Acute necrotizing ulcerative tonsillitis and gingivitis (Vincent's infections).

The cases of two patients with acute necrotizing ulcerative infections are reported. One had involvement of the tonsil and the other, of the gingivae. In both cases a Gram stain revealed abundant fusiform rods and spirochetes. Both patients responded to treatment with oral penicillin, showing improvement and/or healing in three to six days. These two cases demonstrate the usefulness of the Gram stain in supporting a diagnosis which would otherwise rest solely on clinical grounds.

The etiology of anorectal infections in homosexual men.

The infectious etiology of symptomatic anorectal disease was studied in 52 homosexual men who did not have gonococci on initial Gram stain of anorectal exudate. Herpes simplex virus (HSV) was isolated from the anal canal or rectum in 15 of the 52 (29 percent) men and characteristically caused severe anorectal pain and focal ulcerations visible on sigmoidoscopy. Despite negative initial Gram stains, seven men (14 percent) had anorectal gonococcal infection. Six (12 percent) had syphilis, including two with dark-field positive anal lesions. Four were infected with enteric pathogens, including Gi

Genetic resistance to Trypanosoma congolense infections in mice.

The mechanisms of genetic resistance or "trypanotolerance" to infection with Trypanosoma congolense were investigated in two strains of mice. One strain C57BL, is outstandingly resistant to most stabilates of T. congolense and can survive for over 80 days, whereas CFLP, in common with most other strains, generally succumbs in less than 20 days. Evaluation of several pathophysiological and immunological parameters showed that after infection both strains initially developed similar levels of parasitemia, anemia, biochemical derangement, and immunosuppression. The most outstanding difference was

Heat treatment for certain chronic granulomatous skin infections.

Four cases of chronic granulomatous skin infections (two due to Sporothrix schenckii and two to Mycobacterium marinum) were treated primarily by the intermittent application of local hyperthermia. This treatment was initiated either because of intolerance to conventional iodide therapy for sporotrichosis or as interim therapy while awaiting diagnosis of the mycobacterial infections. The response indicated that the application of heat is a useful adjunct to the therapy of these infections. Our experience, and that of others, suggests that in some cases heat alone may be curative.

[Hypotheses on the dynamic ecology of Loa infections].

In the light of the known data about epidemiological cycles of human and simian loiasis in Africa, and the eventual relationships between these infections, a hypothetical schema is presented, giving a whole view of the dynamic ecology of the Loa systems. This schema involves a primitive simian savannah cycle, from which, the filaria passing from monkey to man, a human savannah cycle, and then the classical human forest cycle are developed. The primitive savannah cycle could also be at the origin of simian forest cycles, but the exchanges between monkeys and man seem very unlikely inside the fo…

[Slow virus infections of the central nervous system].

The major slow viral infections discovered in man manifest themselves in chronic neurological disease. Studies of these diseases have allowed a better understanding of the mechanisms of virus persistence in the central nervous system. Findings are summarized from recent investigations on such slow viral infections as subacute sclerosing panencephalitis, progressive multifocal leukoencephalopathy and the spongiform encephalopathies. From the examples presented it is evident that there is a great diversity of virus-host interactions in slow viral infections of the central nervous system.

[Live vaccines for the prevention of intestinal infections].

Shigella live vaccine was applied orally, four or five times, to 12000 children, aged between one and seven years. Morbidity in the immunised group was 3.5 times as high (9.3 times after boostering) as that in the non-immunised control group. Immunity could be provided for four months, and it was strictly specific of types. Results so far obtained from animal experiments, such as pulmonary and kerato-conjunctival tests on mice, are likely to support the idea of developing a combination vaccine from streptomycin-dependent Sh. flexnerica and Sh. sonnei mutants. Oral immunisation of mice, using 0

Virus infections in children with acute lymphoblastic leukaemia.

The pattern of virus isolation and illness was studied in 64 children with acute lymphoblastic leukaemia (ALL) during periods of apparent infection and when the children were well. The virus isolation rate of 2.2 viruses per child a year is similar to that previously found in normal children. In only 32% of children with symptoms were viruses found and 14.5% had viruses isolated when asymptomatic. The children with ALL appear to be more vulnerable to multiple virus infections and to excrete the virus for longer periods. This may be due to failure of production of both local and systemic antibo…

Management of infections associated with soft contact lens.

This study suggests that there is a higher incidence of infectious corneal ulcers with soft contact lenses than with hard contact lenses. Pseudomonas aeruginosa was the most commonly identified organism associated with soft contact lens corneal ulcers. Early and aggressive management can improve the prognosis of this disease. A plan of therapy and management is recommended and discussed.
