Right Brain: Finger-Nose, There and Back Again.

This poetic reflection highlights how routine bedside tests become opportunities for multifaceted learning for trainees. The "finger-to-nose" test used in neurologic examinations not only provides insight into neurologic signs but also teaches about clear communication, patience, and connecting with the patient's way of thinking. Drawing on real-life, day-to-day clinical encounters, the piece explores the patient's often misinterpreted instructions and the unexpected responses they provoke as the trainee learns to administer the neurologic examination.I say it gently, as a part of my examinati

Brain compliance: a new assessment for clinical practice?

The correlation between pulse wave velocity (PWV) and brain compliance acquired using Brain4care (B4C) monitors is an emerging field of study that aims to better understand cerebrovascular health and its implications for cognitive function and brain aging. Increased arterial stiffness, often due to aging or hypertension, impairs brain compliance, which is crucial for maintaining cerebral homeostasis. This impairs cerebral perfusion, causing microvascular brain damage, which may contribute to long-term cognitive impairment. The B4C sensors monitor cerebral compliance through the analysis of pul…

Neuroimmune dynamics and cytokines in traumatic brain injury.

Traumatic brain injury (TBI) is a leading cause of neurological disability, associated with higher rates of cognitive complications that negatively affect recovery. Myriad cytokine and chemokine pathways propagate the secondary responses to injury. This review discusses the integration of peripheral and central immune cytokine and chemokine signaling cascades after TBI and recovery, with a focus on preclinical work. We first discuss key cytokine and chemokine interactions influencing recovery and long-term deficits. Next, we discuss the major cell types that propagate and respond to the inflam…

Brain Abnormalities in Prenatally Diagnosed Rubinstein-Taybi Syndrome.

Rubinstein-Taybi syndrome (RSTS; OMIM 180849) is a rare autosomal dominant disorder characterized by craniofacial dysmorphism, broad halluces and thumbs, variable structural abnormalities and intellectual disability. It is caused by a pathogenic variant in the CREBBP or EP300 genes. Although distinctive features are described, prenatal diagnosis is rare. We report five prenatally diagnosed cases of RSTS, all with brain abnormalities. On neurosonography, corpus callosum abnormalities were found in 3/5 cases. The remaining two cases had abnormal posterior fossas, one with vermian hypoplasia and …

Early structural brain abnormalities in borderline personality disorder.

BACKGROUND: Structural imaging studies of borderline personality disorder (BPD) have identified regions of reduced and increased cortical volume, as well as volume reductions in the hippocampus and amygdala, although with considerable variability across studies. Examining adolescent patients with the disorder can reduce potential confounding effects such as later development of affective and other comorbid disorders. METHODS: Fifty-one adolescents (48 females, 3 males) with BPD and without comorbid disorders and with 43 matched healthy controls underwent whole-brain voxel-based morphometry (V…

Endothelial gateways for brain lipid uptake and metabolism.

Lipids, which constitute half of the brain's solid matter, are essential for forming specialized membranes of neural cells, providing energy sources, and facilitating cell-to-cell communication. Although the blood-brain barrier restricts lipid movement between peripheral circulation and the brain, multiple mechanisms supply the building blocks necessary to synthesize the diverse lipid species present in the central nervous system (CNS). In this issue of the JCI, Song et al. characterize specialized microvascular niches that metabolize circulating triglyceride-rich lipoproteins (TRLs) to delive…

Estimation of brain activity sources of sympathovagal dynamics.

Characterizing source brain activity in relation to peripheral neural-autonomic function is crucial in neuroscience research. Despite recent advances for estimating brain-heart interplay (BHI), the specific intracortical sources underlying this interaction remain poorly understood, and especially a non-invasive model to localize the brain source of cardiac autonomic functions is lacking. In an effort to estimate the joint intrinsic activity of central and autonomic systems, this study extends canonical EEG source localization by introducing a framework for brain source reconstruction of sympat…

Gut-brain communication: Functional anatomy of vagal afferents.

There is increasing interest in interoceptive mechanisms as a key player in mental health. The vagus nerve is an important pathway of communication between the body and the brain, and recent advances in neurobiological techniques have enabled the identification of function-specific populations of vagal sensory neurons. Here we briefly review this progress, focusing on vagal innervation of the gut and its involvement in ingestive behavior, metabolic regulation, and immune defense. While we have learned much about the organization of the peripheral interface of the sensory vagal system, dissemin…

Hypoglycemia-Induced Irreversible Brain Injury: A Case Report.

Hypoglycemia-induced brain injury, though less common than hyperglycemia, can lead to significant neurological sequelae and mortality. We present a case of a 76-year-old female with a history of poorly controlled diabetes who presented with altered mental status and unresponsiveness. Despite the correction of hypoglycemia and supportive medical management, the patient's neurological status did not improve. Neurological examination revealed myoclonus and severe encephalopathy, prompting further investigation. Imaging studies, including contrast-enhanced CT and MRI of the head, demonstrated find…

Brain regulation of pulmonary dysfunction induced by stroke.

Ischaemic stroke, characterized with sterile brain tissue injury, can lead to acute pneumonia and pulmonary dysfunction, which are the primary causes of death in ischaemic stroke patients. The association of neural and molecular mechanisms between the infarcted brain area and the development of pneumonia and pulmonary dysfunction are not understood. We used whole-body plethysmography, the respiratory patterns test and arterial blood gas analysis to examine pulmonary function. In addition, Holter monitoring, electrophysiological recordings, chemogenetic manipulation, fibre photometry, ELISA, im…

The Heart-Brain Axis: Key Concepts in Neurocardiology.

The heart-brain axis involves complex interactions between the cardiovascular and nervous systems via a network of cortical and subcortical structures working with the autonomic nervous system and intracardiac nervous system. Heart-brain interactions may be divided into 2 broad categories: cardiac effects of neurological disease and neurological effects of cardiac disease. The pathogenesis of neurogenic cardiac effects is thought to involve a neurogenic cascade where sudden shifts in autonomic balance lead to an exaggerated catecholamine release. This can occur in acute neurological conditions…

Impaired Functional Brain-Heart Interplay Sustains Depressive Symptomatology.

Depressive symptoms are a leading worldwide cause of mental disorders and disability, strongly affecting emotional processing and regulation. Leveraging recent evidence on the cardiogenic generation of emotion, we hypothesize that dysfunctional behavior in depressive symptomatology is sustained by impaired nervous-system-wise dynamics. Accordingly, this study aims to experimentally characterize functional Brain-Heart Interplay (BHI) patterns specific to emotional dysregulation and processing in subjects exhibiting depressive symptoms compared to healthy controls. Functional BHI has been estima…

Dietary Therapy for Disorders of Gut-Brain Interaction.

Disorders of gut-brain interaction (DGBI) in pediatrics such as irritable bowel syndrome, functional abdominal pain, and functional dyspepsia pose a challenging set of syndromes that limit patient quality of life and may impair optimal nutritional intake. Nutrition plays a key role in management of DGBI by promoting gut motility and modulating visceral sensations. Therapeutic diets can be safely implemented toward the treatment of pediatric DGBI, but it is essential that providers understand the roles and limitations of various diets and assess for restrictive patterns of eating.

Unraveling the Brain-Heart Axis: More Than Inflammation.

Cardiovascular disease remains the leading cause of death worldwide, with a rising burden projected over the coming decades. Although traditional risk factors such as diabetes, hypertension, and dyslipidemia form the main targets of prevention strategies, many individuals carry a "residual" risk secondary to systemic inflammation which remains underrecognized. Landmark trials have established inflammation as a causal and modifiable driver of atherosclerotic cardiovascular disease. This review focuses on how inflammation can modulate interconnected diseases of the brain and heart. Growing evide…

[Deep Brain Stimulation: From History to Recent Advances].

Deep brain stimulation (DBS) has been a common treatment for Parkinson's disease, tremors, and dystonia since its approval in Japan for movement disorders in 2000. In the EU and USA, DBS has been studied as a therapeutic option for depression, obsessive-compulsive disorder, Tourette syndrome, and cluster headache. DBS functions via a neurophysiological mechanism in movement disorders rather than pharmacological mechanisms. In this review, I have attempted to provide an overview of DBS from its history to the most recent information.
