Sleep Disordered Breathing in Joubert Syndrome: Disease Spectrum in a Pediatric Cohort.

BACKGROUND: Joubert syndrome (JS) is a rare, autosomal recessive condition with multisystem manifestations. Sleep disordered breathing (SDB) is a prominent feature, with patients characteristically presenting with a mixture of hyperpnea and apnea. Reports describing whether the respiratory abnormalities change as patients grow older are lacking. METHODS: We describe the clinical course of 21 children with JS followed over 16 years in a tertiary pediatric hospital. RESULTS: Fourteen children underwent polysomnography, while 11 had multiple repeat studies. Patients exhibited a range of SDB, in

Evaluation of Secondary Post-Tonsillectomy Bleeding among Children in Saudi Arabia: Risk Factor Analysis.

BACKGROUND: Tonsillectomy is one of the most widely recognized surgeries for tonsillitis. It is the default surgery option for obstructive sleep apnea, recurrent tonsillitis, and recurrent peritonsillar abscess. However, there are limited risk factor analyses in Saudi Arabia related to tonsillectomy. The goal of study is to evaluate and analyze the risk factors for secondary post-tonsillectomy bleeding in Saudi Arabia. METHOD: This retrospective review study was directed at King Abdullah Specialized Children's Hospital, Riyadh, Saudi Arabia. Univariate and multivariate investigations were per

[A patient with olivopontocerebellar atrophy combined with sleep apnea syndrome].

A 52-year-old man with olivopontocerebellar atrophy was admitted to Ehime University Hospital for examination of sleep apnea. Overnight polysomnographic study revealed that his sleep apnea was predominantly the central type. With progress of the disease, the dominance of apnea pattern changed from the central type to the obstructive type. After tracheostomy, the apnea index decreased from 15 per hour to 12.4 per hour. However, the apnea index of the central type increased from 1.5 per hour to 12.4 per hour. These findings suggest that the effect of tracheostomy is not always beneficial in slee

Discharge after tonsillectomy in pediatric sleep apnea patients.

OBJECTIVES: Outpatient tonsillectomy has gained favor in recent years, however patients with obstructive sleep apnea/hypopnea syndrome have been excluded from outpatient surgery criteria. It is the practice of the senior author to discharge patients after tonsillectomy with a respiratory disturbance or apnea hypopnea index of 5 or less. The purpose of this study is to examine the respiratory complication rate based on respiratory disturbance or apnea hypopnea index, and co-morbidities in order to determine which pediatric patients with obstructive sleep apnea/hypopnea syndrome can be safely di

Central sleep apnea.

A central sleep apnea is the absence of respiratory effect, and, this, the absence of airflow during sleep. Central hypopnea, a related disorder, is also discussed. The sensory component of central sleep apnea; defects involving the integrative and executive neurons; non-neurologic causes of central sleep apneas, including chronic obstructive pulmonary disease and congestive heart failure; diagnosis; treatment; and other topics are reviewed.

[Apnea during sleep and wakefulness in term newborns].

Apneas in the different sleep states are commonly observed in the full-term healthy newborn infant. Central and short apneas prevail whilst apneas greater than 15 sec. are rare; a marked incidence of short apneas (less than 10 sec.) was observed in active sleep, even though apneas are not exclusive of active sleep. There is a strong inter-individual variability of apnea incidence. Apnea incidence in a state is not positively correlated to apnea incidence in different behavioural states: on the contrary there seems to be an opposite correlation between incidence in AS and QS. Apnea occurrence i

Dynamic Upper Airway Imaging during Wakefulness in Obese Subjects with and without Sleep Apnea.

RATIONALE: Obesity is a major risk factor for obstructive sleep apnea. Although greater dimensional changes in the upper airway during wake respiration have been noted in patients with apnea compared with control subjects, whether these differences remain in the presence of obesity is unknown. OBJECTIVES: To evaluate upper airway anatomic characteristics and airway compliance (distensibility) in obese subjects with obstructive sleep apnea compared with obese control subjects. METHODS: Dynamic magnetic resonance imaging was performed in 157 obese subjects with apnea and 46 obese control subje

[Diurnal sleep study for sleep related breathing disorders--the utility of nasendoscopic diurnal polysomnography using diazepam].

UNLABELLED: Twenty two patients with obstructive sleep apnea syndrome were examined by nocturnal polysomnography (n-PSG), and the obstructive sites in the upper airway were observed by nasendoscopic diurnal polysomnography using diazepam (n-d-PSGD). The types of apnea were divided into three groups according to the obstructive sites in the upper airway: soft palate apnea, tongue base apnea, and combined type apnea. Among the 22 patients, there were 17 soft palate apneas, 3 tongue base apneas and 2 combined type apneas. All patients underwent uvulopalatopharyngoplasty (UPPP). Preoperative value

Central sleep apnea.

Central apnea syndrome is a disorder with protean manifestations and concomitant conditions. It can occur as a distinct clinical entity or as part of another clinical syndrome. The pathogenesis of central sleep apnea (CSA) varies depending on the clinical condition. Sleep-related withdrawal of the ventilatory drive to breathe is the common denominator among all cases of central apnea, whereas hypocapnia is the final common pathway leading to apnea in the majority of central apnea. Medical conditions most closely associated with CSA include heart failure, stroke, spinal cord injury, and opioid

Targeting volume overload and overnight rostral fluid shift: A new perspective to treat sleep apnea.

Sleep apnea is a common condition associated with increased morbidity and mortality. Continuous positive airway pressure and oral appliances are efficient for treating sleep apnea; however, they are often poorly tolerated. Therefore, alternative therapies are needed. Overnight rostral fluid shift has been implicated in the pathogenesis of sleep apnea, particularly in conditions associated with fluid overload. Fluid shift predisposes to both obstructive and central sleep apnea, with the type of sleep apnea being related to whether the fluid shifts from the legs into the neck or chest, respectiv

Advances in Treatment of Sleep-Disordered Breathing.

BACKGROUND: Sleep-disordered breathing, composed of obstructive sleep apnea (OSA) and central sleep apnea (CSA), affects millions of people worldwide carrying with it significant morbidity and mortality. Diagnosis is made by polysomnography, and severity of sleep apnea is determined by the apnea-hypopnea index (AHI). Positive airway pressure (PAP) therapy has been the gold standard in treating both OSA and CSA. PAP therapy can greatly reduce AHI burden as well as morbidity and mortality and improve quality of life. AREAS OF UNCERTAINTY: However, patients report difficulties adhering to PAP th

Occurrence of Coronary Collaterals in Acute Myocardial Infarction and Sleep Apnea.

Background In patients with acute myocardial infarction (MI), cardioprotective effects of obstructive sleep apnea are postulated on account of hypoxemic preconditioning. The aim of this single-center substudy was to investigate a potential association between obstructive sleep apnea and the presence of coronary collaterals in patients with first-time acute MI who have been enrolled in an ongoing, multicenter clinical trial. Methods and Results In TEAM-ASV I (Treatment of Sleep Apnea Early After Myocardial Infarction With Adaptive Servo-Ventilation Trial; NCT02093377) patients with first acute

Effect of Sleep Disorders on Blood Pressure and Hypertension in Children.

PURPOSE OF REVIEW: In this review, we aim to discuss the pathophysiologic basis of hypertension in sleep disorders and the current evidence in the medical literature linking sleep disorders and hypertension in children. RECENT FINDINGS: The medical literature in adults is clear about the contribution of sleep disorders, poor sleep quality, and sleep deprivation to hypertension and increased cardiovascular risk. The literature on cardiovascular consequences of sleep disorders in children is not as robust, but there is some evidence of early cardiovascular changes in children with sleep depriva

The effect of chronic nocturnal oxygen administration upon sleep apnea.

Administration of nocturnal oxygen for 1 night to patients with obstructive sleep apnea (OSA) causes a moderate reduction in apnea frequency without improving hypersomnolence. Therefore, we administered oxygen chronically to patients with OSA to determine: whether apnea frequency would be further reduced, whether the effect of oxygen upon apnea frequency is correlated with an increased ventilatory response to hypoxia and hypercapnia, and whether hypersomnolence improves with more prolonged oxygen administration. In a single-blinded, nonrandomized trial, we compared the effects of 1 month of ox

Inferior sagittal osteotomy with hyoid bone suspension for obese patients with sleep apnea.

BACKGROUND: An inferior sagittal osteotomy with a sliding mandibular osteotomy and hyoid bone suspension, combined with a uvulopalatopharyngoplasty, is advocated in patients with obstructive sleep apnea who fail to tolerate nasal continuous positive airway pressure. Some investigators have suggested that this surgical procedure may be less effective in obese patients. METHODS: Preoperative and postoperative nocturnal polysomnograms and lateral cephalometric radiographs were obtained on 12 obese patients with obstructive sleep apnea who underwent these procedures. RESULTS: The mean +/- SD res

Sleep-disordered breathing in patients with myelomeningocele.

OBJECT A paucity of literature examines sleep apnea in patients with myelomeningocele, Chiari malformation Type II (CM-II), and related hydrocephalus. Even less is known about the effect of hydrocephalus treatment or CM-II decompression on sleep hygiene. This study is an exploratory analysis of sleep-disordered breathing in patients with myelomeningocele and the effects of neurosurgical treatments, in particular CM-II decompression and hydrocephalus management, on sleep organization. METHODS The authors performed a retrospective review of all patients seen in their multidisciplinary spina bifi

Mandibular advancement devices and treatment-emergent central sleep apnea (TECSA): Coincidental or critical finding?

Treatment-emergent central sleep apnea (TECSA) can occasionally be observed when obstructive sleep apnea (OSA) is managed with most therapeutic approaches, including the mandibular advancement device (MAD). Although TECSA appears to be a low-incidence condition, evidence is lacking on its mechanism and possible health risks. The primary objective of this critical review is to provide guidance to clinicians managing OSA with MAD. Prompt return to a sleep medicine specialist is recommended, however, when a patient continues to present complaints or reports potentially associated with central sle

Sleep disordered breathing in patients with heart failure.

Sleep disordered breathing (SDB) is a common co-morbidity in patients with heart failure (HF). Both its forms - central and obstructive sleep apnea - are highly prevalent in this population. SDB fragments sleep, impairs life quality, worsens exercise tolerance, worsens HF and is an independent predictor of poor prognosis. Still, SDB remains largely undiagnosed. Therefore, early detection of SDB seems to be of primary importance, especially in the presence of new diagnostic and therapeutic methods. Treatment with continuous positive airway pressure (CPAP) increases left ventricular ejection fra

Systematic Comparison of Respiratory Signals for the Automated Detection of Sleep Apnea.

Sleep apnea is one of the most common sleep disorders. It is characterized by the cessation of breathing during sleep due to airway blockages (obstructive sleep apnea) or disturbances in the signals from the brain (central sleep apnea). The gold standard for diagnosing sleep apnea is performing an overnight polysomnography recording which contains, among others, a wide array of respiratory signals. Respiration information can also be extracted from other physiological signals such as an electrocardiogram or from a bio-impedance measurement on the chest. Studies have shown that algorithms can b
