The safety of corticosteroid therapy in Crohn's disease with an abdominal mass.

Steroid therapy is often avoided in the Crohn's disease patient with a palpable abdominal mass, for fear of dissemination of infection, should the mass prove to contain an abscess. In the present study, 24 patients with Crohn's disease and a palpable abdominal mass were treated with high dose steroids. In 15, the mass resolved completely and, in another nine, it decreased in size by at least 50%. Fourteen of 24 patients eventually required resection for persistence or recurrence of Crohn's disease activity with or without the abdominal mass, but in all the operation was performed electively. A…

Spelunking as a manifestation of a counterclaustrophobia.

Material is presented from the case history of a patient whose interest in spelunking (cave exploration) was found to be an unconscious expression of a type of counterclaustrophobia. Both oedipal and preoedipal determinants of the claustrophobic anxieties are delineated. Of particular note in this instance is the testicular element in the genesis of the patient's claustrophobia. His confusion of the movements of his testicles into his inguinal canals during childhood defecation with the movements of the feces themselves lent an special intensity to his fear of being flushed away from the mothe…

An anlysis of three parameters of covert positive reinforcement.

Investigated the active components of covert positive reinforcement (CPR) with a 2 X 2 X 2 factorial design (N = 64) that consisted of an S- … ups (p less than .01). Self-rating measures of anxiety on the Self-Rating Rat Anxiety Scale and the Fear Intensity Scale reflected overall decrements of fear from pretreatment to posttreatment and follow-up. Follow-up analyses indicated that plans should be made for the generalization of treatment effects by varying the treatment setting and perhaps by …

Paternal domination as a cause of somnambulism.

The cases of 15 men admitted to the neuropsychiatric service of a Navy hospital for somnambulism indicate that sleepwalking is an aggressive or sexual motor activity seemingly aimed primarily at a fear-inspiring father. Although the patients varied in age from 18 to 36 years and in rank from seaman to chief petty officer, eight being married and seven single, all were from small-town or rural homes in the low economic class. Their training was religious and strict. They professed great respect for their fathers, to the extent that they could not criticize them or acknowledge hostility toward t…

A latent profile analysis of romantic attachment anxiety and avoidance.

We conducted latent profile analyses on community (n = 1663) and clinical (n = 575) samples to determine whether continuous scores of attachment anxiety and avoidance would lead to the identification of theoretically consistent and clinically useful profiles. We then compared these profiles according to gender, relationship status, psychological distress, and relationship satisfaction. Analysis on the community sample yielded four profiles: secure, preoccupied, dismissive, and fearful individuals; whereas, the clinical sample yielded three profiles: secure, preoccupied, and fearful individuals…

Blunted neural response to implicit negative facial affect in anorexia nervosa.

BACKGROUND: People with anorexia nervosa (AN) have difficulties in a wide range of social-emotional processes. Previous work suggests atypical involvement of the prefrontal cortex (PFC), amygdala, insula, and fusiform gyri during social-emotional processing in AN. METHODS: Twenty women with AN and twenty healthy comparison (HC) women were presented with happy, fearful, and neutral faces during a functional magnetic resonance imaging study. Group differences were investigated in the following regions of interest: lateral PFC, amygdala, insula, and fusiform gyri. RESULTS: The HC participants s…

Overcoming the Social Stigma on Mood Disorders with Dancing.

In our society, the social stigma against people who suffer from mood disorders is a very powerful factor that negatively affects the healing of patient. He is often isolated from the others for the fear of being judged "fool, crazy or dangerous" or discriminated and emarginated for his mental health problem. For this reason, a cornerstone of mood disorder rehabilitation is the bringing out of the patient from his isolation, the reintegration of the user in the social context with the increase and the improvement in the quality of interpersonal relationships in the family and in the external c…

The effects of emotion priming on visual search in socially anxious adults.

This study examined the effects of emotion priming on visual search in participants characterised for different levels of social anxiety. Participants were primed with five facial emotions (angry, fear, happy, neutral, and surprised) and one scrambled face immediately prior to visual search trials involving finding a slanted coloured line amongst distractors, as reaction times and accuracy to target detection were recorded. Results suggest that for individuals low in social anxiety, being primed with an angry, surprised, or fearful face facilitated visual search compared to being primed with s…

Sub-clinical anxiety and the onset of alcohol use disorders: longitudinal associations from the Baltimore ECA follow-up, 1981-2004.

The current study examines the longitudinal relationship between anxiety disorders in general, specific phobia in particular, and subsequent … e baseline interview in 1981. Although baseline specific phobias were common, neither the number of fears nor any specific fear was statistically associated with the onset of alcohol use disorders. Rather, the findings suggested a modest association between adult sub-clinical specific phobia (without substantial distress or interference) and later-onse …

CONCLUSIONS: Overall, these studies provide preliminary evidence for a self-verification model of social anxi …

Anxiety sensitivity in adolescents with somatoform autonomic dysfunction and adolescents with insulin dependent diabetes mellitus.

Anxiety sensitivity is defined as a belief that anxiety or fear may cause illness, embarrassment, or additional anxiety. The main purpose of this study was to find out if there were differences among adolescents with insulin dependent diabetes mellitus, adolescents with somatoform autonomic dysfunction and their healthy peers in different aspects of psychological functioning and anxiety sensitivity. The sample consisted of 93 subjects, aged 12 to 16. Hamburg Neuroticism and Extraversion Scale, Child Behaviour Checklist and Childhood Anxiety Sensitivity Index were administrated. The adolescents…

BACKGROUND: Both behavioral inhibition system (BIS) and behavioral approach system (BAS) sensitivity have been proposed to play a role in so … cial observation anxiety across all three samples. In addition, individuals with generalized social fears were found to report both higher levels of BIS and lower levels of BAS compared to individuals with few or specific social fears. CONCLUSION: These findings suggest that a complete motivational account of generalized social anxiety should include …

Diversity of effective treatments of panic attacks: what do they have in common?

By comparing efficacious psychological therapies of different kinds, inferences about common effective treatment mechanisms can be made. We … es is that they reduce the immediate expectancy of a panic attack, disrupting the vicious circle of fearing fear. Modifying expectation is usually regarded as a placebo mechanism in psychotherapy, but may be a specific treatment mechanism for panic. The fact that this is seldom the rationale communicated to the patient creates a moral dilemma: Is it et …

How preferential is the preferential encoding of threatening stimuli? Working memory biases in specific anxiety and the Attentional Blink.

Temporal visual working memory (VWM) biases in spider anxiety were studied with an Attentional Blink paradigm. In Experiment 1, participants viewed pictures sequentially presented at rates of 80 ms and were instructed to memorize two target pictures. We varied time between targets and valence of the second target (neutral: mushroom, positive: blossom, negative: spider). In Experiment 2, spider fearfuls and non-anxious controls (both without snake anxiety) participated. Here we tested two negative targets: disorder-related spiders and disorder-irrelevant snakes. Both positive and negative items…

Brief report: factor structure of the Childhood Illness Attitude Scales (CIAS).

OBJECTIVE: To examine the factor structure of the Childhood Illness Attitude Scales (CIAS). The CIAS is a 35-item self-report measure based on the Illness Attitudes Scales, designed for use with school-age children. The CIAS measures fears, beliefs, and attitudes associated with health anxiety and abnormal illness behavior in childhood. METHODS: CIAS item responses for 201 school-age children were subjected to principal-components analysis with oblique rotation. RESULTS: The CIAS was best conceptualized as comprising four factors: fears, help seeking, treatment experience, and symptom effect…

Musician's cramp: a case report and literature review.

Musician's focal dystonia is a motor dysfunction that appears in artists after years of repetitive and fine movements during performance. This is the condition most feared by musicians because it leads to difficulties in controlling movements, which can interrupt or terminate their careers. It is characterized by the onset of involuntary muscle contractions and movements; its distinguishing feature is that it only occurs during a specific and well-defined action. It is rarely diagnosed because those who experience it do not seek medical attention for fear or shame, but also because many physic…
