An Interferon-Response Transcriptomic Signature of Lymphovascular Invasion in Prostate Cancer.

Lymphovascular invasion is an adverse pathologic feature in prostate cancer, but its independent molecular drivers remain unclear due to strong confounding by tumor grade and stage. We performed a confounder-adjusted transcriptomic analysis of 403 TCGA-PRAD samples. Differential expression was adjusted for Gleason score and pathological T stage. A transcriptional profile associated with LVI was derived and tested in multivariable logistic and Cox proportional hazards models for biochemical recurrence-free survival, with bootstrap internal validation. After multivariable adjustment, 129 genes w

Epidemiology and Risk Factors for Non-Muscle-Invasive Bladder Cancer: Insights into Prevalence, Risk Stratification, and Implications for Early Diagnosis.

Bladder cancer is the 6thth most common type of cancer in the United States and tobacco smoke remains the leading environmental risk factor of the disease. Non-muscle invasive bladder cancer (NMIBC) makes up the majority of bladder cancer cases and is a subtype of bladder cancer that has a high heterogenicity in rates of recurrence and prognosis. The aim of this article is to dive into the prevalence of NMIBC, review the literature and guidelines on how NMIBC is risk stratified, and discuss the utility of early diagnosis of this disease and ways this can be achieved.

Modeling osteosarcoma invasion with fibroblast and tumor spheroids in functionalized hydrogels beads.

Osteosarcoma (OS) is a highly aggressive bone tumor with a strong propensity for metastasis. To develop a physiologically relevant in vitro platform for tumor invasion studies and drug testing, we engineered a three-dimensional OS model that mimics the tumor microenvironment using hydrogel-based multicellular spheroid beads. Tumor spheroids (∼125 μm) were generated and co-cultured with WS1 fibroblasts in VitroGel® RGD, a hydrogel matrix that promotes cell adhesion via integrin binding. The hydrogel beads, approximately 2-3 mm in size, supported robust growth of both MG63 and highly metastatic

Lack of oncological significance of prophylactic splenectomy for type 4 gastric cancer invading the greater curvature: a multicenter retrospective study.

The necessity of prophylactic splenectomy for 4 gastric cancer with greater curvature invasion remains unclear, as this subtype was excluded from the pivotal JCOG0110 trial. This study aimed to evaluate the short-term and long-term outcomes of splenectomy specifically in this population. We analyzed patients with clinically diagnosed advanced (cT2 or deeper) type 4 gastric cancer invading the greater curvature who underwent radical total gastrectomy at 11 institutes between 2005 and 2015. To minimize selection bias, inverse probability of treatment weighting (IPTW) was performed. Patients with

Surrogacy of Intermediate Clinical Endpoints for Overall Survival in Patients With Localized Muscle-Invasive Bladder Cancer.

BACKGROUND: Although intermediate clinical endpoints (ICEs) may expedite completion of randomized controlled trials (RCTs) evaluating perioperative systemic treatments for localized muscle-invasive bladder cancer (MIBC), no validated surrogate for overall survival (OS) has been established. We aimed to assess the surrogacy of pathologic complete response (pCR), pathologic objective response (pOR), and disease-free survival (DFS) for OS. PATIENTS AND METHODS: We analyzed 4,828 patients with MIBC (cT2-T4N0M0) who underwent radical cystectomy (RC) with or without neoadjuvant chemotherapy (NAC) a

Invasion dynamics of multifocal breast tumor spheroids in three-dimensional extracellular matrix.

Breast cancer in human patients often occurs as multifocal lesions, and the mechanisms through which primary tumors in proximity interact with each other is not well understood. Here, we study an in vitro model of bifocal breast tumors by embedding tumor spheroids of MDA-MB-231 cells in 3D extracellular matrices (ECMs) made of type I collagen at different concentrations. Combining quantitative experiment, mathematical modeling, and numerical simulation, we show that collective traction force from the spheroids creates dipolar deformation in the ECM. As a result, in the inter-spheroid region, w

Thymic carcinoma presenting with cardiac tamponade and superior vena cava invasion.

We present the case of a woman in her 40s who presented with progressive shortness of breath, facial flushing, tachycardia that worsened when supine and bilateral leg swelling. Initial evaluation included a chest X-ray, which revealed widened mediastinum and a transthoracic echocardiogram with evidence of cardiac tamponade. A pericardiocentesis was performed, draining 900 mL of haemorrhagic fluid. Subsequent CT imaging identified a large anterior mediastinal mass invading into the superior vena cava (SVC) and extending towards the right atrium. A needle biopsy confirmed the diagnosis of thymic

Metabolic buffering restricts phenotype switching in melanoma.

The impact of the metabolic microenvironment on epigenetically plastic cancer cells underpins phenotypic heterogeneity, a major cause of metastasis and therapy resistance. Nutrient limitation is a key microenvironmental stress, and can cause cells to transition from proliferative to invasive phenotypes, however, whether cancer cells have the capacity to delay phenotype switching remains unknown. Here, using melanoma as a model, we reveal that the ability to buffer glucose availability by accumulating and mobilizing glycogen can determine cancer cell phenotypic transitions. While proliferating

En bloc upper and lower lobe bisegmentectomy for non-small-cell lung cancer invading the fissure.

BACKGROUND: The optimal surgical procedure for tumors extending deep into the lung parenchyma of the opposite lobe beyond the fissure remains controversial. The current study aimed to examine whether en bloc upper and lower lobe bisegmentectomy can be an option for such tumors. METHODS: This study included patients who underwent surgery for cN0 non-small-cell lung cancer invading the adjacent lobe beyond the fissure between the upper and lower lobes. The perioperative and long-term outcomes of the bisegmentectomy and extended lobectomy/pneumonectomy groups were compared. RESULTS: The bisegme

Structural and Mechanical Characterization of Collagen-Hyaluronan Hydrogels Used to Study Cancer Cell Invasion through the Bladder Wall.

Collagen-hyaluronic acid (Col-HA) hydrogels are widely studied as biomimetic materials that recapitulate the environmental physical and mechanical properties crucial for understanding the cell behavior during cancer invasion and progression. Our research focused on Col-HA hydrogels as an environment to study the invasion of bladder cancer cells through the bladder wall. The bladder is a heterogeneous structure composed of three main layers: urothelium (the softest), lamina propria (the stiffest), and the muscle outer layer, with elastic properties lying between the two. Thus, the bladder cance

Percutaneous acetabuloplasty for the treatment of acetabular metastasis from sublingual gland adenoid cystic carcinoma: a case report.

BACKGROUND: Adenoid cystic carcinoma of sublingual gland is a rare malignant tumor. When adenoid cystic carcinoma metastasizes to the acetabulum, it is highly challenging to choose a minimally invasive surgery. The treatment of acetabular metastatic tumors needs to not only maintain the original tumor treatment plan but also relieve pain and reconstruct the acetabular structure and mechanical stability. Therefore, the treatment of acetabular metastasis from sublingual gland adenoid cystic carcinoma is a complex issue. CASE PRESENTATION: We treated a 72-year-old Han Chinese woman with acetabul

Age related trends in the utilization of neoadjuvant chemotherapy for muscle invasive bladder cancer.

PURPOSE: The current standard of care for muscle invasive bladder cancer (MIBC) is neoadjuvant chemotherapy (NAC) followed by radical cystectomy (RC). Previous research has shown under-utilization of NAC for treatment of MIBC, especially among the elderly. Our aim was to stratify NAC use by decade of life and analyze trends in use over time along with recording pathologic downstaging and perioperative outcomes. MATERIALS AND METHODS: The National Cancer Database was queried for patients with cT2-4NanyM0 MIBC treated with RC from 2010 to 2016 with urothelial carcinoma. Nineteen thousand nine h

Digital spatial profiling of the microenvironment of muscle invasive bladder cancer.

Muscle invasive bladder cancer (MIBC) is a molecularly diverse disease with varied clinical outcomes. Molecular studies typically employ bulk sequencing analysis, giving a transcriptomic snapshot of a section of the tumour. However, tumour tissues are not homogeneous, but are composed of distinct compartments such as the tumour and stroma. To investigate the molecular profiles of bladder cancer, whilst also maintaining the spatial complexity of the tumours, we employed whole transcriptome Digital Spatial Profiling (DSP). With this method we generated a dataset of transcriptomic profiles of tum
