Cutting seton for complex anal fistulas.

PURPOSE: A standardised protocol for performing a cutting seton has not been well described in the existing literature. The aim of this study was to examine our experience of treatment of complex anal fistulas by cutting seton over 15 years in our hospital, detailing surgical technique, results and complications. METHODS: Between 1990 and 2004, 112 patients with complex anal fistulas were treated by applying cutting setons in our hospital. The elastic band from a surgical glove was used as the seton material. The seton was re-tightened for the first time in the second week after the initial o

Lucja Frey--historical relevance and syndrome review.

BACKGROUND: This article documents an objective review ofthe neuro-anatomical, diagnostic and clinical implications of the auriculotemporal syndrome (Frey's syndrome). The incidence of Frey's syndrome after parotidectomy as cited in the literature varies. It may also be a sequela to a variety of inflammatory, infective and traumatic aetiologies. METHOD: An electronic search using the search engine Google, Medline and Pubmed was performed under 'Lucja Frey', 'Gustatory sweating', 'The auriculotemporal syndrome', 'Botulinum toxin'. Relevant papers were systematically reviewed from 1965 to prese

Intra-gastric balloon therapy and weight reduction.

BACKGROUND: Intra-gastric (TBT) can achieve pre-operative weight loss in obese patients, and may make surgery safer and easier. We report our experience in weight reduction and outcome in morbidly obese patients. METHODS: Data were prospectively collected in 50 consecutive morbidly obese patients undergoing intra-gastric balloon therapy. All balloons were inserted and removed by one consultant operator and injected with 500-600 mls of methylene blue-stained saline. Patients were admitted after balloon insertion to control nausea and reflux symptoms. Analysis was performed on post-insertion da

Epidermal growth factor therapy and wound healing--past, present and future perspectives.

The role ofepidermal growth factor (EGF) has been extensively investigated in normal and pathological wound healing. It is implicated in keratinocyte migration, fibroblast function and the formation of granulation tissue. Since the discovery of EGF, the first growth factor to be isolated, over 45 years ago, growth factor therapy has progressed into clinical practice in the treatment ofwounds. The investigation EGF in wound healing has progressed from the treatment of acute wounds, to its limited effect in chronic wounds. EGF is readily degraded in the chronic wound environment, but with the re

Intra-operative culture swabs in acute appendicitis: a waste of resources.

INTRODUCTION: Antibiotics gained a place in the management of acute appendicitis when the bacterial aetiology was demonstrated. Culture swabs were obtained routinely during appendicectomies to guide antibiotic use. Although current antimicrobial therapy use has become prophylactic, empirical and broad spectrum, this age-old practice still remains. Our study questions the value of this traditional practice. MATERIALS AND METHODS: All adult and paediatric patients undergoing emergency appendicectomy over three years were retrospectively reviewed. Microbiology and appendix histology reports were

A rare case of follicular thyroid carcinoma in ovarian struma.

Struma ovarii is a form of specialised mature teratoma, with predominantly mature thyroid tissue in an ovarian teratoma as seen in 2% of cases. Its malignant transformation is even rarer and is seen in only 5% of those cases. This 40-year-old female patient had an incidental finding of a pelvic mass during investigation of secondary amenorrhoea. She underwent a staging laparotomy and pelvic clearance. The histopathology revealed a bilateral mature teratoma of the ovary with follicular thyroid carcinoma in the right ovarian struma (malignant struma). A total thyroidectomy was performed followed

Are routine microbiological investigations indicated in the management of non-perianal cutaneous abscesses?

BACKGROUND: It is common practice to take a specimen of pus for microscopy and bacterial culture during drainage of abscesses. The aim of this study was to determine if routine culture and sensitivity had any therapeutic value in the care of patients with non-perianal cutaneous abscesses. PATIENTS AND METHODS: A retrospective analysis ofall patients undergoing drainage ofa cutaneous abscess during a two year period (June 2003 - June 2005) was performed. Patients were identified from the hospital database and theatre records, and those with perianal, pilonidal or surgical wound sepsis were exc

Transhiatal oesophagectomy: A simple technique to carry out gastric or colonic conduit pull-up.

Following transhiatal oesophagectomy, delivery of the conduit into the posterior mediastinum and neck can potentially result in its devascularisation. A simple technique is described to protect the conduit during this phase of the operation. This procedure has now been performed in 56 consecutive cases (54 gastric and two colonic conduits). In one case (1.8%) the colonic conduit had to be removed due to venous engorgement. The anastomotic leak rate was 8/56 (14%), and 12 (21%) patients required oesophageal dilatation for a stricture. There were no cases of ischaemia of the conduit. This techni

Superglue in otology.

BACKGROUND: The use of stitches and staples for the closure of surgical wounds is associated with complications for both the surgeon and the patient. Histoacryl (butyl1-2cyanoacrylate glue) is widely used for wound closure in Accident and Emergency departments, in particular for facial and scalp wounds. METHOD: We have used Histoacryl for closure of various surgical incisions in a series of 50 cases and assessed the cosmetic outcome at three to four weeks. RESULTS: There were no wound related complications in the form of infection or dehiscence. In one case however, there was formation of sy

Recurrent haemarthrosis of knee joint due to arterio-venous malformation of the superficial femoral artery.

Recurrent haemarthrosis involving the knee joint is commonly due to bleeding, a coagulation disorder or previous surgery. We have recently encountered a case of recurrent haemarthrosis involving the left knee joint of a 72 year old man due to arterio-venous malformation of the superficial femoral artery. Vascular malformations in patients with recurrent haemarthrosis without previous history of operation and a normal bleeding and clotting screen, should be considered a possible cause.

Recent consensus on the classification of rhinosinusitis--a way forward for research and practice?

Prior classification of rhinitis was into three categories: acute, subacute and chronic rhinosinusitis. The advantages of this classification were obvious but they were offset by some disadvantages. For example, the previous classification did not take account of the mechanisms underlying the condition or the clinical outcome. Hence, there was a need for evidence-based sinusitis classification guidelines. The American Academy of Otolaryngology-Head and Neck Surgery and other related societies published a classification of the condition and suggested clinical research strategies for patients wi

Wegener's granulomatosis: a rare, chronic and multisystem disease.

Wegener's granulomatosis (WG) is an autoimmune disease which has a clinical predilection for the upper airways, lungs and kidneys. It is a necrotising granulomatous vasculitis which is associated with a distinct autoantibody--the antineutrophil cytoplasmic antibody (ANCA). A heightened index of suspicion by clinicians is needed in the diagnosis of this complex and rare condition. A multidisciplinary approach should then be used to treat this chronic multisystem disease. Treatment involves the use of various regimens of corticosteroids and immunosuppressive medication. Mortality due to WG has b

Inadvertent parathyroid excision during thyroid surgery.

BACKGROUND: Thyroidectomy is a commonly performed operation for the treatment of thyroid disease. Inadvertent removal of parathyroid glands is a recognised complication of this operation and may have consequences on the longterm regulation of calcium homeostasis post-operatively. We aimed to establish the incidence of parathyroid gland excision during surgery for thyroid disease and whether there was an effect on calcium control in patients in whom inadvertent parathyroidectomy had occurred. METHODS: A retrospective review of thyroid operations at our hospital from 2001 to 2004 was carried ou

Treatment of sepsis: current status of clinical immunotherapy.

While antibiotics address the root cause of sepsis--that of pathogen infection--they fail to provide an adequate cure for the condition. Currently, 30% to 50% of septic patients die, and this figure is likely to increase in line with the proliferation of multi-drug resistant bacteria. With an increased understanding of the immune response, it has been proposed that modulation of this defence mechanism offers the best hope of cure. Many entry-points in the immune system have been identified and targeted therapies have been developed, but why are these not in routine clinical practice? This revie

Histological examination of circular stapled 'doughnuts': questionable routine practice?

BACKGROUND: Left-sided colonic resections are often anastomosed by the use of the circular stapling gun. Most surgeons routinely submit the resulting set of 'doughnuts' for histological examination. AIM: The aim of this study is to question the need for this practice by providing our own experience of the impact of 'doughnut' submission on patient management. PATIENTS AND METHODS: Patients who had undergone a stapled anastomosis for colorectal cancer resection (1998-2004) were identified from the department cancer database and clinical records and histopathological reports were reviewed for
