Acquired port-wine stain occurring in association with a congenital lesion.

Acquired port-wine stains (PWSs) are rare but well recognized, and have often been reported in association with localized trauma. Various drugs have also been implicated as aetiological factors. We report the case of a 71-year-old man with an extensive congenital PWS affecting the left side of the face, upper chest and upper back. At the age of 69 years, he began to develop similar areas on his right hand and forearm, left hand and left knee. This corresponded temporally to being started on simvastatin and metformin, both of which may promote angiogenesis.

Sirenomelia.

Sirenomelia is a rare malformation of caudal part of embryo. It is characterised by complete or partial fusion of the legs into a single lower limb. Abnormalities of the kidneys, large intestines and genitalia are common. Sirenomelia cases have only one umbilical artery and one vein. Upper body birth defects are rare and include abnormalities of heart, lungs, arms, spine and brain. Here we report a case of sirenomelia with uncommon upper body birth defects involving right forearm and hand, and the rib cage. Vascular steal phenomenon cannot explain the upper body birth defects.

Unilateral pityriasis rosea in a child.

BACKGROUND: Various atypical variants of pityriasis rosea were published in the literature. There are very few reports abour pityriasis rosea being unilateral. MAIN OBSERVATIONS: We present a female child who presented with acute onset of scaly eruptions clinically consistent with a diagnosis of pityriasis rosea, affecting only one side of the body involving waist, thigh, forearm and leg on right side. CONCLUSIONS: It is extremely unusual for pityriasis rosea to present the eruptions only on one side of the body. This case is presented here for its rarity in clinical practice.

Three-flap cover for total hand degloving.

Re-surfacing after a total hand degloving injury is one of the most difficult management problems in hand surgery. We present one such case that was managed using three flaps for cover. The radial forearm flap is very thin and pliable, resulting in satisfactory coverage of first web space and the thumb remnant, and facilitates a mobile thumb. The groin-hypogastric flap covered the remnant fingers admirably. The patient had a good first web and could use the hand to lift objects and hold a pen to write legibly within 2 months of the injury.

Tumoral calcinosis demonstrated on Tc-99m sestamibi scintigraphy.

A 61-year-old uremic male under hemodialysis underwent Tc-99m sestamibi scintigraphy due to suspicious secondary hyperparathyroidism. The images showed increased uptake in the right forearm, left shoulder, right knee and, to a lesser extent, in the left hip and the right thigh. Subsequent Tc-99m MDP bone scintigraphy and x-ray studies revealed intense extraosseous uptake and calcified masses in areas corresponding to those found in Tc-99m sestamibi scintigraphy, suggesting tumoral calcinoses. We demonstrate the rare case in which Tc-99m sestamibi was taken up by tumoral calcinoses.

McArdle disease with rhabdomyolysis induced by rosuvastatin: case report.

The rosuvastatin inducing rhabdomyolysis in McArdle disease (MD) has not been reported to date. A 35-years-old man had exercise intolerance, muscular fatigue and cramps during physical activity since infancy. He presented severe rhabdomyolysis episode with seizure and coma after use of rosuvastatin. The investigation showed increased serum creatine-kinase levels and the forearm ischemic exercise did not increase venous lactate. The muscle biopsy showed subsarcolemmal and central accumulation of glycogen and absence of the myophosphorylase enzyme. The statin induced myopathy is discussed and th…

A new syndrome with prenatally diagnosed thoracoschisis, hiatal hernia and extremities' agenesis: case report.

Thoracoschisis is a very rare congenital anomaly and is usually associated with limb and abdominal wall defects forming part of limb-body wall complex. We here present a case of a 29-week-4-day pregnancy ended with intrauterine demise. Postmortem examination revealed hiatal hernia, thoracoschisis and protrusion of some part of liver from this defect, left forearm agenesis and right arm and right hand agenesis. The findings accompanying thoracoschisis in this presented case exclude any previously reported related syndrome and we believe that this is a new syndrome.

Successful revascularisation of near total amputation of the upper limb after ten hours of warm ischaemia.

Near total amputation of the upper limb if unsalvageable would cause severe disability. However, delayed revascularisation can be life threatening. We report two cases of revascularisation of the upper limb following near total amputation that was successful and functional after a warm ischaemic time of ten hours. The first was a traction avulsion injury of the arm leaving major nerves contused but in continuity. The second was a sharp injury through the mid-forearm attached by only a bridge of skin. Attempting revascularisation of a proximal injury beyond 6 hours, in selected cases is worthwh…

Alveolar rhabdomyosarcoma in infantile spinal muscular atrophy: coincidence or predisposition?

We report two unrelated patients with infantile spinal muscular atrophy (SMA) types II and IIIa who developed alveolar rhabdomyosarcoma (ARMS) at 15 and 19 years, respectively. The tumours were located in the forearm, within severely atrophic flexor muscles. They displayed a similar histology and shared the most common translocation, t(2;13)(q35;14) in ARMS. Since cell proliferation is increased in de- and regenerating muscle and the PAX3/FKHR fusion protein activates myogenic transcription, it is tempting to speculate whether severe muscle atrophy in SMA might predispose to malignant transfor…

Concomitant coronary and subclavian steal caused by ipsilateral subclavian artery stenosis and arteriovenous fistula in a hemodialysis patient.

Patients with chronic renal failure, because of concomitant conventional cardiovascular and uremia-associated risk factors, are at risk of developing diffuse and accelerated atherosclerosis involving both the coronary and peripheral territories. We report an end-stage renal failure patient with a history of coronary artery bypass surgery who developed both angina and dizziness during hemodialysis via a left forearm arteriovenous fistula. Magnetic resonance imaging diagnosed the presence of significant subclavian artery stenosis. The patient then underwent successful percutaneous stenting of th…

Thalamic neuronal activity correlated with essential tremor.

Animal studies suggest that an olivocerebello-bulbospinal pathway mediates harmaline induced tremor, which resembles essential tremor in humans. However, recent evidence suggests that thalamocortical pathways participate in essential tremor. Thalamic single neuron activity has been analysed during thalamotomy for essential tremor. It has been shown by spectral cross correlation analysis that thalamic activity has a significant, linear relation to forearm EMG activity during tremor. The presence of this tremor related activity at the site where a lesion abolishes essential tremor suggests that …

Immunoglobulin A (IgA) deposits in lesional skin of a patient with blepharochalasis.

We describe a 21-year-old male patient with blepharochalasis, a form of localized acquired cutis laxa. He had a 13-year history of recurrent swelling attacks of the eyelids of unknown origin leading to periocular localized cutis laxa. Histology of lesional skin confirmed almost complete loss of elastic fibres in the reticular and papillary dermis. Immunofluorescence and immunoelectron microscopy studies showed abundant immunoglobulin A (IgA) deposits around the remaining elastic fibres. Control skin of the forearm was negative. These findings support the hypothesis that immunopathogenetic mech…

A case of linear type of lichen sclerosus et atrophicus?

We report a case of the linear type of lichen sclerosus et atrophicus (LSA). The patient had linear atrophic lesions from the left upper back to the left hand. Histological findings obtained from the upper back were those of typical LSA. Histology of the forearm showed mild changes suggestive of LSA. There were no histological features suggesting any association with localized scleroderma in either specimen. The relation between LSA and localized scleroderma is unclear at present. This case suggests that there is a linear form in LSA as already recognized in localized scleroderma.

[Technique and problems of total parathyroidectomy with autotransplantation in secondary hyperparathyroidism].

Between 1982 and 1990, 20 patients with chronic renal failure underwent total para-thyroidectomies and autotransplantations as treatment for secondary hyperparathyroidism. Fourteen cases were cured of their symptoms and their serum PTH levels was restored to normal. Recurrent hyperparathyroidism developed in 6 cases. In 3 of the 6 cases, excision of parathyroid tissue from the forearm could easily be performed under local anesthesia. Of the other cases, 2 had five and one had six glands. Several localizing methods should be performed before operation, in order to overlook the parathyroid gland…

Souter-Strathclyde arthroplasty of the rheumatoid elbow. 23 cases followed for 3 years.

Twenty-three elbows in 17 rheumatoid arthritis patients have undergone unconstrained Souter-Strathclyde elbow replacements since March 1984. One patient developed a deep-wound infection, and 4 others had a temporary ulnar nerve paresis. At follow-up 3 (0.5-6) years postoperatively, there was a moderate improvement in the arc of movements: 25 degrees in extension-flexion and 45 degrees in forearm rotation. Pain relief was achieved in 20 cases. Three elbows required revision, two following recurrent dislocation and the other after a humeral fracture and component loosening.

McArdle's disease with myoadenylate deaminase deficiency: observations in a combined enzyme deficiency.

Exercise and work potential of a patient with coexistent myophosphorylase and myoadenylate deaminase (AMPDA) deficiency was compared with that of three patients with myophosphorylase deficiency alone. The patient with the combined defect failed to produce an abnormal rise in serum ammonia or hypoxanthine as seen in the other patients after forearm exercise. Maximum oxygen consumption and work rates during cycle ergometer testing were similar in all patients, but well below controls. The occurrence of two defects involving short-term energy metabolism in muscle presents an opportunity to define…

Segmental Neurofibromatosis with Angiolipomas.

A 28 year old male had right axillary freckling, a few cafe-aulait macules and numerous freckles over the right upper half of his body involving the -scapular region, shoulder, chest wall, upper arm and proximal half of the forearm Numerous, bilateral subcutaneous nodules which could possibly be dismissed clinically as subcutaneous nodules of neurofibromatosis, proved to be angiolipomas histopathologically. It is suggested that in the cal molluscum fibrosum, histopathology of subcutaneous nodules should always be resorted to in suspected cases of Neurofibromatosis even when there are suggestiv…

Experimental analysis of EMG feedback in treating dystonia.

Recent clinical reports have suggested that electromyographic feedback offers promise as a palliative therapy for neuromuscular disorders. This study utilized a single-subject experimental analysis to evaluate the effects of EMG feedback training on a 20-year-old man with adult-onset idiopathic dystonia to achieve control of orofacial dysfunctions. Feedback training produced reductions in levels and variability of tension in facial and forearm muscles, as measured by EMG. Reliable blind observations of facial control indicated that feedback training enabled the patient to produce normal facial…

[Tiger man: diagnosis at a glance in F18-FDG-PET-CT for the evaluation of muscle weakness and hypercalcemia].

A 68-year-old patient presented with weakness, recurrent falls, unintentional weight loss, and confusion. Symptomatic hypercalcemia secondary to a Vitamin D intoxication was initially identified as cause. Over time, proximal myopathy, edema of the forearms, persistent hypercalcemia with elevated 1,25 Vitamin D and increased inflammatory markers developed. A PET-CT showed disseminated linear uptake in the skeletal muscles («tiger man sign»). After muscular biopsy the suspicion of muscular sarcoidosis was confirmed. High-dose steroid therapy resulted in rapid clinical improvement.
