Anatomical reconstructions of pediatric airways from endoscopic images: a pilot study of the accuracy of quantitative endoscopy.

OBJECTIVES/HYPOTHESIS: To evaluate the accuracy of three-dimensional (3D) airway reconstructions obtained using quantitative endoscopy (QE). We developed this novel technique to reconstruct precise 3D representations of airway geometries from endoscopic video streams. This method, based on machine vision methodologies, uses a post-processing step of the standard videos obtained during routine laryngoscopy and bronchoscopy. We hypothesize that this method is precise and will generate assessment of airway size and shape similar to those obtained using computed tomography (CT). STUDY DESIGN: Thi

Proposal to utilize simplified Swensen protocol in diagnosis of isolated pulmonary nodule.

BACKGROUND: The problem of diagnosing whether a solitary pulmonary nodule is benign or malignant is even greater in developing countries due to a higher prevalence of infectious diseases. These infections generate a large number of patients who are generally asymptomatic and with a pulmonary nodule that cannot be accurately defined as having benign or malignant etiology. PURPOSE: To verify the percentages of benign versus malignant non-calcified nodules, the length of time after contrast agent injection is spiral computed tomography (CT) most sensitive and specific, and whether three postcont

Obesity and diabetes: newer concepts in imaging.

Quantifying body fat is currently an area of active research. Recent studies have shown that the quantity and location of fat in different compartments have varying clinical significance. This information can now be obtained from computed tomography (CT) or magnetic resonance (MR), and it can inform clinical decision making for patient management. Diabetes patients with insulin resistance and hyperinsulinemia have nonalcoholic fatty liver disease (NAFLD) ranging from steatosis to nonalcoholic steatohepatitis (NASH) and cirrhosis, typically diagnosed by liver biopsy or serum markers. There is n

Analysis of mice deficient in both REV1 catalytic activity and POLH reveals an unexpected role for POLH in the generation of C to G and G to C transversions during Ig gene hypermutation.

Multiple DNA polymerases are involved in the generation of somatic mutations during Ig gene hypermutation. Mice expressing a catalytically inactive REV1 (REV1AA) exhibit reduction of both C to G and G to C transversions and moderate decrease of A/T mutations, whereas DNA polymerase η (POLH) deficiency causes greatly reduced A/T mutations. To investigate whether REV1 and POLH interact genetically and functionally during Ig gene hypermutation, we established REV1AA Polh(-/-) mice and analyzed Ig gene hypermutation in the germinal center (GC) B cells. REV1AA Polh(-/-) mice were born at the expect

Factors associated with fluoroscopy exposure during pediatric catheter ablation utilizing electroanatomical mapping.

PURPOSE: Exposure to ionizing radiation during electrophysiologic procedures in children is believed to increase the risk of future malignancy. Electroanatomical navigation can reduce exposure, but the cohort of children who derive the greatest benefit from this approach is incompletely defined. We sought to determine factors associated with fluoroscopy exposure with conventional catheter ablation versus electroanatomical navigation. METHODS: A retrospective review of all ablation procedures over a 5-year period during the transition to electroanatomical navigation performed by a single elect

Effectiveness of MR enterography for the assessment of small-bowel diseases beyond Crohn disease.

The use of cross-sectional imaging techniques for the noninvasive evaluation of small-bowel disorders is increasing. The effectiveness of magnetic resonance (MR) enterography for the evaluation of Crohn disease, in particular, is well described in the literature. In addition, MR enterography has an evolving though less well documented role to play in the evaluation of other small-bowel diseases, including various benign and malignant neoplasms arising in isolation or in polyposis syndromes such as Peutz-Jeghers, inflammatory conditions such as vasculitis and treatment-induced enteritis, infect

Pediatric capsule endoscopy: review of the small bowel and patency capsules.

BACKGROUND AND OBJECTIVE: Because capsule endoscopy (CE) avoids ionizing radiation, deep sedation, and general anesthesia, CE may be valuable in pediatrics. We report a single pediatric center's experience with the use and safety of CE. METHODS: In a retrospective review of consecutive CE studies, 284 CE studies were performed in 277 patients with a mean age of 15 (±3.7) years during a 5-year period. The youngest to swallow the capsule was 4.6 years old. Twenty capsules were placed. Overall, 245 (86%) patients underwent CE for suspected (184, 65%) or confirmed (61, 21%) Crohn disease (CD); 27

Foot Type Biomechanics Part 2: are structure and anthropometrics related to function?

BACKGROUND: Many foot pathologies are associated with specific foot types. If foot structure and function are related, measurement of either could assist with differential diagnosis of pedal pathologies. HYPOTHESIS: Biomechanical measures of foot structure and function are related in asymptomatic healthy individuals. METHODS: Sixty-one healthy subjects' left feet were stratified into cavus (n=12), rectus (n=27) and planus (n=22) foot types. Foot structure was assessed by malleolar valgus index, arch height index, and arch height flexibility. Anthropometrics (height and weight), age, and walk

[Multiple myeloma: current recommendations for imaging].

CLINICAL/METHODICAL ISSUE: Imaging in monoclonal plasma cell disease serves to detect end organ damage, i.e., osteoporosis or bone destruction. Diffuse or circumscribed bone marrow infiltration without damage to mineralized bone is so far not regarded as end organ damage. STANDARD RADIOLOGICAL METHODS: Skeletal plain x-ray film survey to detect bone destruction, osteoporosis or fractures. METHODICAL INNOVATIONS: Whole body low-dose computed tomography (CT) and whole body magnetic resonance imaging (MRI) allow a more sensitive assessment of both mineralized bone and bone marrow, with greater

Possibilities for improved early breast cancer detection by Padé-optimized magnetic resonance spectroscopy.

There are major dilemmas regarding the optimal modalities for breast cancer screening. This is of particular relevance to Israel because of its high-risk population. It was suggested that an avenue for further research would be to incorporate advances in signal processing through the fast Padé transform (FPT) to magnetic resonance spectroscopy (MRS). We have now applied the FPT to time signals that were generated according to in vitro MRS data as encoded from extracted breast specimens from normal, non-infiltrated breast tissue, fibroadenoma and cancerous breast tissue. The FPT is shown to res

[Validation of the Ottawa rules for the Portuguese population: a prospective study].

UNLABELLED: Acute ankle injury is one of the main reasons for observation of patients in the emergency department. Only 15% present with clinically significant fractures, but they are almost always referred for radiography. The Ottawa ankle rules have provided specific directions for carrying out radiography in these situations, allowing a reduction in hospital costs and reducing exposure to ionizing radiation. The aim of the study is to verify that the protocol of the Ottawa rules for evaluating ankle trauma can adapt to a sample of the Portuguese population. MATERIAL AND METHODS: This prosp

Expanding nail or expanding femur? An adverse event with the expandable intramedullary nail.

The expandable intramedullary nail is self-locking and has the advantage of reducing operating time and exposure to ionizing radiation. The nail is recommended for simple diaphyseal fractures involving the middle third of long bones, where the nail can bypass the fracture site by at least 5 cm. We encountered a unique complication with the expandable nail in a simple transverse shaft fracture at the junction of the middle and distal third of the left femur in an otherwise healthy 57-year-old man. The fracture was reduced and a 12-mm expandable nail was inserted. Following full expansion, intra

Patterns of care and outcomes after computed tomography scans for headache.

BACKGROUND: concerns exist about potential overuse of computed tomography (CT) scans for headache in ambulatory care. METHODS: we sought to examine health services use, brain tumor diagnosis, and death during the year after CT scanning for headache by linking records of an audit of 3930 outpatient CT brain scans performed in 2005 in Ontario, Canada, to administrative databases. RESULTS: of 623 patients receiving CT scans for a sole indication of headache, few (2.1%) scans contained findings potentially causing their headache. For most patients, the index CT scan was the only one received ove

Sonographic diagnosis of pneumothorax.

PURPOSE: Over the last decade, the use of ultrasound as a technique to look for pneumothorax has rapidly evolved. This review aims to analyze and synthesize current knowledge on lung ultrasound targeted at the diagnosis of pneumothorax. The technique and its usefulness in different scenarios are explained, and its merits over conventional radiology are highlighted. METHODS: A systematic literature search (1995-2010) was performed, involving PubMed, to describe the more recent scientific evidence on the topic. Moreover, this review is also a synopsis of experts' opinion and personal clinical e

Resuscitative Long-Bone Sonography for the Clinician: Usefulness and Pitfalls of Focused Clinical Ultrasound to Detect Long-Bone Fractures During Trauma Resuscitation.

Bone has one of the highest acoustic densities (AD) in the human body. Traditionally, bone has been considered to be a hindrance to the use of ultrasound (US), as US waves are reflected by the dense matrix and obscure underlying structures. The intense wave reflection, however, can clearly illustrate the cortical bony anatomy of long bones, making cortical disruption obvious. Ultrasound can be used at the bedside concurrently with the overall trauma resuscitation, and may potentially limit the patient's and treating team's exposure to ionizing radiation, corroborate clinical findings, and augm

X-band EPR imaging as a tool for gradient dose reconstruction in irradiated bones.

PURPOSE: Various tools are currently available for dose reconstruction in individuals after accidental exposure to ionizing radiation. Among the available biological analyses, Monte Carlo simulations, and biophysical methods, such as electron paramagnetic resonance (EPR), the latter has proved its usefulness for retrospective dosimetry. Although EPR spectroscopy is probably the most sensitive technique, it does not provide spatial dosimetric data. This information is, however, highly desirable when steep dose gradient irradiations are involved. The purpose of this work was to explore the possi

Fast MRI evaluation of pulmonary progressive massive fibrosis with VIBE and HASTE sequences: comparison with CT.

PURPOSE: The aim of this prospective study was to evaluate the diagnostic utility of volumetric interpolated breath-hold examination (VIBE) and half-Fourier-acquisition single-shot turbo spin-echo (HASTE) fast magnetic resonance imaging (MRI) sequences in the evaluation of pulmonary progressive massive fibrosis (PMF) in comparison with computed tomography (CT) imaging. If fast MRI is proven to be diagnostically significant, this modality can be used for diagnosis and follow-up studies of PMF patients. MATERIALS AND METHODS: Twenty-two PMF lesions from 20 coal workers were evaluated. After CT

Overcoming cancer therapy resistance by targeting inhibitors of apoptosis proteins and nuclear factor-kappa B.

Chemo- or radioresistance markedly impairs the efficacy of cancer therapy and involves anti-apoptotic signal transduction pathways that prevent cell death. In resistant cancer cells, both inhibitors of apoptosis proteins (IAPs) and nuclear factor-kappa B (NF-kappaB) play a pivotal role in preventing apoptosis triggered by a variety of stresses, facilitating them as potential targets in cancer treatment. Furthermore, mounting evidences have established the crosstalks between IAPs (eg. XIAP, cIAP-1, cIAP-2) and proteins involved in NF-kappaB signaling (eg. TRAF2, RIP1, TAB1). Second mitochondria

Clinical perspectives in Crohn's disease. Objective measures of disease activity: alternatives to symptom indices.

Advances in fecal and serum inflammatory biomarkers, endoscopy, and radiology have led to a rapid expansion of modalities for diagnosis and disease activity assessment of Crohn's disease. Although no test is recognized as the most accurate for assessing disease activity, ileocolonoscopy remains the single test that may approach the gold standard for clinical diagnosis. Serum C-reactive protein concentrations have been shown to correlate reasonably well with clinical, endoscopic, and radiologic measures of disease activity, and they appear to have prognostic value in certain settings.
