Non-surgical casting versus surgical reduction for children with severely displaced distal radial fractures (the CRAFFT Study): a multicentre, randomised, controlled non-inferiority trial and economic evaluation.

BACKGROUND: Severely displaced distal radial fractures are among the most common and controversial injuries in children. Despite observational evidence of reliable remodelling with growth in younger children, their alarming radiographic appearance-particularly when completely displaced (off-ended)-has driven routine surgical reduction and fixation. The Children's Radius Acute Fracture Fixation Trial (CRAFFT) aimed to evaluate the clinical and cost-effectiveness of surgical reduction compared with non-surgical casting. METHODS: This pragmatic, multicentre, randomised, non-inferiority trial inc

A new method for evaluating radial neck fractures based on Judet classification.

AIMS: To propose a new method for evaluating paediatric radial neck fractures and improve the accuracy of fracture angulation measurement, particularly in younger children, and thereby facilitate planning treatment in this population. METHODS: Clinical data of 117 children with radial neck fractures in our hospital from August 2014 to March 2023 were collected. A total of 50 children (26 males, 24 females, mean age 7.6 years (2 to 13)) met the inclusion criteria and were analyzed. Cases were excluded for the following reasons: Judet grade I and Judet grade IVb (> 85° angulation) classificatio

Regional Anesthesia as an Alternative to Procedural Sedation for Forearm Fracture Reductions in the Pediatric Emergency Department.

BACKGROUND: Pediatric forearm fractures are common injuries in the pediatric emergency department (PED). Pediatric procedural sedation (PPS) is often required for forearm fracture reductions and pain control for casting. Bier blocks and hematoma blocks are types of regional anesthesia (RA) procedures that can be performed as a potential alternative to PPS. OBJECTIVE: The objective of this study is to compare the safety of RA with that of PPS. We hypothesized that RA has a safety profile that is equal or superior to PPS as well as a shorter duration of treatment in the PED. METHODS: Pediatric

Outcomes of Radial Head Fractures Treated With the "Tripod Technique".

PURPOSE: For fractures requiring operative fixation, the "tripod technique" using headless compression screws has recently been described as a less invasive alternative to open reduction and internal fixation with plate and screws. The purpose of this study was to evaluate the clinical and radiographic outcomes of the tripod technique for the treatment of radial head and neck fractures. METHODS: We performed a retrospective chart review of all radial head and neck fractures treated with the tripod technique at our institution over a 10-year period. Patients with less than 6 months of follow-u

Nonoperative Treatment Compared with Volar Locking Plate Fixation for Dorsally Displaced Distal Radial Fractures in the Elderly: A Randomized Controlled Trial.

BACKGROUND: The usage of volar locking plate fixation for distal radial fractures has increased in older patient populations, despite the fact that surgical treatment in the elderly population has not clearly been proven to be superior to nonoperative treatment. The purpose of the present study was to compare nonoperative treatment with volar locking plate fixation with regard to clinical outcome for elderly patients with dorsally displaced distal radial fractures. METHODS: In this study, 140 patients were randomly allocated to nonoperative treatment with a plaster splint (n = 72) or volar lo

Treatment of impacted greenstick forearm fractures in children using bandage or cast therapy: a prospective randomized trial.

BACKGROUND: To evaluate the relative risks and advantages of using soft bandage therapy (BT) for impacted greenstick fractures of the distal forearm compared with cast therapy (CT). METHODS: At a single center from September 2005 to October 2006 all patients with an impacted greenstick fracture were randomized to BT or CT. RESULTS: A total of 92 patients were randomized and assigned. No fracture displacement was seen. Three patients from the BT group were converted to standard treatment. A significant difference in pain was seen between both groups after 1 week in favor of the CT group. Disc

Plate and screw design in fractures of the hand and wrist.

UNLABELLED: As internal fixation of fractures of the hand and wrist has evolved, so too have the implants used for the same purpose. Implants used for the first (1/2) of the 20th century were modeled on implants used to fix long bone fractures. Consequently, they were often bulky. In addition, they often were made of materials which were ill suited to the unique functional and biomechanical requirements of the hand and wrist. As a result, outcomes of internal fixation frequently were suboptimal and complication rates were high. In the past 40 years there has been an evolution in metallurgy, bi

The effect of ultrasound on the healing of muscle-pediculated bone graft in scaphoid non-union.

The use of pedicled vascularised bone grafts from the distal radius makes it possible to transfer bone with a preserved circulation and viable osteoclasts and osteoblasts. Experiments performed at the basic science level has provided substantial evidence that low-intensity ultrasound can accelerate and augment the fracture healing process. Only an adequate double-blind trial comparing treatment by ultrasound stimulation in patients treated by similar surgical techniques can provide evidence of the true effect of ultrasound. This paper describes the results of such a trial. From 1999 to 2004, 2

Strategies for nonrandomized clinical research in hand surgery.

Most clinical research questions in hand surgery may be effectively explored using a variety of nonrandomized study designs. The main advantage of any of these methods is that they are almost always more feasible than a prospective randomized, controlled trial. Although the level of evidence associated with nonrandomized designs is always lower than that of a randomized trial there are many instances in which the inferences based on these designs are sufficiently strong that important and meaningful conclusions can be made. The key considerations in using nonrandomized designs are to frame the

Technical standardization for dual-energy x-ray absorptiometry.

Clinical applications of dual-energy X-ray absorptiometry (DXA) should be updated periodically to address new technologies and inconsistencies in the use of existing technologies. The 2003 Position Development Conference of the International Society for Clinical Densitometry addressed issues regarding hip and forearm regions of interest (ROIs), vertebral fracture assessment (VFA), quality assurance (QA), precision, and cross-calibration. Data are insufficient to determine whether the mean T-score of both hips can be used for diagnosis. However, the mean bone mineral density of both hips can be

Rupture of the extensor pollicis longus tendon after Colles fracture and by rheumatoid arthritis.

During the period 1973-1986, 116 patients with rupture of the extensor pollicis longus tendon were operated upon. The lesions were categorised into three anatomical levels: proximal, intermediate and distal. Direct suture was performed for acute lesions; later reconstructive procedures were by means of tendon transfers or free tendon grafts.

[External fixation of unstable wrist fractures].

The results of external fixation of 66 unstable distal radial fractures were evaluated. In the majority, the radiological results were excellent or good. There was a clear correlation between an excellent or good clinical score and the subsequent radiological result. Upper limb dystrophy which caused a considerable morbidity was found in every third patient. Unstable distal radial fractures with severe initial displacement predispose to reflex dystrophy. External fixation obviously cannot prevent this severe complication, it is even possible that tension on soft tissues, capsules and ligaments

Brace treatment of Colles' fracture.

Twenty patients with Colles' fractures were treated with a functional brace that allows wrist motion. The result was compared with a similar group of patients treated with a plaster cast. Dorsal displacement of the fracture was less and wrist function was better in the brace group. Swelling of the hand during the early stages was observed in the brace group; and because this may necessitate brace adjustment, increased medical supervision is necessary for this period.

The use of dolorimetry in the assessment of post-traumatic algodystrophy of the hand.

We have investigated the use of dolorimetry in the assessment of algodystrophy of the hand. Findings in 12 affected patients following Colles' fracture were compared with age- and sex-matched normal controls. Algodystrophy was associated with a significant increase in tenderness of both the finger joints and bones. Tenderness of the whole hand was quantified by summing the dolorimetry readings at each site within the hand and the ratio of the summed readings for the two hands was defined as the dolorimetry ratio. This ratio was highly reproducible and independent of external variables. It was
