Primary Hyperparathyroidism With Brown Tumor-A Case Mistaken as Non-Ossifying Fibroma Resolved by Biochemical and Radiological Correlation.

Primary hyperparathyroidism (PHPT) is uncommon in the pediatric population and rarely presents with overt skeletal complications such as brown tumors. This case study describes a 16-year-old female who initially presented with abdominal pain and was subsequently found to have severe hypercalcemia (serum calcium 16.4 mg/dL) and markedly elevated parathyroid hormone (PTH 1700 pg/mL) and bilateral nephrolithiasis. There were no features suggestive of multiple endocrine neoplasias. A history of a previously diagnosed "non-ossifying fibroma" in the proximal tibia was later reinterpreted as a brown …

Pheochromocytoma and primary hyperparathyroidism: a very rare association in a neurofibromatosis type 1 patient unmasked by Takotsubo syndrome.

SUMMARY: Neurofibromatosis type 1 is an autosomal dominant disease characterized by cutaneous, bone, and neurocognitive manifestations and an increased risk of neoplasms - more frequently cutaneous neurofibromas and optic gliomas and more rarely pheochromocytomas/paragangliomas, solitary parathyroid adenomas with primary hyperparathyroidism, and breast carcinomas. We present a case of a 48-year-old woman with an unexpected presentation of Takotsubo syndrome that culminated in a synchronous diagnosis of pheochromocytoma, primary hyperparathyroidism, and invasive breast carcinoma (pN2(R0)N1) in …

Efficacy and Safety of Denosumab versus Zoledronic Acid in the Treatment of Hypercalcemia in Primary Hyperparathyroidism: A Retrospective Comparative Study.

OBJECTIVE: Nephrotoxicity of bisphosphonates limits their utility in the management of severe hypercalcemia among patients with primary hyperparathyroidism (PHPT) and impaired renal function. It remains unclear whether denosumab is an effective and safe therapeutic option for hypercalcemia in patients with PHPT. We aimed to compare the efficacy and safety of denosumab and zoledronic acid (ZA) in treating hypercalcemia in patients with PHPT. METHODS: This single-center retrospective comparative study included ninety patients with PHPT and hypercalcemia. Forty-five were treated with ZA (4 mg in…

Ectopic parathyroid adenoma posterior to the trachea in the superior mediastinum causing refractory primary hyperparathyroidism: a case report.

Primary hyperparathyroidism (PHPT) caused by ectopic parathyroid adenomas in the superior mediastinum is rare and often leads to surgical failure. We report a case of severe PHPT in a 50-year-old female presenting with lower limb weakness and bone pain. Laboratory tests revealed marked hypercalcemia (serum calcium 3.48 mmol/L) and extremely elevated parathyroid hormone (PTH) levels (1920 pg/mL). Preoperative imaging, including neck ultrasound and 99mTc-MIBI SPECT/CT, precisely localized the ectopic adenoma to the superior mediastinum at the T1 vertebral level, posterior to the trachea, rather …

The Prevalence and Risk Factors for Persistent Hyperparathyroidism in Post-Kidney Transplant Patients: A Single-Center Retrospective Study.

BACKGROUND: Persistent hyperparathyroidism (PHPT) remains a notable challenge among kidney transplant recipients due to its impact on calcium-phosphorus metabolism, potentially hindering recovery and long-term renal function. Understanding its prevalence and risk factors is vital for enhancing patient outcomes. OBJECTIVE: To determine the prevalence and identify risk factors for persistent hyperparathyroidism in adult Filipino kidney transplant recipients at a tertiary care hospital. METHODOLOGY: A retrospective chart review was conducted for 80 kidney transplant recipients at Cardinal Santo…

Risk factors for renal stone development in adults with primary hyperparathyroidism: a protocol for a systematic review and meta-analysis.

INTRODUCTION: Primary hyperparathyroidism (PHPT) is characterised by overactive parathyroid glands and consequent calcium dysregulation. Renal stones (RS) are a recognised complication and are associated with substantial morbidity. However, RS risk factors in PHPT are not well established, and recent international guidelines identify 'renal stone risk' as a research priority. We aim to summarise and meta-analyse evidence on prespecified risk factors for RS in adults with PHPT. METHODS AND ANALYSIS: This Preferred Reporting Items for Systematic Review and Meta-Analysis Protocols-aligned protoc…

Primary Hyperparathyroidism Due to Parathyroid Adenoma in a Post-Kidney Transplant Patient With Lupus Nephritis: A Case Report.

Hyperparathyroidism is common in patients with existing chronic kidney disease (CKD), typically manifesting as secondary or tertiary hyperparathyroidism. We report a 47-year-old African American woman who initially presented with CKD secondary to lupus nephritis, as well as hyperparathyroidism, thought to be secondary to worsening kidney function. Following a kidney transplant, parathyroid hormone (PTH) was persistently elevated, at levels ranging from 370.4 to 614.9 pg/mL (reference range: 10-65 pg/mL) over the course of 12 months. Uptake on nuclear imaging demonstrated retained radionuclide …

The Utility of Ultrasound-Guided Microwave and Radiofrequency Ablation in Primary and Secondary Hyperparathyroidism: A Systematic Review and Meta-Analysis.

OBJECTIVE: Hyperparathyroidism is classified into primary (PHPT), secondary (SHPT) and tertiary types. While parathyroidectomy (PTX) is standard treatment, minimally invasive thermal ablation (TA) methods are increasingly being explored, especially in high-risk surgical patients. The objective of this study is to review the evidence on TA techniques, microwave (MWA) and radiofrequency (RFA), and compare them to surgical techniques in hyperparathyroidism. METHODS: A systematic search was conducted through Medline via six major databases up to February 2025. Meta-analysis was performed using R …

Spontaneous Quadriceps Tendon Rupture Associated With Severe Secondary Hyperparathyroidism in a Patient on Chronic Hemodialysis: A Case Report.

Spontaneous or minimal-trauma ruptures of the quadriceps tendon are an exceptional clinical entity, representing a highly unusual presentation even among patients with underlying metabolic conditions, including uremic patients on chronic hemodialysis. We report the case of a 40-year-old male patient on chronic hemodialysis with severe secondary hyperparathyroidism, who was admitted for a rupture of the left quadriceps tendon following minimal trauma. Treatment consisted of the surgical reconstruction of the extensor apparatus and management of his hyperparathyroidism.

Development of a nomogram for predicting severe post-parathyroidectomy hypocalcemia in patients with secondary hyperparathyroidism: a retrospective cohort study.

BACKGROUND: Severe hypocalcemia affects 25-50% of patients with secondary hyperparathyroidism (sHPT) after total parathyroidectomy. It results from acute postoperative hypoparathyroidism and rapid skeletal mineral uptake. Severe hypocalcaemia leads to significant morbidity, prolonged hospital stays, and increased healthcare costs. Few risk assessment prediction tools exist; they lack external validation and exhibit methodological shortcomings. To enable evidence-based risk stratification and personalised perioperative management, we developed and internally validated a nomogram for predicting …

The Delicate Balancing of Pros and Cons in the Surgical Management of Hyperparathyroidism in a Young Female with Germline Variant in the CDC73 Gene.

Hyperparathyroidism-jaw tumor syndrome is a rare form of syndromic primary hyperparathyroidism. We describe a young female with a history of common precursor B acute lymphoblastic leukaemia who was diagnosed with overt primary hyperparathyroidism due to a pathogenic CDC73 variant (c.25C > T). This patient posed several challenging management aspects: the development of nephrocalcinosis, the risk for parathyroid carcinoma, and persistent hyperparathyroidism after two selective parathyroidectomies, leading to the decision to perform a total parathyroidectomy. The latter resulted in permanent com…

Hyperparathyroidism-Jaw Tumor Syndrome Associated to a CDC73 Gene Pathogenic VARIANT and a Nonossifying Desmoplastic Fibroma of the Mandible.

Most cases of primary hyperparathyroidism (PHPT) are sporadic and are caused by parathyroid adenomas. Hereditary forms may occur in up to 10% of PHPT patients and are more frequent in younger patients. The hyperparathyroidism-jaw tumor (HPT-JT) syndrome is characterized by PHPT in up to 95% of patients and ossifying fibromas in the jaw in 25%-50%. We describe the case of a 35-year-old male from Bangladesh referred to our hospital due to a voluminous right mandibular swelling: a rare nonossifying fibroma of the mandible was diagnosed. Due to functional impotence, a left shoulder magnetic resona…

Autosomal dominant primary hyperparathyroidism in the Keeshond dog breed is strongly associated with a missense variant in sirtuin-6.

Primary hyperparathyroidism (PHPT) is an inherited disorder that leads to inappropriate secretion of parathyroid hormone by neoplastic parathyroid cells. Dogs of the Keeshond breed are predisposed to adenomas or hyperplasia of the parathyroid that lead to PHPT. The disorder is inherited in a dominant Mendelian fashion with a high age-related penetrance. The age of onset in PHPT-affected Keeshonden is typically later than 8 years. Genome-wide association studies with 27 affected and 42 unaffected Keeshonden genotyped with 30 896 markers identified a region of strong association with the PHPT ph…
