

A Single-Center Review of Clinical Outcomes After Transcatheter Relief of Superior Vena Cava Stenosis in a Pediatric and Young Adult Population.

BACKGROUND: The suspected etiology of superior vena cava (SVC) obstruction is predominantly iatrogenic. Transcatheter interventions relieve stenosis with good long-term results, although there is a paucity of published data in pediatrics. We aim to describe patient characteristics and clinical outcomes after transcatheter SVC intervention at a single-center pediatric quaternary care center. METHODS: Single-center, retrospective study of all pediatric and young adult patients who underwent transcatheter SVC intervention from December 2006 to January 2020. Prevalence of autonomic dysreflexia during spinal cord stimulation after spinal cord injury.

Over the past decade, clinical trials have shown that spinal cord stimulation can restore motor functions that were thought to be permanently impaired in persons with spinal cord injury. However, the off-target effects of delivering electrical impulses to intertwined spinal networks remain largely unknown. This generates safety concerns for this otherwise fast-progressing technology. Herein, we present the prevalence of autonomic dysreflexia (AD) that occurred during implanted spinal cord stimulation testing for motor activation of the lower extremities. Eleven participants with spinal cord in

Sex differences in myocarditis hospitalizations: Rates, outcomes, and hospital characteristics in the National Readmission Database.

Inflammation of the myocardium, or myocarditis, presents with varied severity, from mild to life-threatening such as cardiogenic shock or ventricular tachycardia storm. Existing data on sex-related differences in its presentation and outcomes are scarce. Using the Nationwide Readmission Database (2016-2019), we identified myocarditis hospitalizations and stratified them according to sex to either males or females. Multivariable regression analyses were used to determine the association between sex and myocarditis outcomes. The primary outcome was in-hospital mortality, and the secondary outcom
