Granulomatous disorders are frequently due to a wide variety of infections. Over the past decade advances in molecular diagnostic techniques have allowed identification of organisms involved in granulomatous disorders that previously were of unknown etiology. BACKGROUND: Fungal sinusitis typically occurs in immunocompromised patients. The authors report four cases of fungal sinusitis in immunocompetent young adults, all of whom had proptosis.

Labile toxin producing enterotoxic E. coli (ETEC) were the commonest pathogen isolated from diarrheal stools of hospitalized children (21%) and adults (26%) in Singapore. In a retrospective analysis, we studied the association between ocular atopy and keratomycosis. Forty-one (26.3%) of 156 patients with fungal corneal ulcers were found to have preexisting allergic conjunctivitis. All patients had been using a combination of antibiotics and corticosteroids before developing the corneal ulcer. Fungus isolation on Sabouraud's agar medium was obtained in 74.8% (31/42) ulcers. We have studied amphotericin B concentrations in tissues of 13 cancer patients who died after having received 75 to 1,110 mg (total dose) of amphotericin B-deoxycholate for suspected or proven disseminated fungal infection. Tears were collected from patients with familial amyloidotic polyneuropathy (FAP), orbital tumors, sarcoidosis, trigeminal and facial nerve palsy and corneal ulcer. The development and course of blood diseases in the aged often are determined by two factors: The first is the disturbance of the immune system based on a defective interaction between the T- and B-cell system. These are due to the involution of the thymus gland accompanied by reduced incretion of thymus hormones. The second factor consists in a regressive alteration of a "haemopoietic inductive micro-environment (Trentin), which has its morphological substrate in the circulatory system and the stroma of the bone marrow. Both factors lead to atrophic or diminished recovery of the bone marrow.

We have reviewed 58 cases of bone marrow granuloma at a single institution over a 20-year time span, and have summarized the available English literature. We conclude that bone marrow granulomas are an infrequent pathologic finding which, when found, require definition as to an underlying etiology. Undoubtedly, the illnesses associated with marrow granuloma are similar to those causing granulomatous hepatitis. The following additional statements may justifiably be made based on this review.
