Dieulafoy's Lesion of the Major Duodenal Papilla Presenting as Obscure Upper Gastrointestinal Bleeding: A Case Report.

Dieulafoy's lesion is an uncommon vascular anomaly that can cause life-threatening gastrointestinal bleeding. While the stomach is the most frequently affected site, involvement of the major duodenal papilla is exceedingly rare and may escape detection with conventional forward-viewing endoscopy. We report a rare case of papillary Dieulafoy's lesion presenting as obscure upper gastrointestinal bleeding in a patient receiving rivaroxaban. A 69-year-old woman with deep vein thrombosis receiving rivaroxaban, hypertension, and hypothyroidism presented with recurrent melena and severe anemia (hemog

Chest Pain Due to Anomalous Origin of the Left Circumflex Artery From the Opposite Sinus of Valsalva in a 70-Year-Old Woman.

BACKGROUND An anomalous origin of a coronary artery from the opposite sinus of Valsalva (ACAOS) is a rare congenital coronary abnormality that is usually asymptomatic throughout life. However, an anomalous coronary artery arising from an atypical aortic location can lead to myocardial ischemia, exertional symptoms, or even sudden cardiac death. Therefore, early recognition and risk stratification are essential. Clinical management of ACAOS should be highly individualized and guided by detailed anatomical features, clinical symptoms, and evidence of ischemia. This report describes an older woma

Sport, exercise, and physical activity in children with cardiovascular disease. A Clinical Consensus Statement of the European Association of Preventive Cardiology (EAPC) of the ESC, the Association of Cardiovascular Nursing & Allied Professions (ACNAP) of the ESC, and the Association for European Paediatric and Congenital Cardiology (AEPC).

Regular physical activity offers numerous benefits to children's health and well-being and is essential for reducing cardiovascular risk in children with cardiovascular disease. Despite increasing evidence supporting its benefits, many young patients remain insufficiently active because of concerns regarding exercise related risks, limited expertise. This clinical consensus statement provides practical guidance for the prescription of exercise and physical activity in children and adolescents aged 6 to 17 years with cardiovascular disease. The document aims to bridge the gap between evidence s

Metabolic activation and cytotoxicity of carvedilol mediated by cytochrome P450s in vitro and in vivo.

Carvedilol (CAR) is commonly administered in the treatment of essential hypertension. Current reports suggest that CAR therapy may elevate the risk of hepatotoxicity, occasionally progressing to liver injury. However, the underlying mechanisms of the toxicity remain poor understood. This study investigated CAR-associated hepatotoxicity through reactive metabolites formation. In the microsomal incubation mixture containing CAR (50 μM), four phase I metabolites (M1-M4) were detected. Upon the addition of glutathione (GSH), N-acetylcysteine (NAC), or cysteine as trapping agents, four GSH conjugat…

Alcohol Consumption and Acute Coronary Syndrome: Epidemiology, Pathophysiology, and Clinical Perspectives.

Alcohol consumption is a globally prevalent lifestyle factor with complex and sometimes paradoxical effects on cardiovascular health, particularly regarding acute coronary syndrome (ACS). Earlier epidemiological studies described a J-shaped relationship between alcohol consumption and ACS risk; however, emerging evidence has increasingly challenged the validity of this concept. Mendelian randomization studies, genetic data, and recent pooled analyses suggest that the apparent cardioprotective effects of light-to-moderate drinking are largely attributable to residual confounding, including abst

Utility of serum ERAP1 and ERAP2 as novel biomarkers in the management of recalcitrant warts: A cross-sectional study.

Background The exact mechanisms underlying the eradication of the human papillomavirus (HPV) by cellular immunity remain obscure. Individuals with treatment-resistant warts frequently have immune deficiencies. Amino-peptidases (ERAP1, ERAP2) of the endoplasmic reticulum are essential for the production of antigenic epitopes that attach to the Major Histocompatibility Complex (MHC) class I and activate T-lymphocytes or natural killer (NK) cells. Aim To assess ERAP1 and ERAP2 serum concentrations in patients with resistant warts. Methods The current study included 200 subjects. They were split i

Natural pregnancy following kidney transplantation in an azoospermic male: a case report.

BACKGROUND: Chronic kidney disease (CKD) and its advanced form, end-stage kidney disease (ESKD), disrupt male reproductive capacity, largely due to endocrine disturbances and impaired sperm production. Kidney transplantation (KTx) may reverse these effects by normalizing kidney function and recalibrating the hypothalamic-pituitary-gonadal (HPG) axis. Here, we report the first known case of natural conception in a male with ESKD and non-obstructive azoospermia (NOA) after KTx. CASE DESCRIPTION: A 32-year-old man experienced reduced sexual desire, mild erectile dysfunction, and infertility for

Mayer-Rokitansky-Küster-Hauser Syndrome: Where Does Gynaecological Pathology End and Renal Disease Begin? The Value of a Comprehensive View. Two Case Reports with Adult Onset Kidney Disease and A Review of the Literature.

Background. Mayer-Rokitansky-Küster-Hauser (MRKH) syndrome is a rare congenital condition marked by agenesis or hypoplasia of the upper female reproductive tract. While it is typically recognized for its reproductive implications, renal anomalies - such as unilateral renal agenesis or ectopic kidneys - are also frequent but often underdiagnosed. Recent findings suggest shared embryological origins between the genital and urinary systems, supporting the need for a broader diagnostic and therapeutic perspective. Case Description. We describe two cases of women with MRKH syndrome diagnosed in ado

Postoperative changes of wrist-brachial index following arteriovenous fistula implantation correlate with steal syndrome, a prospective study.

Arteriovenous fistula (AVF) is the preferred vascular access for hemodialysis in end-stage renal disease. Steal syndrome, a complication causing limb ischemia, may follow AVF implantation. This study evaluated whether postoperative wrist-brachial index (WBI) changes predict steal syndrome, including severe cases requiring intervention. In a prospective study, patients undergoing first-time AVF implantation in 2022 were enrolled. WBI was measured preoperatively, immediately postoperatively, at 2 weeks, and at 3 months. Steal syndrome symptoms (pain, pallor, coldness, numbness, and paresthesia)

Perspective on Renal Involvement in Antiphospholipid Syndrome: Implications for Diagnosis, Pathogenesis, and Treatment.

Antiphospholipid syndrome (APS) can affect the kidneys, leading to renal artery and vein thrombosis, allograft loss following transplantation, and microvascular damage referred to as aPL-nephropathy (aPL-N). APL-N is a complex and frequently underdiagnosed condition characterized by an incomplete understanding of its etiopathogenesis and associated with unfavorable renal outcomes. The 2023 ACR/EULAR classification criteria for APS included aPL-N within the microvascular domain. The gold standard for aPL-N is the biopsy, revealing lesions associated with acute thrombotic microangiopathy and chr

Severe pulmonary arterial hypertension development despite lupus nephritis remission: association with podocytic infolding glomerulopathy.

BACKGROUND: Pulmonary arterial hypertension (PAH) associated with systemic lupus erythematosus (SLE) represents a significant complication with incompletely understood pathogenesis. Notably, podocytic infolding glomerulopathy (PIG), a rare pathological phenomenon in SLE-related renal lesions, has not been previously linked to PAH development. The cardiovascular safety profile of belimumab, a biologic agent for SLE treatment, requires continued surveillance, particularly regarding potential pulmonary vascular complications. CASE PRESENTATION: We report a 30-year-old male with SLE who unexpected
