Genetic variants in IFIH1 play opposite roles in the pathogenesis of psoriasis and chronic periodontitis.

The IFIH1 gene is a key factor connecting environmental and genetic factors in the pathogenesis of immune-related diseases. We aimed to investigate whether it has effects on psoriasis, chronic periodontitis and skin test-positive penicillin allergy and to confirm whether these diseases have shared molecular mechanisms originating from shared genetics. Two common variants in IFIH1 were genotyped in 561 patients with psoriasis, 421 patients with chronic periodontitis, 175 patients with skin test-positive penicillin allergy and 1100 shared controls. Then, case-control study was used to analyse th…

Alcohol misuse in patients with psoriasis: identification and relationship to disease severity and psychological distress.

BACKGROUND: Moderate to severe psoriasis is associated with increased alcohol intake and excessive mortality from alcohol-related causes. Alcohol biomarkers provide an objective measure of alcohol consumption. Carbohydrate-deficient transferrin (CDT) is the single most sensitive and specific alcohol biomarker. OBJECTIVES: To assess alcohol consumption in a cohort of patients with moderate to severe psoriasis using standard alcohol screening questionnaires and biomarkers. We investigated whether there was an association between alcohol intake, anxiety, depression and disease severity. METHODS…

Comparison of drug survival rates for adalimumab, etanercept and infliximab in patients with psoriasis vulgaris.

BACKGROUND: Adherence to treatment is an indicator of treatment success. Long-term data on adherence to biologic treatment in psoriasis are lacking. OBJECTIVES: To compare the tumour necrosis factor (TNF)-α inhibitors regarding drug survival rate and safety in patients with psoriasis. METHODS: This study is based on data from the Danish nationwide database DERMBIO covering patients with psoriasis treated with a biologic agent. All patients who received anti-TNF-α treatment in academic referral centres were included. Baseline data, adverse events, time on treatment and reason for stopping tre…

Increased expression of TRAIL and its death receptors DR4 and DR5 in plaque psoriasis.

TNF-related apoptosis-inducing ligand (TRAIL) is recognized as an important regulator of immune responses during infections and various autoimmune-mediated pathologies. Its role in inflammatory dermatoses is largely unknown. We aimed to investigate the expression of TRAIL and its receptors DR4 and DR5 in psoriasis vulgaris. Immunohistochemistry for TRAIL, DR4 and DR5 was performed on samples of lesional (n = 10) and non-lesional (n = 10) skin of patients with plaque psoriasis and skin of healthy volunteers (n = 10). Expression of TRAIL and its receptors was further examined by means of double …

Usefulness of ultrasound imaging in detecting psoriatic arthritis of fingers and toes in patients with psoriasis.

BACKGROUND: Given that clinical evaluation may underestimate the joint damage and that early treatment can slow down psoriatic arthritis (PsA) progression, screening psoriasis patients with imaging tools that can depict early PsA changes would entail clear benefits. OBJECTIVE: To compare the ability of X-ray and ultrasound (US) examination in detecting morphological abnormalities consistent with early PsA in patients with psoriasis, using rheumatological evaluation as the gold standard for diagnosis. METHODS: Patients with chronic plaque psoriasis and no previous PsA diagnosis attending our …

Statistical evaluation and experimental design of a psoriasis xenograft transplantation model treated with cyclosporin A.

Psoriasis xenograft transplantation models where human skin is transplanted onto immune-deficient mice are generally accepted in psoriasis research. Over the last decade, they have been widely employed to screen for new therapeutics with a potential anti-psoriatic effect. However, experimental designs differ in several parameters. Especially, the number of donors and grafts per experimental design varies greatly; numbers that are directly related to the probability of detecting statistically significant drug effects. In this study, we performed a statistical evaluation of the effect of cyclosp…

Drug-induced lichenoid dermatitis with histopathologic features of mycosis fungoides in a patient with psoriasis.

BACKGROUND: With recent advancements in the understanding of psoriasis, treatment has evolved toward more targeted therapy in the form of biologics. Although generally safe and well tolerated, various adverse reactions may occur with the use of biologics. OBJECTIVES: The objective of this article is to present a case of a biologic-induced drug eruption initially presenting with histopathologic features of mycosis fungoides (MF). METHODS AND RESULTS: A 47-year-old male presented with an extensive erythematous macular eruption approximately 14 hours after a single dose of etanercept. Histopath…

A case of aseptic pleuropericarditis in a patient with chronic plaque psoriasis under methotrexate therapy.

Methotrexate may rarely provoke serositis, even with low doses and after just a few weeks of therapy. We report here a rare case of pleuropericarditis due to methotrexate. The effusion resolved after the withdrawal of the drug and the beginning of anti-inflammatory therapy; there was no relapse during a 10-month follow-up.

STAT4 gene polymorphism is associated with psoriasis in the genetically homogeneous population of Crete, Greece.

Recent genome-wide association studies (GWAS) of many complex diseases have successfully identified novel susceptibility loci, with many of them being associated with more than one condition. Taking into consideration that different autoimmune diseases may share some common pathogenetic pathways, we hypothesized that STAT4, a susceptibility gene found to be associated with increased risk for systemic lupus erythematosus, rheumatoid arthritis, type 1 diabetes, Sjögren's syndrome, Wegener's granulomatosis, Crohn's disease, and ulcerative colitis may also have a role in psoriasis. Psoriasis is an…

Pharmacoeconomic model of topical treatment options of mild to moderate psoriasis vulgaris in Germany.

BACKGROUND: Psoriasis vulgaris is one of the most common dermatologic diseases in Germany with a prevalence of about 2%. Along with efficacy and safety, costs are an important criterion for selecting appropriate therapy for this chronic disease. The majority of the patients with mild to moderate psoriasis vulgaris can be successfully treated with topical therapy alone. The most important agents are vitamin D(3) analogs and corticosteroids either as single agents or in a fixed combination (Daivobet). METHODS: For the evaluation of the cost-effectiveness of different topical psoriasis treatment…

[Spanish evidence-based guidelines on the treatment of moderate-to-severe psoriasis with biologic agents].

Psoriasis vulgaris is an inflammatory skin disease that is generally chronic and that affects between 1 % and 2 % of the population in industrialized Western countries. It is associated with a marked decline in quality of life. A wide range of treatments are currently available, although surveys conducted before the advent of biologic agents reflected a strong degree of dissatisfaction with the treatments then available. Extensive scientific evidence has been gathered on the safety of biologic agents, and this has led to a review of the role of systemic treatment in general and has allowed new…

The nail as a musculoskeletal appendage--implications for an improved understanding of the link between psoriasis and arthritis.

The nail is considered to be a specialised modification of the skin--an 'epidermal appendage'. However, the phrase 'hanging on by your fingernails' belies a vital aspect of nail microanatomy that has been overlooked, namely that the nail is functionally integrated with the musculoskeletal system. The current article reviews how the nail is functionally linked to the distal phalanx and several distal interphalangeal joint structures, including extensor tendon fibres and the collateral ligaments. New histological images exploring this link are provided. The extensor tendon in particular continue
