BACKGROUND: Prior mixed cohort pitching biomechanic analyses have noted favorable pitching biomechanics and decreased throwing arm kinetics in pitchers with delayed trunk rotation.
METHODS: Three hundred thirty-nine professional pitchers instructed to throw 8 to 12 fastball pitches were evaluated with 3-dimensional motion capture (480 Hz). Pitchers were identified as initiating trunk rotation before the moment of foot contact (EARLY, n = 33) and were propensity matched on the basis of height, age, and weight to pitchers who initiated trunk rotation after foot contact (LATE, n = 132) resulting

BACKGROUND: Proximal humerus fractures are common injuries in elderly patients. Although a majority of fractures can be treated conservatively, open reduction and internal fixation (ORIF) with locking plates is still the most commonly used operative treatment modality. Regarding postoperative treatment, there is currently no consensus in the literature. A restrictive rehabilitation regimen, which can lead to a prolonged and effortful rehabilitation process, may not make full use of the possibilities that modern angular stable implants in combination with adequate patient selection and improved

BACKGROUND: Heterotopic ossification (HO) is a major complication following complex elbow fracture-dislocations and often leads to postoperative stiffness and impaired functional recovery. However, data focusing exclusively on complex elbow fracture-dislocations remain limited. The aim of this study was to determine the incidence and severity of HO, identify independent risk factors, and evaluate its impact on mid-term functional outcome.
METHODS: A retrospective cohort study was performed including 169 patients who underwent surgical treatment for complex elbow fracture-dislocations between

BACKGROUND: Fly-fishing requires rhythmic, coordinated movements to successfully cast a fly line. Previous studies have shown that the biomechanical demands of fly-casting may cause some individuals to develop upper extremity pain or injuries.
PURPOSE: To report the rates, trends, and contributing factors of upper extremity pain and overuse injuries in a sample of North American recreational fly-fishers.
STUDY DESIGN: Descriptive epidemiology study.
METHODS: Participants were reached via 3 popular online fly-fishing forums in April 2019. Each consenting North American participant over 18 ye

Focal axillary artery stenosis or occlusion is a rare occurrence that is most commonly encountered after severe trauma involving the shoulder joint or proximal humerus. Other etiologies that can lead to axillary artery injury or pathology include different vasculitides, radiation arteritis, crutch-injury, and peripheral arterial disease. In this case report, a 70-year-old woman was referred for asymmetrically decreased right brachial artery systolic pressure and right-hand paresthesia with overhead abduction. Further evaluation with imaging revealed critical stenosis of the right axillary arte

Women undergo operative vaginal delivery (OVD) as an alternative to caesarean section when complications arise in the second stage of labour. The perinatal mortality associated with OVD is very low, and most of the perinatal morbidity is minor. However, when serious adverse events occur, such as traumatic birth injury, shoulder dystocia, cerebral palsy and perinatal death, there are medico-legal implications. There is also the potential for litigation in relation to maternal pelvic floor injury, which is increased with OVD. Obstetricians performing and supervising OVDs need to be aware of the

Most closed humeral shaft fractures can be successfully managed nonsurgically. However, fractures for which closed treatment is unsuccessful are stabilized using either plates or intramedullary nails. There are shortcomings associated with each technique, including the potential complications of nonunion, infection, shoulder pain, and radial nerve injury. Minimally invasive plate osteosynthesis (MIPO), an innovative alternative treatment, is gaining in popularity. This technique is based on the anterior humeral shaft providing a relatively safe surface for plate application, and limited open e

Six cases with cervical cord contusion are described. All were over 50 years old, had an hyperextension injury of their cervical spine, suffered from tetraparesis predominant in the upper limbs and showed no X-ray evidence of injury to the cervical spine. Marked cervical spondylosis was present in five cases, one also had a narrow spinal canal. Cases No. 3 and 4 had an atypical recovery pattern. Shoulder and elbow movements improved last or not at all, while in the classic cases the hand muscles recovered last. Upper motor neuron damage was thought to be the cause, a result of intermediate ins

BACKGROUND: Galeazzi fractures are a rare injury to occur in adults, which are typically associated with high energy trauma. To our knowledge, the reported cases in the literature have only been associated with motor vehicle accidents, falls from a height, or athletic endeavors. The application of the force in the setting of a Galeazzi fracture has been disputed, but it is believed to occur with the wrist in hyperextension and pronation.
CASE REPORT: We describe a 27-year-old male who presented with multiple gunshot wounds that included his right forearm. The injury sustained to his right for

In a previous study, compared with throwing athletes with superior labral anterior posterior (SLAP) tears, those with concomitant SLAP tears and rotator cuff tears (RCTs) had significantly poorer outcome scores and return to play. Posterior shoulder instability also occurs in throwing athletes, but no studies currently exist regarding outcomes of these patients with concomitant RCTs. The authors hypothesized that throwing athletes treated with arthroscopic capsulolabral repair for posterior shoulder instability with coexistent rotator cuff pathology would have poorer outcome scores and return

BACKGROUND: Rotator cuff tears are common and not well-understood shoulder problems. Structural and mechanical inhomogeneity of the supraspinatus tendon complicates accurate prediction of risk of tear propagation and may affect appropriate clinical treatment.
HYPOTHESIS: We propose that interactions between the supraspinatus and infraspinatus tendons are critical to load bearing at the glenohumeral joint and warrant investigation.
STUDY DESIGN: Controlled laboratory study.
METHODS: Principal strains in the infraspinatus tendon of cadaveric human shoulders were evaluated with increasing ante

The purpose of this study is to investigate the prevalence of concomitant intraarticular lesions to the glenohumeral joint or to surrounding soft tissue structures with non-randomized prospective case series. High-grade acromioclavicular (AC) joint dislocations result from direct or indirect force impact to the shoulder girdle. Fourty consecutive patients (2 female, 38 male) with high-grade acromioclavicular joint dislocations (Rockwood III: n = 3; IV: n = 3; V: n = 34) who underwent diagnostic arthroscopy at the time of acromioclavicular joint repair were evaluated. Associated pathologic lesi

INTRODUCTION: Reattachment of the supraspinatus (SSP) tendon after spontaneous rupture leads to improved shoulder function. Whether this improvement of function is due to a reversal of muscle atrophy and fat accumulation known to occur after SSP rupture is still debated. Our previous study of late reattachment of SSP (12 weeks) failed to confirm a reversal of muscle atrophy and of fat accumulation.
PURPOSE: To find out whether earlier reattachment (6 weeks) reverses atrophy and fat accumulation of the SSP.
MATERIAL AND METHODS: Reattachment group: in seven rabbits unilateral supraspinatus de

Abnormalities of the tendon of the long head of the biceps brachii muscle commonly associate with other lesions about the shoulder, especially rotator cuff rupture, and are a frequent cause of a painful shoulder. As pathognomonic clinical signs of these lesions are often lacking, noninvasive imaging techniques are practical and valuable. In this report the anatomy of this tendon is described, correlating with sonographic images. Examination technique and typical sonographic findings in pathologic conditions affecting the long bicipital tendon sheath, the substance of the tendon, and the positi

The complications of modern surgical and radiotherapeutic methods of treating mammary neoplasms are studied on the basis of 36 cases. Although swollen arms are rare nowdays following the development of greater pectoral conservative mammectomy, lesions of the shoulder and post-radiotherapy plexal paralysis represent a heavy price to pay for these methods. Shoulder lesions are of two types with different prognosis : scapulo-humeral periarthritis, which is very frequent (22 cases) and related to post-radiotherapy axillary fibrosis, and glenohumeral radio lesions (2 cases), which result in blockag
