Posterior Mini-Incision With Primary Total Hip Arthroplasty: A Nine to Ten Year Follow Up Study.

The question has been raised as to whether small incision surgery will compromise long term results of total hip arthroplasty. We report nine to ten years' outcome with posterior mini-incision. Radiographs were measured for component position, polyethylene wear, fixation, and osteolysis. Sixty-two of the original 86 patients (76 of 100 hips) were alive and available for study with 17 patients deceased (with known results) and seven (8%) lost to follow-up. The result was rated as excellent in 70 of 75 remaining hips (93%). Eighty-nine of 93 hips (96%) with known results had the original implant

Comparison of surgical outcomes and implant wear between ceramic-ceramic and ceramic-polyethylene articulations in total hip arthroplasty.

The results of a prospective multicenter trial comparing 357 hips randomized to total hip arthroplasty with either ceramic-ceramic or ceramic-polyethylene couplings are presented. No statistically significant difference in clinical outcomes scores between the ceramic-ceramic and ceramic-polyethylene groups was observed at any time interval. The mean linear rate was statistically lower (P < .001) in the ceramic-ceramic group (30.5 μm/year) when compared with the ceramic-polyethylene group (218.2 μm/year). The rates of ceramic implant fracture (2.6%) and audible component-related noise (3.1%) we

The causes of the Charcot syndrome.

The Charcot syndrome is a rare complication of neuropathy in diabetes and is characterized by an acute inflammatory episode of the foot that is associated with variable degrees of dislocation, fracture, and deformity. It has no single cause but represents the final common pathway in people who are predisposed to its development by the varying overlap of several different factors. The association of the active phase of the disease with inflammation, increasing osteopenia, and increasing calcification of the arterial walls strongly suggests, however, the involvement of the receptor activator of

Five-year results of metal-on-metal resurfacing arthroplasty in Asian patients.

Clinical results of 50 metal-on-metal resurfacing arthroplasties in 45 Japanese patients were evaluated to a minimum follow-up of 5 years. The predominant diagnosis was developmental dysplasia or dislocation of the hip (70%). One patient died of an unrelated cause and another was lost to follow-up. Two hips received revision surgery, including 1 femoral neck fracture and 1 septic loosening. In the remaining 46 hips, 1 hip showed femoral component loosening. Clinical scores of the 46 hips were satisfactory at the final follow-up. The survival rate at 5 years was 96% when failure was attributed

Treatment of periprosthetic femoral shaft nonunion.

The purpose of this study is to review the contemporary treatment of periprosthetic femoral shaft nonunions. Thirteen periprosthetic femoral shaft nonunions in 13 patients were reviewed retrospectively. The average follow-up was 48.3 months. Five patients were treated with open reduction and internal fixation, 3 patients were treated with open reduction and internal fixation and prosthesis revision, and 5 were treated with partial femoral replacing prosthesis. Functional outcomes were assessed using a modified Harris hip score. Twelve of 13 nonunions were treated successfully as defined by eit

[Imaging of the wrist and of the hand: what is the best modality?].

The authors describe the indications for radiologic explorations in case of traumatic as well as non-traumatic conditions. In case of trauma of the wrist, the radiologic exploration looks for a fracture, a dislocation or a ligament injury. Initially, postero-antérior and lateral views must be completed with an antero-posterior view, a scaphoid incidence and an oblique view. This initial examination can be completed secondarily by specific views for the carpal bones, dynamic X-Rays, CT scan or arthro CT scan. In non-traumatic cases, radiologic explorations look for osteo-articular or soft tissu

[Developmental stenosis of the cervical spinal canal hyperextension injury].

Sixteen cases of cervical spinal cord injury with developmental stenosis of cervical spinal canal were treated. The numbness and quadriplegia of the patients were caused by hyperextension X-ray of the cervical spine showed no fracture or dislocation but the sagittal diameter of the canal and that of the corresponding cervical vertebral body was less than 0.75. All of the patients were treated by operation including laminoplasty on 12 cases. The numbness and quadriplegia in most of the patients were improved obviously after operation. The mechanism of hyperextension injury on the cervical spina

Hip arthroscopy by the lateral approach.

This report describes a new technique by which arthroscopy of the hip provides complete visualization of the joint space. The arthroscope and operative instruments are inserted by a direct lateral approach over the greater trochanter, with the patient in the lateral decubitus position. The involved leg is held in an abducted and flexed position with traction by pulleys hung overhead. Hip abduction and flexion relaxes the capsule and traction separates the joint so that insertion of the arthroscope is facilitated. Arthroscopic examination of the hip joint was performed in 11 patients. The indic

Does THA Offer Clinically Important Benefits Over Hemiarthroplasty for Femoral Neck Fracture in Patients Age 65 Years or Older? A Systematic Review and Meta-analysis.

BACKGROUND: Multiple meta-analyses have compared THA with hemiarthroplasty for displaced femoral neck fractures in patients over 65 years of age; however, clinical equipoise persists. We posit that this persistent uncertainty does not reflect a paucity of evidence but rather stems from critical methodological limitations in prior syntheses-including incomplete evidence searches, inadvertent inclusion of duplicate outcome data from overlapping patient cohorts, and inappropriate pooling of RCTs with nonrandomized or retrospective studies. An updated, rigorously conducted meta-analysis of RCTs-de

[Palmar Displacement in Distal Radius Fractures following Extension Injuries of the Wrist].

BACKGROUND: Fractures of the distal radius have been traditionally classified based on the suspected mechanism of injury. Among clinicians, the terms "Colles" and "Smith" fractures are commonly used to refer to a distal radius fracture that is either displaced to the dorsal (Colles) or palmar (Smith) side of the radius. When analysing x-rays, it is not uncommon for a Smith fracture to be detected in cases where patients describe a fall on the wrist in an extended position. Thus, the question arises how a flexion-type fracture can occur after an extension injury. So far, only little research ha

Anteromedial femoral neck plate with cannulated screws for the treatment of irreducible displaced femoral neck fracture in young patients: a preliminary study.

BACKGROUNDS: Although most displaced femoral neck fractures of young patients can be repositioned successfully by closed reduction, there are still some can not repositioned successfully by this way and open reductions are required. This type of fracture was defined as irreducible displaced femoral neck fracture in this study. The purpose of this study was to introduce a new technique using anteromedial femoral neck plate with cannulated screws fixation by open reduction for the treatment of irreducible displaced femoral neck fractures in young patients. METHODS: Totally 26 patients with irre

Monteggia and equivalent lesions in childhood.

One hundred two children with acute Monteggia lesions treated over a 25 year period were reviewed. Using the Bado classification system, type 1 (53%) and type 3 (26%) fractures were the most common. The type 1 equivalent injury associated with a proximal radius fracture is more common in children than previously reported. The majority of injuries could be treated with closed reduction, except the type 1 equivalent lesions, which required operative treatment in 10 of 14 children. Varus angulation of the ulna was the most common deformity after closed treatment. Nerve injuries occurred in 11% of

Interfragmentary screw fixation of the zygomatic arch in complex midface and zygomaticomaxillary fractures.

Zygomatic arch reduction and fixation is a key point in the treatment of complex midface and zygomaticomaxillary fractures. High-impact frontal trauma can cause posterior displacement of zygomatic bone, with a sagittal fracture of the root of the zygomatic arch extending posteriorly to the glenoid fossa. Miniplate and screw fixation of this fracture requires a large detachment of soft tissue, thus being technically more difficult for proper fixation and increasing the risk of soft tissue damage. This report describes an operative approach for fixation of this type of fracture using an adaptati

Percutaneous Reduction and Provisional Pinning of the Distal Fibula: A Novel Way to Maintain Fibular Length in Temporary Stabilization.

Ankle fractures are commonly observed by orthopaedic trauma surgeons, foot and ankle surgeons, and general orthopaedists with more than 135,000 fractures surgically treated every year in the United States. 1 The soft tissue envelope surrounding the osseous anatomy of the ankle is limited and many injuries, including those that result from higher injury mechanisms and/or those occurring in hosts with vascular and metabolic comorbidities may benefit from a delay in definitive fixation; this may minimize risks of wound healing issues and fracture-related infection. 2,3 Open fractures may benefit

Outcomes of Total Hip Arthroplasty for Congenital Hip Dislocation: A Retrospective Study.

Introduction Congenital hip dislocation (CHD) represents a long-standing public health issue in Morocco, with a late diagnosis often leading to advanced joint degeneration. While periacetabular osteotomy is preferred in young adults with preserved joint space, total hip arthroplasty (THA) becomes necessary in end-stage cases. However, THA poses challenges due to anatomical deformities and the increased likelihood of revision surgeries. This study evaluates the clinical, radiological, and functional outcomes of THA performed for CHD and compares them with existing literature. Materials and meth
