[Osteochondritis dissecans of the knee joint. Results of follow-up].

In a long-term follow-up examination (5-15 years postoperatively) clinical and radiological results after surgical treatment of osteochondri … uch as fibrin glue or acylate glue were used. Using the classification of Arcq in 59.6% of the knee joints excellent and in 18.3% good results were obtained. Regarding the development of osteoarthritis in 56% no signs of osteoarthritis were visible. Worst results were obtained in knee joints in which non-resorbable acrylate glue was used for refixation o …

Function of the glenohumeral ligaments in active stabilisation of the shoulder joint.

UNLABELLED: The joint capsules and the glenohumeral ligaments of 12 human shoulder specimens were histologically investigated by light microscopy. Serial sections of 15 microns thickness were cut. The tissue was stained following the haematoxylin-eosin and van Giesson techniques. For specific identification of neural elements we made use of a special silver impregnation technique, described by Novotny, for staining axons in peripheral nerves. Axons of different diameters ranging from 0.2 microns to 70 microns were discovered within the ligaments. Close to the humeral site we found small nerves

[Comparison among the surgical approaches to tuberculosis of sacroiliac joint].

Forty-three cases (44 joints) with tuberculosis of sacroiliac joints were treated surgically through 3 approaches: (1) Anterior approach was indicated for the cases with abscess in iliac fossa or with fistula in buttock, in 21 cases (22 joints). (2) Posterior approach was used for the cases with or without abscess and/or fistula of buttock, in 20 cases. (3) Antero-posterior combined approach was adopted for the cases with large abscess and/or fistula both at iliac fossa and buttock, in 2 cases (2 joints). Follow-up, ranging between 6 months and 16 years, presented no recurrent case of tubercul…

Incidence of temporomandibular joint symptoms following whiplash injury.

Recently there has been considerable litigation involving the development of temporomandibular joint (TMJ) pain and dysfunction following cervical musculoskeletal injury (whiplash). The purpose of this investigation was to interview, examine, and follow up patients with a diagnosis of whiplash injury to determine the incidence of associated temporomandibular disorders. Patients were divided into two categories: those with and those without radiologic evidence of cervical skeletal injury. In the 63 patients with radiographic evidence of cervical skeletal injury (group 1), none had clicking at t

Sacroiliac joint involvement in primary hyperparathyroidism: pathogenetic considerations.

The present report describes the first case of primary hyperparathyroidism with monolateral sacroiliac ankylosis assessed by coronal CT scans. The coexistence of ankylosis with erosive changes, without features of spondyloarthritis or degenerative changes, could be related to hyperparathyroidism.

Epidermoid cyst in the temporomandibular joint after a dermal graft.

A very rare case of epidermoid cyst of the left temporomandibular join (TMJ) after a dermal graft inserted as the interposing material in the surgical correction of TMJ ankylosis is presented. Two years after the surgery, a cystic lesion the size of a walnut was observed in the TMJ region. The cyst contained an odorless, straw-coloured mucoid fluid and a wall lined by stratified squamous epithelium containing orthokeratin. It contained no hair follicles or sweat glands.

Ankylosis of the temporomandibular joint. Follow-up of thirteen patients.

We undertook a postoperative clinical study of 13 patients with ankylosis of the temporomandibular joints. The study consisted of an evaluation of the surgical concepts of resection and subsequent surgical reconstruction by osteotomy in previous height of the joint space and lining of the glenoid fossa with lyophilized dura. Early mobilization and aggressive physiotherapy are mandatory postoperative measures. According to the theory of mandibular growth as a result of functional matrix, early surgical intervention to correct ankylosis should be performed, regardless of the age of the patient, …

A biomechanical study of the collateral ligaments of the proximal interphalangeal joint.

Collateral ligament injuries of the proximal interphalangeal joint are common. A significant number of these injuries result in complete rupture of the ligament. The forces that damage the ligaments are abduction and adduction stresses. Previous studies have investigated laxity, angulation, and patterns of failure, but detailed biomechanical rupture studies are scant. Sixty-eight proximal interphalangeal joints from fresh human cadaver fingers (average age, 67 years) were stressed at velocities of 1 mm/sec, 4 mm/sec, and 10 mm/sec. Sectioning studies were also done. Four distinct rupture patte…

Complex dislocations of the distal radioulnar joint. Recognition and management.

Contrary to most reported series, it has been the authors' experience that dislocations of the distal radioulnar joint (DRUJ) associated with fractures of the forearm are frequently irreducible. This report reviews the authors' experience with these injuries, focusing on the recognition and management of what the authors call "complex" DRUJ dislocations: dislocations characterized by obvious irreducibility, recurrent subluxation, or "mushy" reduction caused by soft tissue or bone interposition. From 1984 until 1989, at the authors' institution, 11 patients were treated for fractures of the rad

[The value of dynamic examination methods in instability of the shoulder joint].

We prospectively examined 25 patients with shoulder instability by clinical examination and by fluoroscopic and sonographic imaging. 14 patients were classified as recurrent, posttraumatic instabilities, 12 of these anterior and two posterior. Clinical examination revealed the direction of instability in 5 patients, fluoroscopy in 5 and sonography in 3 patients. 11 patients had non-traumatic recurrent instabilities. Of these, clinical examination and fluoroscopy demonstrated identical directions of laxity in 8. Sonography concurred with the other techniques in only three cases. We recommend ma

Magnetic resonance evaluation after temporomandibular joint diskectomy.

The aim of this study was to investigate the value of magnetic resonance imaging after diskectomy of the temporomandibular joint. Magnetic resonance images were obtained before and 12 months after unilateral diskectomy without disk replacement. Magnetic resonance findings at follow-up were correlated to residual pain. At the follow-up, 20 of 28 patients were free of pain in the joint that had been surgically treated, four patients had mild pain, and four patients had significant residual pain. The magnetic resonance images at follow-up showed that the joint space was filled with soft tissue af

The clinicoradiologic predictability of perforations of the soft tissue of the temporomandibular joint.

One hundred thirty-one arthrographic examinations of temporomandibular joints were performed on 119 patients. The relationships between clinical symptoms, tomographic findings, and soft-tissue perforation were examined. Perforations of the articular disk or posterior attachment were revealed in 58 joints. Joints with crepitation showed a high frequency of perforation (20 of 22). Pain in the joint area and limitation of opening correlated to disk reduction. Perforations were observed in 12 of 21 joints with continuous pain. Osseous changes were detected in 89 joints. Joints with abnormal radiod…

Acute septic arthritis of the hip joint in infancy and childhood.

Forty-three children with 45 hip joint infections were admitted to the hospital between 1983 and 1988. The duration of symptoms was the single most important prognostic feature. Almost all children who were treated within four days of symptoms had a satisfactory outcome. Concomitant osteomyelitis of the proximal femur produced by far a worse prognosis than if the infection was confined to the synovium of the hip joint. In addition, patients whose hips were infected with Staphylococcus aureus bacteria were more likely to have a poor prognosis than those whose hips were infected with non-S. aure

Clinical evaluation of fleroxacin in the treatment of bone and joint infections.

The objective of this open label, non-comparative study was to evaluate the efficacy and safety of fleroxacin 400mg administered orally once daily to patients with acute osteomyelitis and/or acute septic arthritis. Nineteen patients (10 males and 9 females) were evaluable for the analysis of clinical efficacy and safety. Of these, 7 (36.8%) had osteomyelitis and 12 (63.2%) had septic arthritis. Bacteriological cures were reported in 6 of 7 patients (85.7%) with osteomyelitis and in 8 of 11 patients (72.7%) with septic arthritis. The median duration of treatment for the clinical cures in osteom

CT arthrography and arthroscopy in chronic glenohumeral joint instability.

The authors evaluate the value of CT arthrography in glenohumeral joint instability. A study was conducted on a group of 16 patients with recurrent dislocation of the shoulder. All patients underwent CT arthrography and arthroscopy. The diagnostic accuracy of CT arthrography was rated for different types of lesions on the basis of arthroscopic confirmation of its findings. Our results, which include a statistical analysis, showed a diagnostic accuracy of 97.3% in Bankart lesions and 100% in Hill-Sachs lesions and loose bodies.
