Implementing a Surveillance-Based Approach to Create a Statewide Viral Clearance Cascade for Hepatitis C Among People With HIV and HCV Coinfection in Connecticut.

OBJECTIVES: Highly effective direct-acting antiviral medications have made it feasible to achieve elimination of hepatis C virus (HCV), including for people with HIV and HCV coinfection. The Centers for Disease Control and Prevention offers guidance for a laboratory surveillance-based HCV viral clearance cascade, which allows public health departments to track the outcomes of people with HCV based on the following steps: ever infected, virally tested, initial infection, and cured or cleared. We examined the feasibility of this approach among people with HIV and HCV coinfection in Connecticut.

2021 European Guideline on the management of proctitis, proctocolitis and enteritis caused by sexually transmissible pathogens.

This guideline intents to offer guidance on the diagnosis and management of patients with gastrointestinal symptoms and a suspected sexually transmitted cause. Proctitis is defined as an inflammatory syndrome of the anal canal and/or the rectum. Infectious proctitis can be sexually transmitted via genital-anal mucosal contact, but some also via digital contact and toys. Neisseria gonorrhoeae, Chlamydia trachomatis (including lymphogranuloma venereum), Treponema pallidum and herpes simplex virus are the most common sexually transmitted anorectal pathogens. Shigellosis can be transferred via ora

Antimicrobial susceptibility of methicillin-resistant Staphylococcus pseudintermedius isolated from veterinary clinical cases in the UK.

Staphylococcus pseudintermedius is a leading aetiologic agent of pyoderma and other body tissue infections in dogs and cats. In recent years, an increased prevalence of methicillin-resistant S. pseudintermedius (MRSP) has been reported. Isolation of MRSP in serious infections poses a major therapeutic challenge as strains are often resistant to all forms of systemic antibiotic used to treat S. pseudintermedius -related infections. This study investigates the occurrence of MRSP from a total of 7183 clinical samples submitted to the authors' laboratories over a 15-month period. Identification wa

Beyond Classification: A Unique Case of Self-Resolving Cutaneous Mucinosis Associated With Streptococcal Infection in a Child.

A 3-year-old boy presented with an unusual combination of indurated skin, sclerotic plaques with lichenification, and yellowish papules. Histopathology revealed diffuse dermal mucin deposits, and laboratory tests showed a positive throat culture for Group A streptococcus and elevated serum anti-streptolysin titers. An 10-day course of oral amoxicillin was associated with near-complete resolution of all dermatological findings within 4 months. This case represents a unique presentation of a self-resolving cutaneous mucinosis, likely associated with a streptococcal infection.

Worrying increase in the risk of vertical transmission of syphilis in Croatia, 2020 to 2024.

Four infants potentially exposed to syphilis infection in utero, meeting World Health Organization surveillance criteria of congenital syphilis (CS), were diagnosed in Croatia between September 2020 and January 2024. We conducted a retrospective analysis of epidemiological, clinical and laboratory data of these cases to assess compliance with surveillance case definitions. As only one confirmed CS case has been reported in Croatia in over 2 decades, these reports signal an increased risk of syphilis vertical transmission and warrant strengthening antenatal screening.

Pulmonary Tuberculosis with Concomitant Aspergillus Fungal Ball in a Diabetic Indian Male: A Rare Case Report.

Pulmonary tuberculosis is rampant in some countries. The disease is an outcome of infection by Mycobacterium tuberculosis and is more common in immunocompromised individuals. Furthermore, mycetoma or a fungal ball can develop in cavitary lesions of tuberculosis. The present case is a rare presentation of pulmonary tuberculosis with concomitant Aspergillus fungal ball in a diabetic Indian male. A clinical examination with a strong laboratory and radiological workup helped establish the final diagnosis. The patient was initiated on anti-tubercular chemotherapy and advised lobectomy.

Ulcerative Colitis: Making the Diagnosis.

Ulcerative colitis is an inflammatory condition of the colon. The diagnosis of ulcerative colitis is based on clinical presentation, endoscopic evaluation, and histologic parameters in the absence of demonstrable alternate etiology. The differential diagnosis remains broad, and infection in particular must be considered and excluded. Although laboratory and radiographic findings can aid in the diagnosis of ulcerative colitis, endoscopy remains the gold standard for diagnosis. A correct diagnosis and disease staging are imperative because these factors affect treatment options and prognosis.

Pregnancy, Labor, and Delivery after Ebola Virus Disease and Implications for Infection Control in Obstetric Services, United States.

Many of the survivors of the 2014-2015 epidemic of Ebola virus disease (EVD) in western Africa were women of childbearing age. Limited clinical and laboratory data exist that describe these women's pregnancies and outcomes. We report the case of an EVD survivor who became pregnant and delivered her child in the United States, and we discuss implications of this case for infection control practices in obstetric services. Hospitals in the United States must be prepared to care for EVD survivors.

[Significance of modern methods for laboratory detection of rabies agents and identification of the zoonose immunological survey].

Analytical review of modern methods of the laboratory detection of rabies and findings of our research indicate high sensitivity and specificity of methods for rapid identification of rabies agents, such as ELISA, reverse-transcriptase PCR for identification of the rabies virus genome, and rabies virus isolation in rat Gasserian ganglion neurinoma, as well as their potential to be included into the State Quality Standard for early detection of rabies in animals to reduce the infection risk among humans and animals.

A case of chronic active Epstein-Barr virus infection mimicking adult-onset Still's disease.

An 83-year-old man was diagnosed with adult-onset Still's disease (AOSD) based on clinical and laboratory findings. However, glucocorticoid had little effect. Epstein-Barr virus (EBV)-DNA was detected in peripheral blood, and autopsy findings confirmed a diagnosis of chronic active EBV infection (CAEBV). CAEBV mimics AOSD, and the presence of articular involvement and leukocytosis does not exclude the possibility of CAEBV. CAEBV should be included in the differential diagnosis of AOSD, and measurement of EBV-DNA is essential.

Rubella vaccination of unknowingly pregnant women during 2006 mass campaign in Argentina.

We report a prospective study of 56 pregnant women inadvertently vaccinated with rubella vaccine during the 2006 campaign performed in Argentina. Of these patients, 48 (87%) were immune, whereas the remaining 9 (16%) were susceptible. In the latter group, 7 presented with a primary reaction to the vaccine confirmed through immunoglobulin (Ig) G antibody avidity testing or seroconversion of IgG titers. During the clinical and laboratory follow-up, newborns did not present evidence of infection or malformations compatible with congenital rubella syndrome.

Multifocal osteonecrosis in HIV/AIDS: a case report.

Many patients with Human Immunodeficiency Virus (HIV) Infection now live long due to the availability of antiretroviral drugs. This has resulted in many hitherto unknown complications being reported. We report a 56-year-old trader who presented with a seven-month history of pain in both shoulders and hips. Laboratory investigations and x-rays confirmed oesteonecrosis of both humeral heads and femoral heads in HIV/AIDS. She is presently on antiretroviral drugs and conservative management of her multiple joint disorders.

Legionella pneumonia presenting with rhabdomyolysis and acute renal failure: a case report.

Legionella pneumophila infection is a recognized but rare cause of rhabdomyolysis. While the mechanism of rhabdomyolysis associated with Legionella is unknown, theories include direct invasion of Legionella into the muscle itself, or the release of its endotoxin into the circulation with subsequent muscle injury. In this case report, we describe a case of Legionella pneumonia presenting as altered mental status, high fever, and rhabdomyolysis with acute renal failure. We review the epidemiology, clinical and laboratory findings, and treatment of the disease.

Fulminant cerebral malaria in a Swiss patient.

Malaria remains the most important parasitic disease worldwide. Falciparum malaria is a medical emergency and requires immediate diagnosis and treatment. Cerebral malaria is a rapidly progressive, potentially fatal complication of Plasmodium falciparum infection. This case, including post-mortem observations, histology, and laboratory diagnosis, emphasizes the necessity of appropriate advice regarding malaria prophylaxis before travel to an endemic area. Malaria should always be considered in the differential diagnosis of patients presenting with fever and/or nonspecific flu-like symptoms afte

[Diagnostic problems with Borrelia burgdorferi infection in children].

Lyme borreliosis has become a serious diagnostic and therapeutic problem of modern medicine. The occurrence of the disease in Europe, and also in Poland, has recently increased. Diagnostic problems with Borrelia burgdorferi infection result from non-characteristic course of the disease in children, without the 1 st stage of the disease (erythema migrans), negative history forwards tick bite, lack of seasonal changes in occurrence and difficulties with interpretation of serological tests results. Direct laboratory investigations are also of little relevance.

Possible undetected outbreaks of cryptosporidiosis in areas of the north west of England supplied by an unfiltered surface water source.

We report a ten-year retrospective analysis of laboratory reports of cryptosporidium infection in the North West of England. Weekly report data from six health authorities known to have been affected by outbreaks associated with a single supply were compared with data from other health authorities in the North West. Following graphical representation of report rates, it would appear that outbreaks in the six health authorities were considerably more common than the average recorded in the national outbreak surveillance system.
