Insulin resistance and clinical aspects of non-alcoholic steatohepatitis (NASH).

Non-alcoholic steatohepatitis (NASH) is one of the most common liver disorders. This is highly prevalent in obese and diabetic subjects. Persons with central obesity are at particular risk. Other clinical predictors are age more than 40-50 years and hyperlipidemias, but none of these factors is invariable for causation of NASH. Other reported associations are, celiac disease, Wilson's Disease and few other metabolic diseases. Drugs, particularly amiodarone, tamoxifen, nucleoside analogues and methotrxate have also been linked to NASH. The disease is evenly distributed in both sexes but advance

Impact of eccentric exercise on walking functional capacity after severe COVID-19: randomized, controlled open-label trial.

INTRODUCTION: Severe COVID-19 infection leads to profound pulmonary, cardiovascular, and neuromuscular impairments, resulting in marked exercise deconditioning and reduced functional walking capacity after hospital discharge. Eccentric cycling offers high mechanical load with low metabolic cost and may therefore represent a relevant rehabilitation strategy to restore functional capacity in people after COVID-19. AIM: To compare functional recovery after 8 weeks of eccentric (ECC) or concentric (CON) training in individuals discharged 1 month previously after a COVID-19 infection. METHODS AND

Massage Therapy for a Voice Student with Behavioral Dysphonia When Singing: A Case Report.

INTRODUCTION: Behavioral dysphonia is a voice condition in which inefficient muscle tension and misuse interrupt the ability of the body to produce the desired vocal function and quality. Treatment methods typically incorporate traditional voice therapy with manual laryngeal manipulation. A lack of treatment approaches for dysphonia exists that address the vocal system including all its subsystems (i.e., breath, phonation, resonance, articulation) and posture. A limited number of reports exist describing the effect of massage therapy to address the vocal system anatomy of a singer with behavio

Functional testosterone deficiency in aging men: Clinical impact, diagnostic pathways, and treatment strategies.

BACKGROUND: Testosterone constitutes an indispensable determinant of male corporeal integrity, psychological resilience, and overall vitality across the life course. Testosterone deficiency (male hypogonadism) represents an endocrine disorder capable of engendering a broad spectrum of somatic derangements and psychosocial sequelae. Its origins may lie in testicular insufficiency, hypothalamic-pituitary dysfunction, or, more subtly, in functional hypogonadism arising from comorbid states such as obesity and type 2 diabetes mellitus. METHODS: This review distills contemporary evidence on the pa

Effects of live-remote exercise on quality of life and other health-related outcomes in cancer survivors: a randomised controlled trial.

PURPOSE: Exercise following cancer treatment has been shown to improve health-related quality of life (HRQoL) and other important health outcomes, yet barriers to participation prevent many cancer survivors from exercising according to recommendations. Live-remote exercise methods could improve accessibility; however, evidence on their efficacy is limited. The EX-MED Cancer Sweden randomised controlled trial compared the effects of a 12-week live-remote online exercise intervention to usual care in adult cancer survivors. METHODS: Two hundred adults who had completed curative treatment for br

Non-commonly prescribed physical exercises, disciplines and sports in breast cancer survivors: a narrative review.

BACKGROUND AND OBJECTIVE: Physical activity is widely acknowledged as a cornerstone in enhancing the quality of life and recovery outcomes in breast cancer survivors (BCSs). Despite robust guidelines emphasizing a multimodal approach combining aerobic, resistance, flexibility, and balance exercises, adherence to exercise prescriptions remains suboptimal. This narrative review aims to explore the effects of non-commonly prescribed physical exercises, disciplines, and sports to broaden options and improve adherence among BCS. METHODS: A comprehensive literature search was conducted in databases

VO2 on-kinetics and aerobic exercise training in females with systemic lupus erythematosus.

Fatigue among females with systemic lupus erythematosus (SLE) is commonly reported as the most debilitating symptom. Slower oxygen uptake (VO2) on-kinetics have been observed among females with SLE, when compared to healthy control individuals. Aerobic exercise training may improve VO2 on-kinetics, however, this has not been examined in the SLE population.ObjectiveTo characterize VO2 on-kinetics in SLE and examine changes after 12 weeks of aerobic exercise training (AET).MethodsFemales (n = 16; 42 ± 10 years) with SLE (SELENA-SLEDAI ≤ 4) completed a cardiopulmonary exercise test (CPET) and a c

When Fatigue Hides A Metabolic Myopathy: A Case Report of Mcardle Disease with Molecular Diagnosis.

UNLABELLED: McArdle disease, or glycogen storage disease type V, is a rare inherited metabolic myopathy caused by mutations in the PYGM gene, resulting in deficiency of the enzyme myophosphorylase. Patients usually present with lifelong exercise-induced fatigue, muscle pain, and sometimes rhabdomyolysis, but the condition is underdiagnosed due to its nonspecific manifestations. This case is distinctive because it documents molecularly confirmed McArdle disease in a woman with persistently elevated creatine kinase and a suggestive family history, underlining the importance of clinical suspicion

Remarkable response to pembrolizumab in PD-L1 overexpressing (≥ 50%) NSCLC and extracranial abscopal effect induced by brain radiotherapy: a case report.

BACKGROUND: Lung cancer remains the most prevalent malignant neoplasm worldwide, with adenocarcinoma being among its most frequent subtypes. The brain is a common metastatic site in patients with lung adenocarcinoma, often associated with poor prognosis, and brain radiotherapy is the standard recommended treatment. However, the occurrence of an extracranial abscopal effect following brain-directed radiotherapy is rare due to the brain's unique immune microenvironment. CASE DESCRIPTION: We present the case of a 64-year-old Asian male who was admitted with complaints of "right-sided hemiplegia,

Differences in checkpoint-inhibitor-induced hypophysitis: mono- versus combination therapy induced hypophysitis.

OBJECTIVE: Immune checkpoint inhibitors (ICIs) are revolutionary in oncology but may cause immune-related (IR) side effects, such as hypophysitis. Treatment with anti-PD-(L)1, anti-CTLA-4 or anti-CLTA-4/PD-1 may induce hypophysitis, but little is known about the differences in clinical presentation or need for different treatment. We analyzed the differences of anti-PD-(L)1, anti-CTLA-4 and anti-CTLA-4/PD-1 induced hypophysitis. METHODS: retrospective analysis of 67 patients (27 anti-PD-(L)1, 6 anti-CLTA-4 and 34 anti-CTLA-4/PD-1 induced hypophysitis). RESULTS: The median time between starti
