Heterotopic Ossification around the Elbow Revisited.

Heterotopic ossification (HO) is the process of ectopic bone formation in the periarticular soft tissues and is usually formed in the elbow, hip and knee joint as a complication of trauma, burns, brain injury or surgical procedures. The development of HO around the elbow joint can cause a severe limitation of range of motion (ROM) and may affect daily activities of the patient. Treatment of ectopic bone formation around the elbow is a challenge for many surgeons. Non-operative treatment usually fails to restore the ROM of the elbow joint; thus, surgery is necessary to restore the function of t

A predictive score of high tibial osteotomy survivorship to help in surgical decision-making: the SKOOP score.

INTRODUCTION: The high tibial osteotomy (HTO) survival rate is strongly correlated with surgical indications and predictive factors. This study aims to assess HTO survival in the long term, to determine the main predictive factors of this survival, to propose a predictive score for HTO based on those factors. METHODS: This multicentric study included 481 HTO between 2004 and 2015. The inclusion criteria were all primary HTO in patients 70 years old and younger, without previous anterior cruciate ligament injury, and without the limitation of body mass index (BMI). The assessed data were preop

Biomechanics associated with tibial stress fracture in runners: A systematic review and meta-analysis.

BACKGROUND: Tibial stress fracture (TSF) is an overuse running injury with a long recovery period. While many running studies refer to biomechanical risk factors for TSF, only a few have compared biomechanics in runners with TSF to controls. The aim of this systematic review and meta-analysis was to evaluate biomechanics in runners with TSF compared to controls. METHODS: Electronic databases PubMed, Web of Science, SPORTDiscus, Scopus, Cochrane, and CINAHL were searched. Risk of bias was assessed and meta-analysis conducted for variables reported in 3 or more studies. RESULTS: The search ret

Lower impact forces but greater burden for the musculoskeletal system in running shoes with greater cushioning stiffness.

In a recent randomised trial investigating running shoe cushioning, injury risk was greater in recreational runners who trained in the shoe version with greater cushioning stiffness (Stiff) compared to those using the Soft version. However, vertical impact peak force (VIPF) was lower in the Stiff version. To investigate further the mechanisms involved in the protective effect of greater cushioning, the present study used an intra-subject design and analysed the differences in running kinematics and kinetics between the Stiff and Soft shoe versions on a subsample of 41 runners from the previous

Orthopedic surgeons and physical therapists differ regarding rehabilitative needs after lower extremity fracture repair.

BACKGROUND: Little evidence is available to guide physical therapy (PT) following lower extremity fracture repair distal to the hip. As such, variability has been reported in the way PT is utilized post-operatively. Examination of current practice by orthopedic surgeons (OS) and physical therapists is needed to inform clinical practice guidelines in this area. OBJECTIVE: To describe current PT referral practices among OS, identify patient and clinical factors that affect PT referral, and examine differences between OS and physical therapists with regard to visit frequency, duration, and use o

Lead limb loading during a single-step descent in persons with and without a transtibial amputation in the trailing limb.

BACKGROUND: Decreased mechanical work done by the trailing limb when descending a single-step could affect load development and increase injury risk on the leading limb. This study assessed the effect of trailing limb mechanics on the development of lead limb load during a step descent by examining individuals with unilateral transtibial amputations who are known to exhibit reduced work in the prosthetic limb. METHODS: Eight amputees and 10 able-bodied controls walked 5 m along the length of a raised platform, descended a single-step of 14 cm height, and continued walking. The intact limb of

The V-effect in fixation of intertrochanteric fractures with proximal femoral nails.

INTRODUCTION: Although one of the optimal treatment methods for fixing intertrochanteric femur fractures (ITFF) is the proximal femoral nail (PFN) that still has some complications, during implantation of PFN, there might be a V-effect on the trochanteric fracture line, which leads to hinging from the inferior of the femoral neck of the fracture and consequently, fixation of the hip in a varus position. The aim of this study was to identify the causes of the V-effect, and possible preventative solutions. HYPOTHESIS: The V-effect is seen at a considerable rate and is an important iatrogenic co

Co-producing a multi-stakeholder Core Outcome Set for distal Tibia and Ankle fractures (COSTA): a study protocol.

BACKGROUND: Ankle fracture is a common injury with a strong evidence base focused on effectiveness of treatments. However, there are no reporting guidelines on distal tibia and ankle fractures. This has led to heterogeneity in outcome reporting and consequently, restricted the contribution of evidence syntheses. Over the past decade, core outcome sets have been developed to address this issue and are available for several common fractures, including those of the hip, distal radius, and open tibial fractures. This protocol describes the process to co-produce-with patient partners and other key

A quality improvement project to assess and refine the handover process at morning trauma meetings.

BACKGROUND: Poor handover and inadequate transmission of clinical information between shifts can result in patient harm. This study was designed to evaluate the impact of implementing a handover protocol on the quality of information exchanged in the trauma handover meetings in a UK district general hospital. METHODS: A prospective single centre observational study was performed at an acute NHS trust, using the define, measure, analyse, improve and control (DMAIC) methodology. Ten consecutive weekday trauma meetings, involving 43 patients, were observed to identify poor practices in handover.

Non-invasive early detection of failure modes in total hip replacements (THR) via acoustic emission (AE).

Total hip replacements (THR) are becoming an common orthopedic surgucal procedure in the United States (332 K/year in 2017) to relieve pain and improve the mobility of those that are affected by osteoarthritis, ankylosing spondylitis, or injury. However, complications like tribocorrosion, or material degradation due to friction and corrosion, may result in THR failure. Unfortunately, few strategies to non-invasively diagnose early-stage complications are reported in literature, leading to implant complications being detected after irreversible damage. Therefore, the main objective of this stud

Clinical Outcomes of Intramedullary and Extramedullary Fixation in Unstable Intertrochanteric Fractures: A Randomized Clinical Trial.

BACKGROUND: The best method for repairing intertrochanteric fractures is still controversial. The fixation methods include extramedullary (EM) and intramedullary (IM). Studies that compare IM and EM fixations for unstable hip fractures are rare. In this study, our goal was to compare the efficacy of EM and IM fixation in treatment of unstable intertrochanteric fractures. METHODS: A total of 113 patients with unstable intertrochanteric were randomized in this cohort study between March 2016 and June 2018 in trauma center of Kashani and Alzahra Hospitals, Isfahan, Iran. Pressure Ulcer Prevalence by Level of Paralysis in Patients With Spinal Cord Injury in Long-term Care.

OBJECTIVE: Scientific literature suggests pressure ulcer (PU) risk increases as immobility increases, indicating that more extensive paralysis confers a greater risk of PU. Yet the specific level of paralysis (ie, hemiplegia vs paraplegia vs quadriplegia), apart from neurodegenerative diagnoses, has never been examined in the long-term care (LTC) population. This study examined the prevalence of PU among LTC residents with different paralysis levels. METHODS: The authors conducted a secondary data analysis of the 2012 US Minimum Data Set of LTC facilities (n = 51,664 residents). Measures incl

Outcomes of Combined Neck and Trochanter Fractures of the Femur Treated with Cephallomedullary Nail in Elderly.

PURPOSE: Proximal femur fractures are classified into intracapsular neck fractures and extracapsular trochanteric fractures, and several related treatment recommendations in elderly patients have already been introduced. Importantly, we have observed cases of combined intra and extracapsular fractures (i.e., ipsilateral neck and trochanter fractures). The purpose of this study is to report the outcomes of combined neck and trochanter fractures of the femur treated with cephalomedullary nail (CMN) in elderly patients. MATERIALS AND METHODS: From January 2010 to December 2014, 410 patients with

Convincing long-term results independent from the postoperative leg alignment following cementless total knee arthroplasty.

BACKGROUND: Cementless primary total knee arthroplasty shows numerous advantages compared with cemented implants (e.g., shorter operation time, preservation of the bone stock). Up to now an increasing number of clinical long-term studies exist. Despite this fact, there is no evidence about the influence of the postoperative leg alignment on the results of cementless knee arthroplasty. There is no work on the clinical outcome of the specific implant, which was used in this study (VANGUARD®, ZimmerBiomet). The purpose of this study was to assess the clinical and radiological long-term results af

The value of musculoskeletal ultrasound in geriatric care and rehabilitation.

The WHO reports that one of the major chronic conditions affecting the elderly worldwide is musculoskeletal disorders that are associated with long-term pain and disability. Considering the healthcare needs of the elderly (i.e. comprehensive, accessible, efficient) and the advantages of ultrasound (US) use (patient-friendly, convenient, cost-effective, and does not require exposure to radiation or magnetic fields), there seems to be a 'gap' in the actual clinical practice. In this paper, we aimed to highlight the potential value of US imaging in the management of the elderly with a wide spectr

The Effect of Age on Technique Variability and Outcome Variability during a Leg Press.

The aim of this study was to determine the effect of aging on power generation and joint coordination during a leg press, in order to increase understanding of how functional movements are affected during the aging process. 44 older and 24 younger adults performed eight sub-maximal power repetitions on a seated leg press dynamometer. Peak power and velocity (at 40% maximum resistance) were measured along with the coordination (coupling angle) of the lower limb joints using the vector coding technique. The younger adults produced significantly greater peak power than the older adults (mean ± SD
