One percent to 2% of pregnant women undergo nonobstetric surgery during pregnancy. Historically, there has been a reluctance to operate on pregnant women based on concerns for teratogenesis, pregnancy loss, or preterm birth. However, a careful review of published data suggests four major flaws affecting much of the available literature. Many studies contain outcomes data from past years in which diagnostic testing, surgical technique, and perioperative maternal-fetal care were so different from current experience as to make these data of limited utility today. This issue is further compounded

According to the REGARD and RAINBOW trials, ramucirumab(RAM)was introduced as second-line therapy for advanced or metastatic gastric cancer. RAM may impair wound healing due to the inhibition of angiogenesis; details on the postoperative course of patients who underwent surgery during RAM treatment remain unclear. Between 2011 and 2016, 93 patients with incurable gastric cancer were treated with RAM in our institute. Among them, 3 patients underwent surgery after RAM treatment. Case 1: A 74-year-old man with liver metastasis from gastric cancer was treated with a paclitaxel(PTX)plus RAM regime

BACKGROUND: It is difficult to determine the amount of eyelid lift in levator resection surgery for congenital ptosis, especially when the surgery is performed under general anesthesia. We aim to introduce an objective and steady method with which to adjust eyelid height, and to identify other factors to be considered during surgery.
METHODS: This is a retrospective, observational study of 110 eyes from 78 patients who underwent levator resection surgery for congenital ptosis. Postoperative eyelid height was observed chronologically. The amount of eyelid lift during surgery was compared with

PURPOSE: To address the use of and knowledge about skills assessment and competency-based training in cataract surgery in European Board of Ophthalmology (EBO) member countries.
METHODS: A survey was emailed directly to all directors of the European societies in ophthalmology. The survey included queries about criteria to undergo training in cataract surgery, regulation of education in cataract surgery, and skills assessment and training methods. In addition, all Danish Eye Departments were further asked to what extent they find assessment tools useful, if competency-based training in catarac

BACKGROUND: Over 80 % of bariatric surgical patients are women with obesity in their reproductive years. Obesity adversely affects fertility; the rapid weight loss following bariatric surgery can increase fecundity. Current guidelines recommend avoiding pregnancy for up to 24 months following surgery, but little is known about current contraceptive care of women who undergo bariatric surgery. Two surveys were undertaken with bariatric surgical and contraceptive practitioners in England to establish current contraceptive practices in both groups.
METHODS: Two anonymous on-line surveys were sen

OBJECTIVE: The ultimate goal of minimally invasive coronary artery bypass grafting (CABG) is day surgery. We evaluated the potential of a new awake CABG protocol using only epidural anesthesia in realizing day surgery.
METHODS: Seventy-two patients who underwent awake off-pump CABG (OPCAB) in our institute from March 2003 were studied. One day before surgery, an epidural catheter was placed at the Th1-2 interspace. On the day of surgery, local anesthetics were delivered via the catheter to provide a level of epidural block from C6 to Th8. Among 1260 patients who underwent OPCAB under general

The treatment of choice for non-metastatic pheochromocytoma is surgical resection. Its goals are to abolish catecholamine hypersecretion, normalize blood pressure, and prevent further tumor growth or progression to metastatic disease. Data on long-term mortality and morbidity after pheochromocytoma surgery are limited. We here report a retrospective study on the long-term outcome after surgery for apparently benign pheochromocytoma at the Radboud University Nijmegen Medical Centre. Data on clinical presentation, treatment, post-surgical blood pressure and recurrence, metastasis and death were

OBJECTIVE: To evaluate visual acuity and quality of life improvement after cataract surgery.
METHODS: Visual acuity and quality of life were assessed in patients undergoing phaco-emulsification. They were of both genders, ranged from forty-six to ninety-two years of age, and were able to understand the SF-36 quality of life questionnaire which explores the differences in quality of life before and after surgery. The questionnaire was completed before cataract surgery and again 6 and 18 months later. Visual acuity was measured with Snellen optotype.
RESULTS: The answers of 150 patients were e

BACKGROUND: Cataract surgery is the most frequently performed ophthalmic procedure worldwide. While benefits gained from cataract surgery outweigh surgical risks, there have been concerns that older persons may have an increased risk of developing age-related macular degeneration (AMD) after cataract surgery.
OBJECTIVE: The Australian Prospective Study of Cataract Surgery and Age-Related Macular Degeneration Study aims to assess the risk of AMD in a large cohort of older patients following cataract surgery. The current report describes the study rationale, design and methodology.

Wound infection is a frequent complication and is related to various parameters: type of surgery, patient's age, nutritional status, associated diseases, length of surgery and hospital stay, use of prosthesis and drainage and finally surgeon's ability. The frequency of wound infection is reported between 1.5%-5.1% after "clean surgery" and the greatest source of microbial contamination is due to GRAM positive cocci either aerobic or anaerobic. The Authors present their experience of ultra short-term prophylaxis with Teicoplanin in 375 patients undergoing major ambulatory surgery. Median age wa

Treatment decisions about lower third molar surgery have important clinical and cost implications. Although indications for surgery have been the subject of a National Institutes of Health (NIH) consensus conference at which several unambiguous criteria were agreed, no prospective investigations have been carried out to compare clinic treatment decisions with these consensus criteria. Treatment decisions made by six Hospital Service oral surgeons were therefore evaluated for 72 consecutive patients (28 men, 44 women) aged 15-44 years (mean age 25 years), referred for lower third molar assessme

Analysis of present status: The institutes for cardiac surgery, amounting to 300 are spread throughout Japan. Consequently, the number of operation for the acquired heart disease, congenital heart disease and thoracic aortic aneurysm, has been counted 8088, 7837, and 839 respectively and in total, 16774 cases were operated in 1984. The increase in number of surgery for new born baby and infant recently proves the remarkable advance of cardiac surgery in Japan. The gradual increase in number of surgery for the aged reflects the Japanese social conditions today. The increase in number of aorto c

Abdominal wall blocks are simple and safe opioid-sparing adjuncts for abdominal procedures that are generally well tolerated. These blocks have been shown to be effective for open abdominal surgeries, though their utility in laparoscopic or robotic surgery has been questioned. The unanticipated conversion from laparoscopic to open surgery may pose a challenge for the anesthesiologist, if regional anesthesia was not discussed before the start of the case as part of the pain-control regimen. Inability to offer an abdominal wall block leads to increased use of opioid medications and less-than-ide

Anesthesia in patients with emphysematous giant bulla undergoing non-thoracic surgery is challenging and can cause serious complications. We report a successful case of lip mass resection in a 65-year-old male with paraseptal emphysema and giant bullae under regional anesthesia using a mental nerve block. The patient presented with a slow-growing ulcerative mass on his lower lip and had a history of non-compliant COPD management. An excisional biopsy was planned. Preoperative workup revealed extensive lung pathology with giant bullae. General anesthesia with positive pressure ventilation in pa

INTRODUCTION: Down syndrome is the most common chromosomal abnormality associated with intellectual impairments. Unexpected deaths are common with this disease. There are certain difficulties in clarifying the cause of death because the manifestations may be quite diverse and involve many organ systems. Atlantoaxial subluxation is a dangerous complication of Down syndrome, as it may lead to cervical cord-medullary compression.
CASE PRESENTATION: Herein, we present a case of Down syndrome in a patient who completely recovered from cardiac arrest due to atlantoaxial subluxation. The neck was im

Coronary ostium obstruction due to dislodgement of the prosthetic valve is a rare and life-threatening complication, and particular caution is required for sutureless aortic valve replacement (AVR) with concomitant valvular surgery. In general, coronary artery bypass surgery is performed when coronary ostium obstruction occurs after AVR, but other options may need to be considered in some cases. Herein, we present a case of coronary artery occlusion in an 82-year-old female patient who had undergone AVR and mitral valve replacement for aortic valve stenosis and mitral valve stenosis at the age
