Triceps to biceps transfer for restoration of elbow flexion following upper brachial plexus injury.

INTRODUCTION: Upper brachial plexus injury in adults causes loss of elbow fl exion; when the primary nerve surgery has failed or the patient seeks treatment after 12 months of injury and pedicled muscle transfers are required. Most commonly, the latissimus dorsi or the Steindler flexorplasty is used. MATERIAL AND METHODS: We have transferred one of the heads of triceps muscle to restore the elbow flexion in such cases. In addition to return of elbow flexion, extension of elbow following surgery is retained. Ten patients suffering from upper brachial plexus injuries underwent transfer of one h

Effects of anti-aggregant, anti-inflammatory and anti-coagulant drug consumption on the preparation and therapeutic potential of plasma rich in growth factors (PRGF).

The prevalence and incidence of trauma-related injuries, coronary heart disease and other chronic diseases increase dramatically with age. This population sector is therefore a regular consumer of different types of drugs that may affect platelet aggregation and the coagulation cascade. We have evaluated whether the consumption of acetylsalicylic acid, acenocoumarol, glucosamine sulfate and chondroitin sulfate, and therefore their presence in blood, could interfere with the preparation and biological outcomes of plasma rich in growth factors (PRGF). Clotting time, clot retraction and platelet

Chronic traumatic diaphragmatic hernia with pericardial rupture and associated gastroesophageal reflux.

Major thoracic and abdominal trauma damages the diaphragm 5% of the time. These injuries may be recognized when they occur but often are discovered months later during work up for related symptoms. Typically, the injury is to the left posterolateral aspect of the diaphragm. Rarely, rupture through the central diaphragmatic tendon into the pericardial space occurs and this results in different symptoms than the more common injury. We present the case of a patient who presented with chest pain, near syncopal episodes and refractory gastroesophageal reflux years after he was struck by a car and h

Arthroscopic reconstruction of the anterior cruciate ligament with patellar-tendon autograft and interference screw fixation. The results at seven years.

Deficiency of the anterior cruciate ligament (ACL) is a common disorder which can lead to changes in lifestyle. We followed 59 patients who had had arthroscopic reconstruction of the ACL using a central-third patellar-tendon autograft for seven years to assess the long-term effectiveness of recent advances in reconstruction of the ACL. The standard criteria for evaluation of the International Knee Documentation Committee, the Lysholm knee score and measurements using the KT 1000 arthrometer all showed satisfactory results. Deterioration in the clinical performance after seven years was associa

Risk factors for restricted motion after anterior cruciate reconstruction.

A retrospective review of 373 patients who had undergone anterior cruciate ligament (ACL) reconstruction utilizing the central third of the patellar tendon was undertaken to identify those factors that placed a patient at risk for restricted postoperative motion (flexion < or = 125 degrees or flexion contracture > or = 10 degrees). Stepwise logistic regression analysis determined that the variables most strongly correlated with restricted final range of motion (ROM) were open surgery (P = .0008) and reconstruction performed < or = 7 days after the initial injury (P = .004). Age, associated men

Destructive Arthropathy of the Interphalangeal Joint Following Revascularization After Degloving Hand Injury: A Case Report.

It is difficult to achieve satisfactory results in the treatment of advanced degloving injury, which is one of the most challenging injuries in hand surgery. In this report, we present a case of marked destructive arthropathy of the interphalangeal joint that developed following revascularization after degloving hand injury. A 37-year-old, right-handed female manual worker sustained a degloving injury of her fingers, including the dorsal hand. We performed revascularization and secondary surgery, which included tenolysis and a tendon graft, to obtain functional improvement. These 2 procedures

Suicidal wrist bites.

Wrist injuries are not uncommon in forensic routine and are usually found in the context of suicides or as a result of psychiatric illnesses, e.g., borderline disorders. Sharp objects (knives, broken glass, etc.) are usually used. In the case reported here, a paranoid-schizophrenic man not only injured himself with razor blades on both wrists, but he also inflicted extensive wrist bite injuries using his dental prosthesis. In addition to the severance of flexor tendons, venous vessels and the left radial artery were torn with subsequent blood loss. At the time of death, there was also acute ex

Pediatric hand ultrasound: common indications, injury, inflammation and masses.

Ultrasound is a uniquely valuable tool for evaluating musculoskeletal problems in the pediatric hand. Acute and chronic injuries involving tendons and ligaments of the hand can be quickly assessed and can guide surgical decision-making. Using ultrasound, palpable lesions and masses can be evaluated for cystic and solid components aiding in differential diagnosis. Additionally, ultrasound provides evaluation of acute versus chronic changes of inflammatory arthritis, assessing disease severity and subclinical synovitis and serving as an adjunct to medical management. This review will cover commo

Traumatic Divergent Elbow Dislocation with Associated Forearm Amputation in an Adult: A Case Report.

CASE: A 49-year-old right-hand-dominant man sustained an auger-related injury that resulted in open dislocation at the left wrist and palm, with complete amputation of the distal aspect of the forearm and the hand. The injury at the elbow included instability with an ulnar coronoid fracture, posterior dislocation of the ulna, and posterolateral dislocation of the radius. To restore stability of the forearm stump and elbow, we performed a complete resection of the radius, open reduction and internal fixation of the coronoid tip, a repair of the lateral collateral ligament, and transfer of the d

Mechanisms and Therapeutic Potential of Stem Cell-Derived Extracellular Vesicles as Nanocarriers in Musculoskeletal Disorders.

The musculoskeletal system consists of bones, joint cartilage, tendons, ligaments, muscles, and their associated nerves and blood vessels. These components work together to maintain human movement and mechanical stability, serving as the critical foundation for sustaining life activities. In recent years, with the acceleration of population aging, the incidence of musculoskeletal-related diseases such as sarcopenia, muscle atrophy, osteoporosis (OP), osteoarthritis (OA), and tendon injuries has continued to rise, significantly impacting patients' quality of life and imposing a heavy social hea…

Is cryotherapy effective for tendon health? A systematic review of evidence and methodological limitations.

BACKGROUND: Cryotherapy is widely used in managing soft tissue injuries, but its scientific foundation and clinical application remain inconsistent. Human evidence-especially concerning tendinopathy-is limited and heterogeneous. Understanding the effects of cold exposure on tendon structure and function is essential for safer and more effective clinical practice. OBJECTIVE: To systematically review the clinical, mechanical, and physiological effects of cryotherapy on human tendons, both healthy and affected by tendinopathy. METHODS: Comprehensive searches were conducted in CENTRAL, MEDLINE,

Autograft Choice for Anatomic Reconstruction of Lateral Ankle Ligaments in Chronic Ankle Instability: A Systematic Review.

Anatomic reconstruction of the lateral ligamentous ankle complex with an autograft is indicated as a surgical approach in chronic ankle instability management. This approach benefits ankles deficient in ligamentous support and those with greater occupational or sporting demands. Autograft choice varies in clinical practice and is driven by surgeon's choice and patient-specific ankle demands. Current literature does not favor any particular autograft with respect to functional outcomes or postoperative complications. This review systematically compares common autografts through patient-reported

Breaking points of human hamstring muscle-tendon complex: A cadaveric study.

Hamstring strain injuries frequently occur during the late swing phase of running; however, the breaking strength of the human hamstring muscle-tendon complexes remains unclear. This study aimed to determine the breaking strength of human hamstring muscle-tendon complexes. To this end, seven cadaveric specimens preserved using the Thiel's method were examined. The biceps femoris long head (BFlh), semimembranosus (SM), and semitendinosus (ST) muscles were isolated and secured to a testing apparatus. Using a material testing machine, tensile load (P) was applied to the distal end of each muscle.

A SIRT1-independent mechanism mediates protection against steroid-induced senescence by resveralogues in equine tenocytes.

Tendinopathy is a common age-related disease which causes significant morbidity for both human athletes and performance horses. In the latter, the superficial digital flexor tendon is an excellent model for human tendinopathies because it is a functional homologue of the human Achilles tendon and a primary site of injuries with strong similarities to the human disease. Corticosteroids have been previously used clinically to treat tendinopathic inflammation, but they upregulate the p53-p21 axis with concomitant reductions in cell proliferation and collagen synthesis in human tenocytes. This phe

Association between running biomechanics and lower limb musculotendinous changes over an adolescent marathon training program.

BACKGROUND: Prospective musculotendinous changes have been observed among adolescents completing marathon training programs. However, examinations of potential factors influencing musculotendinous characteristics do not currently exist. Evaluations of biomechanical parameters and the influence on musculotendinous features and changes over time are warranted. RESEARCH QUESTION: What are the relationships between adolescent running biomechanics and 1) lower extremity musculotendinous characteristics cross-sectionally, and 2) changes in musculotendinous properties over 6-months? METHODS: Thirty

Assessing site-specificity of the biomechanical properties of hamstring aponeuroses using MyotonPRO: A cadaveric study.

BACKGROUND: Hamstring muscles are the most frequently reported sites of muscle strain injuries, especially near the bi-articular muscles' myotendinous junction, where aponeurosis provides a connective tissue network linking muscle fibers to the tendon. This study aimed to investigate the reliability and site-specific differences of hamstring aponeuroses under different conditions (formalin and urea) using MyotonPRO. METHODS: Eight hamstring muscle groups were dissected from four human cadavers (two males and two females) aged 83-93 years. Measurements of the mechanical properties of the apone

Automatic MRI-based rotator cuff muscle segmentation using U-Nets.

BACKGROUND: The rotator cuff (RC) is a crucial anatomical element within the shoulder joint, facilitating an extensive array of motions while maintaining joint stability. Comprised of the subscapularis, infraspinatus, supraspinatus, and teres minor muscles, the RC plays an integral role in shoulder functionality. RC injuries represent prevalent, incapacitating conditions that impose a substantial impact on approximately 8% of the adult population in the USA. Segmentation of these muscles provides valuable anatomical information for evaluating muscle quality and allows for better treatment plan

Polarized macrophages regulate fibro/adipogenic progenitor (FAP) adipogenesis through exosomes.

BACKGROUND: Macrophage polarization has been observed in the process of muscle injuries including rotator cuff (RC) muscle atrophy and fatty infiltration after large tendon tears. In our previous study, we showed that fibrogenesis and white adipogenesis of muscle residential fibro/adipogenic progenitors (FAPs) cause fibrosis and fatty infiltration and that brown/beige adipogenesis of FAPs promotes rotator cuff muscle regeneration. However, how polarized macrophages and their exosomes regulate FAP differentiation remains unknown. METHODS: We cultured FAPs with M0, M1, and M2 macrophages or 2 ×
