Olecranon Osteotomy Exposure for Distal Humeral Fracture Treatment.

BACKGROUND: Olecranon osteotomy (OO) is commonly utilized to improve exposure when treating intra-articular distal humeral fractures. A chevron-shaped osteotomy facilitates reduction and increases surface area for healing1. Following distal humeral fracture reduction and fixation, the OO fragment is fixed with a precontoured plate. The OO technique yields comparable outcomes to alternative techniques1,2. DESCRIPTION: The technique is performed as follows. (1) Imaging is reviewed and preoperative planning is performed. (2) The patient is positioned in the lateral decubitus position with the op

Treatment for displaced sustentaculum tali fractures.

BACKGROUND: Sustentaculum tali fractures are rare foot fractures accounting for 1% of all calcaneal fractures, which accounts for about 1-2% of all bone fractures. Since the importance of sustentaculum tali in maintaining the medial foot arch, its fractures should be dealt with anatomical fixation to avoid post-traumatic arthrosis. OBJECTIVE: Is to delineate the importance of sustentacular tali fractures fixation mentioning operative technique and the post-operative outcome. METHODOLOGY: Ten cases with 10 consequent closed sustentaculum tali fractures were treated by ORIF. Nine of them were

The use of dual C-arms during fixation of calcaneal fractures: a technique tip.

UNLABELLED: We describe a simple technique using dual C-arms (large and mini C-arm together) for open reduction internal fixation of calcaneal fractures in the lateral decubitus position that (1) decreases the difficulty of obtaining proper intraoperative imaging; (2) limits C-arm movement, which decreases risk of contamination and operative time; and (3) minimizes the drawbacks of each imaging fluoroscopic modality. LEVELS OF EVIDENCE: Level V, Technical tip, Expert opinion.

[Closed reposition of an acute midfoot luxation fracture in Charcot arthropathy with the ring fixator].

Diabetic neuropathic osteo-arthropathy (DNOAP; Charcot arthropathy) is a progressive disease characterized by joint luxation, fractures and excessive destruction of foot architecture. The operative therapy is indicated when conservative therapy fails, in progressive breakdown, in hindfoot Charcot and in spreading infections due to plantar ulcers. The complication rate of 10-20% in open surgery is high. We present a minimally invasive technique of closed reduction and application of a ring fixator for reconstruction of the foot architecture and arthrodesis of the involved joints.

Surgery for old ankle fracture. 13 cases followed for 9 (5-15) years.

Between 1978 and 1988, we treated 13 cases of neglected ankle fractures by open reduction and internal fixation or osteotomy 11 (2-36) months after the accident. Having been followed for 9 (5-15) years, the results were evaluated. All the patients were improved, even though restoration of the anatomy of the joint often was difficult. Those operated on after less than 6 months had better function than those with longer delays.

[Injuries of the frontal sinus].

The authors report their experience in the treatment of frontal sinus fractures, between 1979 and 1987, about 67 cases. --52% of these trauma involve alone, the anterior and/or posterior wall of the frontal sinus, with or without cerebrospinal fluid rhinorrhea; --the sinus surgery was composed of cranialization in 20 cases (30%) and open reduction with reconstruction, with or without tube to stent the nasofrontal duct, in 30 cases (48%). Two points appear to them, essential to guide the treatment: --osteomeningeal tear, --naso-frontal duct injury.

Treatment of proximal humeral fractures accompanied by medial calcar fractures using fibular autografts: A retrospective, comparative cohort study.

BACKGROUND: Severe proximal humerus comminuted fractures are often accompanied by medial calcar comminuted fractures and loss of medial support, which are important factors that lead to internal fixation failure. The appropriate treatment for proximal humerus comminuted fractures has not been established. Therefore, this study assessed the outcomes of using a fibular autograft with locking plates to treat severe proximal humerus comminuted fractures. AIM: To investigate the outcomes of using a fibular autograft with locking plates to treat severe proximal humerus comminuted fractures. METHOD

On surgical intervention in the temporomandibular joint.

The aim of this thesis was to evaluate the indications for and the results of temporomandibular joint (TMJ) surgery in patients with long-standing severe orofacial pain and dysfunction as well as in patients with fractures of the condylar neck. The patients with long-standing pain and dysfunction had had symptoms for a mean time of 4 years, had been treated conservatively for a mean time of 2.5 years, and had undergone numerous conservative treatment methods without improvement except for a minor increase in mouth opening capacity. The indications for surgery were strict; only 1% or less of al

Beyond Pipkin IV: Management of Femoral Head-neck Fracture-dislocation with Posterior Acetabular Wall and Peritrochanteric Fracture in an Adolescent Using a Modified Kocher-Langenbeck Approach.

INTRODUCTION: Femoral head-neck fracture-dislocation with associated posterior acetabular wall and per-trochanteric fractures represents an extremely rare and severe "beyond Pipkin IV" injury pattern. CASE REPORT: A 14-year-old boy sustained posterior hip dislocation with femoral head, neck, acetabular, and trochanteric involvement following a road traffic accident. Emergency open reduction and internal fixation was performed using a modified Kocher-Langenbeck posterior approach with trochanteric flip. RESULTS: Radiological union was achieved by 3 months. At 1-year follow-up, the patient had

Utility of external fixation for traumatic periprosthetic fracture after total ankle arthroplasty in patients with rheumatoid arthritis: A report of two cases.

This report describes two cases of implant-stable traumatic periprosthetic fractures after total ankle arthroplasty in patients with rheumatoid arthritis. One case with a low body mass index (14 kg/m2) achieved complete bone union with the use of an external fixator, while the other case with a high body mass index (32.83 kg/m2) failed to achieve bone union with the external fixator; however, complete union was achieved utilising secondary internal plate fixation. Although open reduction and internal fixation using a plate is the standard procedure in implant-stable periprosthetic fracture cas

Cruciate Paralysis Following a Displaced Type II Odontoid Fracture: A Case Report.

A 54-year-old male was admitted to our emergency department by air transport after being hit as a pedestrian by a motorcycle. He presented with impaired motor function in the upper extremities bilaterally while sensation was spared. He presented no motor or sensory impairment of the lower extremities. A computed tomography scan revealed a displaced type II odontoid fracture. Treatment consisted of open reduction and internal fixation of the odontoid with a single screw. The patient's functional outcome was excellent during the two-month follow-up. Cruciate paralysis is a relatively rare althou

Elbow instability following lateral collateral ligament complex avulsion fracture and joint interposition: an uncommon presentation.

The lateral collateral ligament (LCL) complex of the elbow is a capsuloligamentous structure, critical for elbow stability. Though isolated ligamentous injuries have been reported in literature, there are no studies reporting avulsion fractures of the lateral ligamentous complex of the elbow with joint incarceration in a child. An 11-year-old boy presented to the emergency department after a fall from height, with pain and swelling to the lateral side of his left elbow. Radiographs established a fracture of the lateral condyle and CT imaging confirmed an avulsion of the lateral ligamentous com

Fixation of a Proximal Humerus Fracture With an Intramedullary Nail.

Proximal humerus fractures (PHFs) are common injuries seen mostly in elderly patients. Although nonoperative management is acceptable for most patients, surgical fixation is often advocated for younger patients or active, elderly patients with reconstructable fractures. A variety of fixation methods exist: percutaneous pinning, open reduction and internal fixation with locking plates, and intramedullary nailing are viable options. Modern intramedullary nails serve as excellent options for the fixation of many operative PHFs. In this Supplemental Digital Content 1 (see video, http://links.lww.c

The surgical treatment of anterior column and anterior wall acetabular fractures: short- to medium-term outcome.

Isolated fractures of the anterior column and anterior wall are a relatively rare subgroup of acetabular fractures. We report our experience of 30 consecutive cases treated over ten years. Open reduction and internal fixation through an ilioinguinal approach was performed for most of these cases (76.7%) and percutaneous techniques were used for the remainder. At a mean follow-up of four years (2 to 6), 26 were available for review. The radiological and functional outcomes were good or excellent in 23 of 30 patients (76.7%) and 22 of 26 patients (84.6%) according to Matta's radiological criteri

Surgical technique for minimally invasive fibula fracture fixation.

This paper describes a minimally invasive percutaneous technique for reduction and fixation of distal fibula fractures using plate osteosynthesis. We believe this technique benefits patients with poor quality soft tissue envelopes. So far a total of 25 patients have undergone percutaneous fixation, 22 females and 3 males. At no stage yet has a minimally invasive procedure been abandoned intra-operatively in favour of conversion to an open procedure. The mean age was 61.6 years (range 25-80 years). The mean time to surgery was 2.00 days (range 0-5) and mean time to discharge was 4.20 days (rang

Orthopaedic management of the patient with osteopetrosis.

Osteopetrosis is a sclerosing bone dysplasia characterized by hard, brittle bone secondary to dysfunctional osteoclast resorption. The three main forms are malignant autosomal recessive, intermediate autosomal recessive, and benign autosomal recessive. These various clinical manifestations ultimately are caused by genetic mutations affecting acidification of Howship's lacuna. Common radiographic features include a generalized sclerosis, rugger jersey spine, and endobone formation. Medical problems include cranial nerve palsies and pancytopenia. Because cortical and cancellous bone thickness is

Acute traumatic L5-S1 spondylolisthesis.

Acute traumatic L5-S1 spondylolisthesis is a rare condition, almost exclusively the result of major trauma, frequently associated with L5 transverse process fracture and neurologic deficit. In recent years, open reduction and internal fixation with posterior stabilization has been the method of treatment most frequently reported. In the current case, the lesion was found in a victim of an automobile accident. Signs of a right L5 root deficit but no sphincter dysfunction were present. A computed tomography scan revealed several fractures in the posterior parts of L5 and anterior displacement of

Operative treatment of a transverse fracture-dislocation at the S1-S2 level.

The authors recently treated a multiply injured patient who sustained a rare fracture-dislocation at the S1-S2 level. The S1 vertebra was displaced forward into the pelvic cavity and was located just in front of the S2 vertebra. Because the patient also had extensive neurologic injury to the lumbar plexus and instability of the pelvic ring, operative treatment was deemed necessary. Surgery to stabilize the pelvis and decompress the lumbar plexus proved successful, and the patient experienced marked improvement in her postoperative neurologic function. Nonoperative treatment has traditionally b
