The effect of infraorbital nerve block on emergence agitation in children undergoing cleft lip surgery under general anesthesia with sevoflurane.

BACKGROUND: Sevoflurane is widely used in pediatric anesthesia. However, a high incidence of emergence agitation (EA) after general anesthesia with sevoflurane has been reported and pain has been regarded as a significant contributing factor. The objective of this prospective randomized, placebo-controlled trial was to determine whether infraorbital nerve block reduces EA in children undergoing repair of cleft lip after sevoflurane. METHODS: In this randomized, placebo-controlled trial, we enrolled 110 children (5 months to 6 years of age), who were scheduled for cleft lip surgery, and random…

A genome-wide study of de novo deletions identifies a candidate locus for non-syndromic isolated cleft lip/palate risk.

BACKGROUND: Copy number variants (CNVs) may play an important part in the development of common birth defects such as oral clefts, and indiv … equently among oral cleft cases than controls. We also observed wider de novo deletions among cleft lip and palate (CLP) cases than seen among cleft palate (CP) and cleft lip (CL) cases. CONCLUSIONS: This study presents a region where de novo deletions appear to be involved in the etiology of oral clefts, although the underlying biological mechanisms …

Simultaneous correction of deviated columella and wide nostril floor using the Y-V advancement in unilateral cleft lip nasal deformities.

Here, we describe an operative method for the simultaneous correction of deviated columella and wide nostril floor in unilateral cleft lip nasal deformities. The V-shaped flap of the affected nostril floor was inserted into the side wall of the columella using the Y-V advancement. By suturing the anterocaudal septum, which is often deviated to the unaffected side, with the subcutaneous tissue of the affected alar base, it was possible to simultaneously reposition the columellar base in the midline as well as narrow the wide nostril floor. This method was technically simple, with low invasivene…

Surgical treatment modality for facial esthetics in an obstructive sleep apnea patient with protrusive upper lip and acute nasolabial angle.

For patients with severe obstructive sleep apnea syndrome (OSAS), maxillomandibular advancement (MMA) offers a good treatment option by physically expanding the skeletal framework. However, facial esthetics can be aggravated by MMA in patients with OSAS who have a normally positioned maxilla, a protrusive upper lip, and an acute nasolabial angle. Therefore, surgical treatment planning should be customized according to diverse skeletodental and soft-tissue patterns to produce a favorable change in facial esthetics and sleep function in patients with OSAS. In this case report, good treatment res…

Transverse fascial suspension with muscle bow traction: advantages for full-thickness lip reconstruction involving the oral commissure using free flap.

Large full-thickness oral defects involving the oral commissure continue to be a challenge for reconstructive surgeons. Although local flaps are the best option for full-thickness lip reconstruction, they are unavailable for large defects. In particular, recent advances in microsurgery have extended the available surgical options using free flaps, but for full-thickness large oral defects involving the oral commissure, it is still difficult to obtain good function and competence. The major disadvantages are the drooping and loosening of the reconstructed lip and the difficulty in restoring a n…
