Magnesium is the second most abundant intracellular cation and is a fundamental cofactor in a multitude of cellular enzymatic reactions. Magnesium deficiency causes diverse clinical features predominantly due to cardio- and neurotoxicity. We describe a case of severe hypomagnesaemia associated with intermittent downbeat nystagmus, cerebellar ataxia, generalised convulsions and a supraventricular tachycardia. On MRI imaging, a transient lesion of the cerebellar nodulus was observed, which has not, to our knowledge, been previously described in isolated hypomagnesaemia.

Magnesium deficiency is a common clinical problem. Dietary malabsorption and renal wasting are the commonest causes. Hypomagnesemia induces a state of functional hypoparathyroidism due to decreased secretion of parathormone (PTH) as well as resistance to its action. Contrary to typical autoimmune hypoparathyroidism, it is associated with decreased levels of serum phosphate. We report a patient who presented with hypocalcemic tetany associated with hypomagnesaemia due to renal wasting. Subsequently, he improved with magnesium supplementation of therapy.

Long-term use of proton pump inhibitors can lead to serious hypomagnesaemia. Intestinal magnesium absorption takes place by passive paracellular and active transcellular transport. It has been hypothesized that proton pump inhibitors impair the active transcellular magnesium transport. The resulting hypomagnesaemia may cause hypoparathyroidism, hypocalcaemia and hypokalaemia. Proton pump inhibitor-induced hypomagnesaemia is reversible: it resolves when proton pump inhibitors are stopped. The indication for long-term proton pump inhibitor treatment should be evaluated periodically.

An ion-exchange procedure is described for the analysis of dolomite, limestone and chrome-iron ore. In dolomite and limestone, calcium and magnesium are separated by eluting with 1M acetate buffer (pH 6) ; in chrome-iron ore, chromium(VI) is first separated from other metals by passage through a cation-exchanger, Dowex 50W-X8 (H+ form), when chromium(VI) is not adsorbed. Iron, aluminium and magnesium are subsequently recovered from the resin and are determined by conventional methods.

Association of hypomagnesemia and neurological deficits is a well documented issue, but despite its importance, magnesium levels are rarely tested during the acute phase of a stroke. The struvite crystallization process can recover struvite crystals as a valuable slow-release fertilizer from the side stream of wastewater treatment plants (WWTPs). The purpose of this study is to demonstrate the crystal growth characteristics and determine the appropriate recovery criteria for a struvite crystallization pilot plant.
