Maternal vitamin A deficiency and neonatal microphthalmia: complications of biliopancreatic diversion?

Biliopancreatic diversion (BPD) for morbid obesity carries a serious risk of nutritional deficiencies that might impair embryogenesis. Consequently, attention should be given to the potential of risk to the fetus of BPD in women of childbearing age. We present the case of a pregnant woman who had undergone BPD 8 years previously, with documented vitamin A deficiency, who gave birth to a child with bilateral microphthalmia. Infectious and genetic causes of microphthalmia were excluded. A search of the literature revealed that vitamin A deficiency may cause a disruption of ocular development. We

Autistic behaviors in offspring of mothers abusing alcohol and other drugs: a series of case reports.

Although autistic-like behaviors were described even in the earliest reports of fetal alcohol syndrome, it was only recently that fetal alcohol syndrome and autism were reported as a dual diagnosis in six school-aged children. The purpose of the present series of case reports is to describe marked autistic characteristics in three much younger children (25-36 months) with histories of prenatal exposure to alcohol and other drugs. The behavioral characteristics of these children are described and compared with current diagnostic criteria for autistic disorder. In addition, longitudinal scores o

The relation between pulmonary hypoplasia and amniotic fluid volume: lessons learned from discordant urinary tract anomalies in monoamniotic twins.

BACKGROUND: Adequate amniotic fluid (AF) volume is one of several factors felt to be essential for normal lung development. Renal agenesis and urinary tract obstruction usually result in oligohydramnios and pulmonary hypoplasia. CASE: Two sets of monoamiotic twins with discordant urinary tract anomalies were seen. One twin in each set had anomalies that in a singleton or diamiotic pregnancy would likely have resulted in fetal pulmonary hypoplasia and subsequent death. However, neither of these infants had pulmonary hypoplasia. One infant is unique in being the first case reported of normal pu

Dermatoglyphic asymmetry in fetal alcohol syndrome.

The hypothesis was tested that prenatal alcohol exposure disrupts developmental homeostasis, as reflected in increased dermatoglyphic fluctuating asymmetry. Twenty-two patients with fetal alcohol syndrome (FAS) and 9 with fetal alcohol effect (FAE) were matched for sex with 31 controls. On each patient, the right a-b dermal ridge count was subtracted from the left to obtain the asymmetry value. Group differences were tested by analysis of variance (ANOVA), and paired two-tailed t tests. Mean asymmetry increased from control (1.94) through FAE (2.78) to the FAS (4.00) group. Group differences w

[Status of the cardiovascular system of the fetus and newborn infant in different degree of severity of maternal hypertensive syndrome].

Examinations of 260 pregnant women with hypertensive syndromes and their 60 full-term newborns, carried out with the use of echography of the umbilical artery, umbilical vein, aorta, and internal carotid artery of the fetus have demonstrated a high informative value of these methods for the assessment of the type of fetal condition and prediction of the newborn's adaptive mechanisms of the cardiovascular system. Significant changes of the cardiovascular system were revealed in small-for-date fetuses and in chronic fetal hypoxia, as well as functional changes in the newborns of the mothers with

A study on the pattern of alkaline phosphatase activity correlated with observations on silver-impregnated structures in the developing mouse brain.

Alkaline phosphatase activity was localised in specific groups of cellular processes, nuclei and fibre tracts in fetal mouse brain at 9.5-17.5 days post coitum. A radial band of intense enzymatic activity extended longitudinally within the rhombencephalon at 10.5 to 15.5 days. This enzymatic band was topographically related to the efferent nuclei of the cranial nerves, except those nuclei which had migrated laterally to their final positions. The enzymatic activity was seen in some but not all silver-impregnated fibre tracts that appeared early in development, but such association disappeared

Prenatal diagnosis of oculocutaneous albinism by electron microscopy of fetal skin.

Oculocutaneous albinism was diagnosed prenatally by electron microscopic examination of fetal skin samples taken during fetoscopy at 20 weeks of gestation. Melanosome development in hair bulb melanocytes progressed no further than stage II, indicating a lack of melanin synthesis. In 4 age-matched control fetuses, numerous stage IV melanosomes, signifying active melanin synthesis, were identified. The diagnosis was confirmed after the pregnancy was terminated at 22 weeks. Examination of the fetal eye showed absence of pigment in the retinal epithelium and uvea at a stage when ocular melanogenes

Antenatal diagnosis of genodermatoses.

The technics of fetoscopy and fetal skin biopsy have expanded our capabilities for antenatal diagnosis of certain hereditary skin disorders (genodermatoses). The fetus and placenta can be visualized directly and skin biopsy specimens can be obtained for light and electron microscopic studies, tissue culture, and biochemical analysis. These technics have been utilized successfully in the prenatal diagnosis or exclusion of epidermolysis bullosa, epidermolytic hyperkeratosis, and harlequin ichthyosis in fetuses known to be at risk for these disorders, and they offer tremendous potential for prena

[Pulmonary valve aplasia with ventricular septal defect. Hemodynamics and embryology].

The combination of absent pulmonary valve with ventricular septal defect, subvalvular pulmonic stenosis and aneurysm of the pulmonary artery presents a characteristic heamodynamic and clinical profile. It should be differentiated from other morphologically related cardiovascular malformations. According to the degree of right ventricular outflow tract obstruction, the circulatory pattern and the clinical picture is varying widely. We are differentiating an obstructive and a congestive form of the disease. An own case is demonstrated. After a short review of the literature the clinical and haem

The value of amniotic fluid diamine oxidase estimations in the management of severe rhesus iso-immunization.

Serial estimations of the enzyme, diamine oxidase (D.A.O.) in liquor amnii were made in 82 rhesus sensitised women. Normal levels were found in severe disease with a favourable fetal outcome, but abnormal levels were found in 50% of the pregnancies in which severe disease was associated with fetal death. In addition, when liquor bilirubin levels predicted moderate-severe disease, D.A.O. levels were abnormal in 75% of the pregnancies in which the fetus succumbed. Abnormally low D.A.O. levels in amniotic fluid were frequently associated with the subsequent development of disseminated intra-vascu

Management of single fetal death in twin-to-twin transfusion syndrome: a role for fetal blood sampling.

OBJECTIVE: Intrauterine death of one twin in monochorionic pregnancies is associated with an increased mortality and morbidity of the cotwin. This is likely to occur as a consequence of acute hemodynamic changes due to feto-fetal hemorrhage at the time of death of the cotwin. We assessed the role of fetal blood sampling and intrauterine transfusion to rescue the survivor. MATERIALS AND METHODS: We managed 12 cases of single intrauterine death at between 17 and 26 weeks' gestation in monochorionic twins complicated by twin-to-twin transfusion syndrome (TTTS). All these cases had been treated e

Placental fatty acid transfer: a key factor in fetal growth.

The functionality of the placenta may affect neonatal adiposity and fetal levels of key nutrients such as long-chain polyunsaturated fatty acids. Fetal macrosomia and its complications may occur even in adequately controlled gestational diabetic (GDM) mothers, suggesting that maternal glycemia is not the only determinant of fetal glycemic status and wellbeing. We studied in vivo the placental transfer of fatty acids (FA) labeled with stable isotopes administered to 11 control and 9 GDM pregnant women (6 treated with insulin). Subjects received orally ¹³C-palmitic, ¹³C-oleic, and ¹³C-linoleic a

Apoptosis in mouse fetuses from dams exposed to T-2 toxin at different days of gestation.

T-2 toxin (2 mg/kg b.w.) was orally inoculated to pregnant mice at gestational day (GD) 8.5, 9.5, 10.5, 11.5, 12.5, 13.5, 14.5, 15.5 and GD 16.5, respectively, and the fetuses were examined 24 hours later. The number and region of pyknotic or karyorrhectic cells varied according to inoculation date. In the GD 13.5-subgroup, a moderate to high number of pyknotic or karyorrhectic neuronal cells were observed in the central nervous system, peri-ventricular zone to subventricular zone, and pyknosis or karyorrhexis were also observed in a small number of chondroblasts and chondrocytes. In the GD 16

Regulation of left-right patterning in mice by growth/differentiation factor-1.

The transforming growth factor-beta (TGF-beta) superfamily encompasses a large group of structurally related polypeptides that are capable of regulating cell growth and differentiation in a wide range of embryonic and adult tissues. Growth/differentiation factor-1 (Gdf-1, encoded by Gdf1) is a TGF-beta family member of unknown function that was originally isolated from an early mouse embryo cDNA library and is expressed specifically in the nervous systemin late-stage embryos and adult mice. Here we show that at early stages of mouse development, Gdfl is expressed initially throughout the embry

Single-cell transcriptome analysis reveals onco-fetal reprogramming with implications for drug resistance and clinical outcome.

Onco-fetal reprogramming of the tumor ecosystem induces fetal-like characteristics, leading to tumor growth and malignant progression. Here, we develop a robust computational pipeline to systematically identify onco-fetal (OF) cells, using single-cell transcriptomic data from corresponding fetal and tumor tissues across multiple cancer types. OF cells are consistently detected across diverse malignancies and exhibit pronounced stem-like properties. Core transcription factors involved in onco-fetal reprogramming are identified, including PTMA, ATF5, TCF4, and FOS, whose coordinated activities m

[Pulsations of the umbilical vein: pathophysiologic aspects and fetal outcome].

Continuous forward flow to the fetal heart in the umbilical vein is a normal Doppler finding. Altered fetal hemodynamics can cause a pulsatile flow pattern in the umbilical vein. Pulsations in the umbilical vein were diagnosed in 14 pregnancies complicated by fetal hydrops, cardiac malformations, arrhythmia or severe intrauterine growth retardation. To document the fetal outcome, the results were analysed retrospectively. Compared with a normal Doppler group (N = 56), a significantly higher rate of perinatal death (Alpha < 1%), (64% vs 1.75%) was diagnosed, when pulsations in the umbilical vei

A review of the amino acid metabolism in placental function response to fetal loss and low birth weight in pigs.

The fertility of sows mainly depends on the embryo losses during gestation and the survival rate of the post-farrowing piglets. The selection of highly-prolific sows has been mainly focused on the selection of genotypes with high ovulatory quota. However, in the early- and post-implantation stages, the rate of embryo losses was increased with the increase of zygotes. Among the various factors, placental growth and development is the vital determinant for fetal survival, growth, and development. Despite the potential survival of fetuses with deficient placental development, their life-condition
