[Peripherally inserted central catheter: description of its use in neonatal and pediatric ICU].

The purpose of this descriptive, retrospective, documental study is to describe the use of peripherally inserted central catheters (PICC) in a neonatal and pediatric intensive care unit regarding their insertion, maintenance, and removal. This study also characterized the population which received the catheter through descriptive and statistical analysis of 176 instruments filled out by nurses, in a two year period. The population attended consisted of 125 patients, mainly premature (43.2%) and male (60%). The basilic and cephalic (43.2%) veins were primarily used for the insertion of a 1.9Fr

Esophageal Atresia and Tracheoesophageal Fistula: A Retrospective Review from a Tertiary Care Institute.

BACKGROUND: A survey of neonates with esophageal atresia and tracheoesophageal fistula (EA ± TEF) to determine additional factors responsible for poor surgical outcomes in our institution where employing an improved standard of care can ameliorate the outcome. MATERIALS AND METHODS: We carried out a retrospective review of 54 neonates, who underwent surgical repair of EA± TEF over a 5-year period. We collected data regarding the patients' demographics, perioperative findings, records of neonatal intensive care, and ascertained the effects of gender, gestational age, birth weight, age at opera

International variations in application of the best-interest standard across the age spectrum.

OBJECTIVE: Ethically and legally, assertions that resuscitation is in a patient's best interest should be inversely correlated with willingness to forego intensive care (and accept comfort care) at the surrogate's request. Previous single country studies have demonstrated a relative devaluation of neonates when compared with other critically ill patients. STUDY DESIGN: In this international study, physicians in Argentina, Australia, Canada, Ireland, The Netherlands, Norway and the United States were presented with eight hypothetical vignettes of incompetent critically ill patients of differen

Intrauterine Adhesion Band Causing Fetal Leg Compression: A Case Report of a Rare Asherman Syndrome Complication in Pregnancy.

Asherman syndrome (AS), characterized by intrauterine adhesions, can lead to various pregnancy complications. We report a case of a 31-year-old woman who was admitted at 29+2 weeks due to preterm premature rupture of membranes at our university hospital. The woman had a history of AS following emergency curettage. Ultrasonography revealed an adhesion band in the lower uterine segment, closely positioned to the fetus's leg. Antibiotic therapy and antenatal corticosteroids were administered to prevent infection and neonatal respiratory distress syndrome. Two weeks later, the woman developed labo

Emergent Bedside Thoracotomy for Decompression of an Incarcerated Neonatal Bochdalek Hernia: A Case Report.

Congenital diaphragmatic hernia (CDH) is a rare and potentially life-threatening condition caused by incomplete diaphragm formation, allowing abdominal organs to herniate into the thoracic cavity. This typically results in pulmonary hypoplasia and, rarely, cardiac compression with subsequent cardiopulmonary collapse. We report the emergency management of a three-week-old term neonate with a previously undiagnosed left-sided Bochdalek hernia who presented at an external hospital with cardiac arrest and mediastinal shift due to massive obstructing enterothorax. Following stabilization and transf

Neuro-multimodal monitoring in neonatal brain injury: Current applications, challenges, and future directions.

Advances in neuromultimodal monitoring (MMM) have significantly enhanced early detection and prognostic evaluation of brain injury in neonatal intensive care. This review summarizes the application and integration of multimodal monitoring technologies for brain injury assessment in newborns. Compared to single-modality approaches, MMM-based combined monitoring strategies demonstrate superior accuracy in both injury identification and prediction of neurodevelopmental outcomes. Meanwhile, artificial intelligence (AI) methods are increasingly being utilized for multi-source data integration and m

Long-Term Peritoneal Dialysis Using a Tenckhoff Catheter in a Premature Infant With Homozygous Autosomal Recessive Polycystic Kidney Disease: A Case Report.

Polycystic kidney disease (PKD) is a clinical condition characterized by the presence of renal cysts and kidney enlargement. When it presents during the neonatal period, it can lead to progressive renal damage and eventually result in end-stage renal failure. Peritoneal dialysis (PD) can be particularly challenging in the neonatal intensive care unit due to the infant's thin abdominal wall and the often unsuitable size and shape of available catheters. As a result, the use of such catheters in infants is associated with high mortality and significant complications, including leakage, peritonit

Neonatal multisystem inflammatory syndrome (MIS-N) associated with prenatal maternal SARS-CoV-2: a single-center experience.

OBJECTIVES: Neonatal Multisystem Inflammatory Syndrome (MIS-N) is believed to result either from the transplacental transfer of maternal SARS-CoV-2 antibodies or from a delayed hyperinflammatory response triggered by the neonate's own immune reaction to SARS-CoV-2 infection. In this study we aim to explore the clinical presentation, laboratory findings, and current approaches to the diagnosis and management of MIS-N in neonates. MATERIAL AND METHOD: A total of 15 infants with MIS-N followed in Bursa Uludag University Neonatal Intensive Care Unit between January 2022 and January 2023 were incl

Neuroleptic Malignant Syndrome in a 10-Month-Old Ex-Preterm Infant with Delirium.

In recent times, atypical antipsychotics are increasingly being used in the neonatal intensive care unit (NICU) for the management of neonatal delirium. As the recognition of delirium in NICU infants increases, caution should be exercised with use of antipsychotics for management, given associated adverse effects. Neuroleptic malignant syndrome (NMS) is a rare adverse drug reaction associated with exposure to antipsychotics and other antidopaminergic medications. Most reported cases of NMS in pediatric patients have been in older children on antipsychotic medications. We present a case of a 10

[Elective induction of labor in nulliparous women : should we stop ?].

In Wallonia, almost one fourth of cesarean sections are performed on nulliparous women with vertex nonanomalous singleton gestations who underwent induction of labor. The purpose of this study is to compare maternal and neonatal outcomes with elective induction of labor versus spontaneous onset of labor. Data for all deliveries at CHU de Liège over a two-year period were obtained. Women with vertex nonanomalous singleton gestations who delivered from 3900 to 40+6 weeks were selected. We tested the association of elective induction and operative vaginal delivery, cesarean section, post-partum h

Ultrasound of acquired posterior fossa abnormalities in the newborn.

Neonatal brain sonography is part of routine clinical practice in neonatal intensive care units, but ultrasound imaging of the posterior fossa has gained increasing attention since the burden of perinatal acquired posterior fossa abnormalities and their impact on motor and cognitive neurodevelopmental outcome have been recognized. Although magnetic resonance imaging (MRI) is often superior, posterior fossa abnormalities can be suspected or detected by optimized cranial ultrasound (CUS) scans, which allow an early and bed-side diagnosis and monitoring through sequential scans over a long period

Bacillus cereus bacteraemia complicated by a brain abscess in a pre-term neonate.

Bacillus cereus is a common laboratory and environmental contaminant. Reports of severe infections are mainly limited to immunocompromised individuals. In reported cases, the time interval between bacteraemia and neuro-invasion appears to be very short, highlighting the importance of rapid and definitive identification and susceptibility testing of invasive B. cereus. We report a case of a neonatal B. cereus bacteraemia complicated by a brain abscess from a neonatal intensive care unit. The neonate presented with bradycardia and desaturations with increased oxygen requirements. Initial blood c

Large-for-Gestational-Age Neonates in Type 1 Diabetes and Pregnancy: Contribution of Factors Beyond Hyperglycemia.

Despite significant reductions in serious adverse perinatal outcomes for women with type 1 diabetes in pregnancy, the opposite effect has been observed for fetal overgrowth and associated complications, such as neonatal hypoglycemia, shoulder dystocia, and admission to the neonatal intensive care unit. In addition, infants born large for gestational age (LGA) have an increased lifetime risk of obesity, diabetes, and chronic disease. Although exposure to hyperglycemia plays an important role, women who seemingly achieve adequate glycemic control in pregnancy continue to experience a greater ris
