Fostering Infant Responsivity and Reciprocity - Support to Thrive (FIRRST): a multisite randomized control trial of a pre-emptive telehealth intervention for infants with early signs of Autism Spectrum Disorders in Italy.

BACKGROUND: Early prodromal signs of Autism Spectrum Disorder (ASD) can be detected within the first year of life, a period of heightened neuroplasticity. Pre-emptive, parent-mediated interventions delivered during this window may improve developmental outcomes and reduce ASD symptom severity, but evidence from adequately powered randomized trials in Europe remains limited. Protocol for a pragmatic randomized controlled trial evaluating a care pathway incorporating ultrasound-guided platelet-rich plasma injection plus physiotherapy versus physiotherapy alone in patients with L5/S1 lumbar disc herniation.

BACKGROUND: Lumbar disc herniation (LDH) causes low back pain and lower-limb neurological symptoms. Conservative and surgical treatments have similar outcomes and recurrence rates within two years. Anti-inflammatory drugs or opioids reduce inflammation but are unsuitable for long-term use. Physiotherapy improves mobility and function, but has limited evidence for reversing disc herniation. Platelet-rich plasma (PRP) is emerging as a regenerative therapy for LDH. Combining ultrasound-guided PRP with McKenzie therapy may enhance disc repositioning, tissue repair, and sustained recovery. This pra

The neural-physiological coupling mechanism underlying positive emotion sharing boosting interpersonal connection among strangers.

Emotional sharing plays a crucial role in boosting interpersonal connections among strangers. The present study aims to explore the interpersonal emotion regulation and social connectedness function of emotional sharing among strangers and the underlying neural-physiological coupling mechanism. Thirty-five stranger dyads (sharer vs. listener) completed the emotion sharing task (positive, neutral and negative) in which sharers recalled emotional events then shared their emotions to the listener. We recorded each pair of participants' brain activities (using functional near-infrared spectroscopy

Characteristics of maintenance of wakefulness test in drug-naïve patients with narcolepsy type 1 and type 2, and relationship with other measures of sleepiness.

STUDY OBJECTIVES: We aimed to describe the characteristics of standard maintenance of wakefulness test (MWT), outside of clinical trials, in a sample of drug-naïve patients with narcolepsy type 1 (NT1) and type 2 (NT2). METHODS: Consecutive drug-naïve patients with narcolepsy underwent two days of continuous PSG recording, the multiple sleep latency test (MSLT), then night-PSG and, on the following day, MWT. MWT results were correlated with MSLT and Epworth sleepiness scale (ESS). Patients in the two lower tertiles of MWT mean sleep latency (mSL) were compared to those in the upper tertile.

Using cortical auditory-evoked potential responses to verify the objective benefit of aural rehabilitation in adult cochlear implant users.

INTRODUCTION: Cortical auditory-evoked potential (CAEP) responses are used clinically to assess auditory processing and support cochlear implant (CI) mapping procedures post-implantation. This study aims to examine CAEP responses as an objective measure of neurophysiological changes following a structured Aural Rehabilitation Program (ARP) in long-term adult CI users, with poor baseline speech perception performance. Advancing our understanding of the mechanisms underlying improved post-implantation outcomes in poor performing CI users may inform the development of targeted interventions and r

Neurofeedback as an emerging rehabilitation strategy for Parkinson's disease: a narrative review.

BACKGROUND: Parkinson's disease (PD) is a progressive neurodegenerative disorder characterized by motor and non-motor symptoms associated with dysfunction of cortico-basal ganglia-thalamo-cortical networks. Because current treatments remain largely symptomatic, there is increasing interest in adjunctive rehabilitation strategies capable of targeting disease-relevant neural activity more directly. Neurofeedback (NF), delivered through electroencephalography (EEG), functional magnetic resonance imaging (fMRI), or deep brain stimulation (DBS)-guided platforms, has emerged as a potential self-regu

Nerve ultrasound, neuronopathy and cough predict sensory neuropathy patients with RFC1 expansions.

The finding of biallelic pathogenic pentanucleotide RFC1 expansions has extended the spectrum of disease in cerebellar ataxia, neuropathy and vestibular areflexia syndrome. It is clear that for many, a sensory neuropathy is an early feature and raises the question of how to identify which patients with this common neurophysiological presentation should be tested genetically for the condition. We identified patients with idiopathic, sensory predominant neuropathies who had attended the Neurophysiology Department of Auckland Hospital for nerve conduction studies. We undertook a systematic clinic…

Methylphenidate Reversal of Dexmedetomidine-Induced Versus Ketamine-Induced Sedation in Rats.

BACKGROUND: Dexmedetomidine and ketamine have long elimination half-lives in humans and have no clinically approved reversal agents. Methylphenidate enhances dopaminergic and noradrenergic neurotransmission by inhibiting reuptake transporters for these arousal-promoting neurotransmitters. Previous studies in rats demonstrated that intravenous methylphenidate induces emergence from isoflurane and propofol general anesthesia. These 2 anesthetics are thought to act primarily through enhancement of inhibitory Gamma-aminobutyric acid type A (GABA A ) receptors. In this study, we tested the behavior

Precuneus magnetic stimulation for Alzheimer's disease: a randomized, sham-controlled trial.

Repetitive transcranial magnetic stimulation (rTMS) is emerging as a non-invasive therapeutic strategy in the battle against Alzheimer's disease. Alzheimer's disease patients primarily show alterations of the default mode network for which the precuneus is a key node. Here, we hypothesized that targeting the precuneus with TMS represents a promising strategy to slow down cognitive and functional decline in Alzheimer's disease patients. We performed a randomized, double-blind, sham-controlled, phase 2, 24-week trial to determine the safety and efficacy of precuneus stimulation in patients with

Typical and Atypical Development of Visual Expertise for Print as Indexed by the Visual Word N1 (N170w): A Systematic Review.

UNLABELLED: The visual word N1 (N170w) is an early brain ERP component that has been found to be a neurophysiological marker for print expertise, which is a prelexical requirement associated with reading development. To date, no other review has assimilated existing research on reading difficulties and atypical development of processes reflected in the N170w response. Hence, this systematic review synthesized results and evaluated neurophysiological and experimental procedures across different studies about visual print expertise in reading development. Literature databases were examined for r

Utility of intraoperative electromyography in placing C7 pedicle screws.

OBJECTIVE: The C7 vertebral body is morphometrically unique; it represents the transition from the subaxial cervical spine to the upper thoracic spine. It has larger pedicles but relatively small lateral masses compared to other levels of the subaxial cervical spine. Although the biomechanical properties of C7 pedicle screws are superior to those of lateral mass screws, they are rarely placed due to increased risk of neurological injury. Although pedicle screw stimulation has been shown to be safe and effective in determining satisfactory screw placement in the thoracolumbar spine, there are f

Facilitation of biological motion processing by group-based autism specific social skills training.

Abnormalities in neurophysiological correlates of social perception are a well-known feature of autism spectrum disorder (ASD). However, little is known if and how ASD specific behavioral interventions may affect neural processing in ASD. The aim of the current study was to investigate for the first time, whether the group-based social skills training SOSTA-FRA would elicit changes in neurophysiological correlates of social perception in high-functioning ASD individuals aged 8-17 years. Event-related potentials (ERPs) of a facial emotion recognition (FER) and a biological motion perception tas

Peripheral neuropathy predicts nuclear gene defect in patients with mitochondrial ophthalmoplegia.

Progressive external ophthalmoplegia is a common clinical feature in mitochondrial disease caused by nuclear DNA defects and single, large-scale mitochondrial DNA deletions and is less frequently associated with point mutations of mitochondrial DNA. Peripheral neuropathy is also a frequent manifestation of mitochondrial disease, although its prevalence and characteristics varies considerably among the different syndromes and genetic aetiologies. Based on clinical observations, we systematically investigated whether the presence of peripheral neuropathy could predict the underlying genetic defe

Lower thoracic degenerative spondylithesis with concomitant lumbar spondylosis.

OBJECTIVE: Degenerative spondylolisthesis of the spine is less common in the lower thoracic region than in the lumbar and cervical regions. However, lower thoracic degenerative spondylolisthesis may develop secondary to intervertebral disc degeneration. Most of our patients are found to have concomitant lumbar spondylosis. By retrospective review of our cases, current diagnosis and treatments for this rare disease were discussed. MATERIALS AND METHODS: We present a series of 5 patients who experienced low back pain, progressive numbness, weakness and even paraparesis. Initially, all of them w

How does the brain rapidly learn and reorganize view-invariant and position-invariant object representations in the inferotemporal cortex?

All primates depend for their survival on being able to rapidly learn about and recognize objects. Objects may be visually detected at multiple positions, sizes, and viewpoints. How does the brain rapidly learn and recognize objects while scanning a scene with eye movements, without causing a combinatorial explosion in the number of cells that are needed? How does the brain avoid the problem of erroneously classifying parts of different objects together at the same or different positions in a visual scene? In monkeys and humans, a key area for such invariant object category learning and recogn
