Sarcoidosis: rarely a single system disorder.

Sarcoidosis is a systemic disease with a number of extrapulmonary manifestations, rarely limited to a single system. The objective of this review is to provide a overview of the systemic features of sarcoidosis and their surveillance. Sarcoidosis is disease that can affect any organ system, resulting in a number of complications that can cause significant morbidity and mortality, including ophthalmological and cardiac complications. A number of investigations play a role in identifying manifestations of sarcoidosis. Respiratory physicians have an important role to play in identifying complicat

[Research advances on mechanism and lesion location of vertical nystagmus].

Vertical nystagmus is a vertical nystagmus caused by structural abnormalities and/or dysfunction of the central vestibular system and observed in situ in the center of the eyeball. There are two kinds of nystagmus (UBN) and downbeat nystagmus (DBN) according to the direction of nystagmus. The diagnosis of UBN is mainly made by naked eye or electronystagmography/viewer. It is a common neuro-ophthalmologic sign in the field of vestibular medicine. In this paper, the mechanism of vertical nystagmus formation and the location of lesions were briefly introduced, in order to provide help for the dia

[Endogenous endophthalmitis following staphylococcus aureus meningitis].

Endogenous endophthalmitis is a rare complication of acute meningitis. A healthy 82-year-old woman suddenly developed septic bacteremia and meningitis owing to Staphylococcus aureus (methicillin-susceptible Staphylococcus aureus); she presented with a high fever and drowsiness. Improvement of these initial symptoms unveiled impaired bilateral visual acuity. Detailed ophthalmological assessments diagnosed it as probable endogenous endophthalmitis, which was immediately treated with vitrectomy. Subsequently, the patient totally recovered without any neurologic or ophthalmic sequela.

GOOD VISUAL OUTCOME IN A BILATERAL MULTIFOCAL SYPHILITIC CHORIORETINITIS, DESPITE LATE DIAGNOSIS.

PURPOSE: To report a case of a good visual outcome in a patient with bilateral multifocal syphilitic chorioretinitis, despite the late diagnosis. METHODS: Ophthalmologic examination, multimodal imaging, including fundus photography, angiography, and optical coherence tomography. RESULTS: The authors describe a 47-year-old heterosexual man with a bilateral multifocal syphilitic chorioretinitis that was lately diagnosed and despite that had a good visual outcome. CONCLUSION: The diagnosis of ocular syphilis is a challenge; however, once it has been made, even lately, it is eminently treatable

Ring-shaped myelinated retinal nerve fibers at fovea.

We aim to report an unusual case of myelinated retinal nerve fibers (MRNFs) at fovea. A 39-year-old woman presented with visual impairment and her visual acuity was 20/80 in the right eye. Ophthalmologic examination revealed MRNF lesions at inferior and superior poles of optic disc in the right eye. Furthermore, a ring-shaped MRNF lesion with feathery edges was observed at fovea. MRNF lesions are rarely seen at macula, and to our knowledge, this is the first report of an MRNF lesion at fovea.

Case report: bilateral optic nerve head drusen and glaucoma.

Optic nerve head drusen is an incidental finding in current ophthalmological practice. Although patients rarely display symptoms, structural, and functional defects, there are exceptional cases when clinical appearance can make it difficult to diagnose underlying or coexisting conditions, such as glaucoma. The following case report demonstrates how overlapping optic nerve pathologies can interfere in clinical judgement and therapeutic decision making in a young male patient, with relevant family history for both glaucoma and bilateral optic nerve head drusen.

Comparison of treatments in a cataract surgery with circular response.

Circular data are a natural outcome in many biomedical studies, e.g. some measurements in ophthalmologic studies, degrees of rotation of hand or waist, etc. With reference to a real data set on astigmatism induced in two types of cataract surgeries we carry out some two-sample testing problems with the possibility of common or different concentration parameters in the circular set up. Detailed simulation study and the analysis of the data set, including redesigning the cataract surgery data, are carried out.

A rare case of bilateral optic nerve sheath meningioma.

A 60-year-old female presented with gradual, painless, progressive diminution of vision, and progressive proptosis of left eye since 7 years. Ophthalmological examination revealed mild proptosis and total optic atrophy in the left eye. Magnetic resonance imaging (MRI) and computed tomography (CT) brain with orbit showed bilateral optic nerve sheath meningioma (ONSM) involving the intracranial, intracanalicular, intraorbital part of the optic nerve extending up to optic chiasma and left cavernous sinus.

[Bilateral papilledema, unilateral loss of vision and abducens nerve palsy].

A 35-year-old woman complained of headache, reduced visual acuity, restricted visual field in the right eye and blindness in the left eye. The examination of the retina showed papilledema and peripapillary hemorrhages in both eyes. Magnetic resonance imaging (MRI) revealed a sinus thrombosis. Despite modern imaging technologies sinus thrombosis is an often overlooked, life-threatening disease and needs immediate treatment in order to avoid long-term consequences. An ophthalmological examination can be pioneering as it leads to further imaging.

Lateral rectus muscle paralysis induced by ribavirin and pegylated interferon-α2a in a patient with HIV/HCV co-infection.

We report a lateral rectus muscle paralysis occurring 2 weeks after initiation of an interferon-α and ribavirin treatment in a patient with human immunodeficiency virus (HIV) and hepatitis C virus (HCV) virus co-infection. This patient presented with horizontal diplopia that appeared rapidly and without any other neurological symptoms. Symptoms fully resolved with treatment interruption without any ophthalmological sequelae. This side effect is rare and has never been reported in a HIV-HCV co-infected patient.

[Acquired ptosis in allergic blepharoconjunctivitis in children and adolescents].

Acquired ptosis is a common ophthalmologic problem. It is in the large majority of cases aponeurotic and occurs in the elderly as an involutional disorder or after ophthalmic surgery. In younger patients, it may occur after ocular trauma, periocular infection, contact lens wear, or palpebral edema. Allergic blepharoconjunctivitis is an unsuspected cause of acquired ptosis. Our study investigated patients (5 to 15 years old) with a history of allergic blepharoconjunctivitis and having a unilateral or bilateral ptosis. Physiopathological hypotheses are discussed. Management of acquired ptosis is

Doxycycline-induced pseudotumor cerebri.

Tetracyclines are a commonly prescribed medication for the treatment of acne vulgaris that are associated with pseudotumor cerebri (PTC). With doxycycline specifically, however, the incidence of PTC is very rare. A patient was using oral doxycycline and topical retinoids for acne, and within two months she developed PTC. This case illustrates that despite the rarity of doxycycline-induced PTC, patients and physicians should be aware of this possibility. Furthermore, in the setting of new-onset headaches or visual changes, early ophthalmologic examination for papilledema is recommended for earl

Abnormal ocular electrophysiology in Sjögren-Larsson syndrome.

Two siblings who were born out of consanguineous marriage in an Indian family and had typical clinical manifestations of Sjögren-Larsson syndrome were found to have normal electroretinogram and abnormal visual evoked potential results. Abnormal visual evoked potential results along with the presence of distinctive foveal and parafoveal deposits, which could have a prognostic bearing on the visual acuity of the patients, emphasize the importance of a complete ophthalmological and electrophysiological evaluation in all patients with Sjögren-Larsson syndrome.

Albinism and developmental delay: the need to test for 15q11-q13 deletion.

We report on a 17-month-old African girl with cutaneous and ophthalmologic features of oculocutaneous albinism type 2 as well as microcephaly, absent speech, and tremulous movements. Mutations of the P gene within the Angelman/Prader-Willi syndrome critical region at 15q11-q13 cause oculocutaneous albinism type 2. Comorbid oculocutaneous albinism and Angelman syndrome were suspected and confirmed by cytogenetics. Phenotypic features of Angelman syndrome or Prader-Willi syndrome in a patient with albinism should prompt further investigation.

Unusual eye signs in Wegener's granulomatosis.

Wegener's granulomatosis is a rare necrotising vasculitis not easily diagnosed due to the obscurity of its diverse clinical features. Despite its comparatively low incidence, the unusual ophthalmic manifestations seen in this disease warrant extra caution from attending rheumatologists. In this case, bilateral peripheral ulcerative keratitis preceded any systemic symptoms. Timely recognition of the significance of this ophthalmic complaint and prompt ophthalmological consultation can help achieve early diagnosis and treatment of this potentially fatal rheumatological disease.

[Proposal for a protocol for the staging of incontinentia pigmenti in pediatric age].

Incontinentia Pigmenti (IP) is an X-linked dominant disorder of skin with neurologic and ophthalmologic involvement. IP predominantly affects females because the mutations are usually lethal in males in utero. IP is characterized by abnormalities of neuroectodermal tissues. IP is caused by mutations in a gene called NEMO, which is required to activate the NF-kB pathway. We present a diagnostic protocol for IP and a meta-analysis of the clinical spectrum of IP in 82 patients cited by MEDLINE in the European literature from 2000 to 2006.

[Incontinentia pigmenti: a case study].

Incontinentia pigmenti is a rare hereditary, dominant, X-linked disorder. It involves the skin, the teeth, the eyes and the central nervous system. The case we report is an infant girl aged 2 months. She had typical skin lesions associated with severe impairment of her left eye. We comment on the clinical, histological, genetic, and therapeutic characteristics of this rare disease. Ophthalmologic examination should be made early in order to diagnose ocular involvement at an early stage of the disease to provide for greater treatment possibilities.

Renal vascular disease in neurofibromatosis type 2: association or coincidence?

Neurofibromatosis type 2 (NF2) remains a challenging diagnosis in childhood where there may be no neurological involvement. A 12-month-old male in whom NF2 was suspected because of characteristic ophthalmological and cutaneous lesions is reported. Cranial MRI showed no tumours. A pathogenic mutation was identified on NF2 gene analysis. The child developed hypertension due to renal vascular disease. Although renal vascular disease is a recognized complication of neurofibromatosis type 1 (NF1), it has not been reported in NF2.

[Eyelid tick bite].

CASE REPORT: We describe a patient who was bitten on his right upper eyelid by a common canine tick. The tick was impregnated with gasoline prior to being removed. DISCUSSION: Tick bites are uncommon in ophthalmologic practice. They occur more frequently in rural areas, especially in spring and summer. Correct identification of the problem and treatment is essential. The tick needs to be completely removed in order to avoid the embedding of different parts of the insect. Zoonoses, such as rickettsiosis, arbovirus and paralysis due to ticks must be ruled out.

[Giant mucocele of all paranasal sinuses with massive bilateral globe displacement].

Although of benign nature and slowly progressive, paranasal sinus mucoceles may, depending on their localization, cause a multitude of ophthalmological symptoms due to compression and displacement of adjacent tissue. Here we report the unusual case of a patient suffering from a progressively growing giant mucocele that developed years after ENT surgery and that was neglected for almost 2 decades despite massive symptoms. This case report demonstrates the importance of including mucoceles of the paranasal sinuses into the differential diagnosis of unilateral or bilateral proptosis.
