Type four fracture of the medial epicondyle: a true indication for surgical intervention.

Fourteen consecutive cases with type 4 fracture of the medial epicondyle were evaluated following open reduction and internal fixation of the displaced medial epicondyle. The mean age was 9.7 years (range 6-16) and the mean follow-up was 17.2 months (range 12-24). Operative treatment yielded excellent results with no loss of functional range of motion, residual deformity or instability. There were three cases with pre-operative symptoms of ulnar nerve injury which made a good recovery following neurolysis of the ulnar nerve. Type 4 fractures are commonly associated with intra-articular entrapm

Surgical treatment of displaced medial epicondyle fractures in adolescent athletes.

Eight adolescent athletes (average age, 11 years; range, 9 to 15) underwent open reduction and internal fixation of acute, displaced medial epicondyle fractures. Fixation was achieved with a screw and washer. Four patients (50%) had associated elbow dislocations. Elbow motion in a brace was initiated 4 days after surgery. The brace allowed full flexion and extension but protected the elbow against valgus stress. Bracing was continued for 4 weeks. The average duration of followup was 10 months (range, 6 to 13). All fractures united, and full motion was achieved in seven patients. One patient lo

Totally absorbable fixation in the treatment of fractures of the distal femoral epiphyses. A prospective clinical study.

Nine adolescent patients with a femoral fracture involving the physeal plate were treated by using self-reinforced absorbable polyglycolide (SR-PGA) and poly-L-lactide (SR-PLLA) screws with a follow-up for an average of 2 years and 2 months. During the follow-up all but one of the femurs became skeletally mature. In two of nine patients a clinically significant growth disturbance occurred. The average length difference of the femurs was -5 mm (ranging from +8 mm(-)-41 mm). One valgus deformity was noted. In four patients a lengthening and in four patients a shortening of the operated femur wer

Ankle injuries in skiing.

With the development of new ski equipment and improvement in skiing techniques and slope grooming, the incidence, mechanism, and type of ankle injuries suffered during skiing have changed. Ankle sprains, peroneal tendon dislocations, Achilles tendon ruptures, and ankle fractures all can happen as the result of skiing. However, the incidence of these injuries has been dramatically lowered in the past two decades. All of these injuries should be treated aggressively to allow the skier to return to skiing. The early functional treatment of acute ankle sprains will return many skiers to activity q

The ideal methods for the management of rib fractures.

The clinical treatment choices for multiple rib fractures and flail chest are controversial. For example, among conservative treatment and surgical treatment, different studies have different conclusions and recommendations. Furthermore, early clinical research was mainly focused on the treatment of flail chest due to its severity. Nowadays, the treatment for multiple rib fractures patients without a flail chest is drawing an increased clinical interest. However, we are facing many challenges for the treatment of rib fractures, such as insufficient understanding of the available treatment opti

Outcomes of Distal Ulna Fractures Associated With Operatively Treated Distal Radius Fractures.

Background: The purpose of this study was to report outcomes in patients with nonstyloid distal ulna fractures treated in conjunction with open reduction internal fixation (ORIF) of distal radius fractures. Methods: A retrospective review of all patients who had undergone ORIF of a distal radius fracture over a 5-year period at a single institution was performed. Radiographic review was performed to identify patients with a concomitant fracture of the distal ulna. Radiographs were examined to determine whether and how the distal ulna fracture was stabilized and to assess healing of the distal

Treatment of unstable peritrochanteric femoral fractures using a 95 degrees angled blade plate.

OBJECTIVE: To clarify the efficacy of a 95-degree angled blade plate fixation in the treatment of unstable peritrochanteric fractures of the femur. DESIGN: Retrospective, clinical study. SETTING: University hospital. PATIENTS: Thirty-nine consecutive patients with peritrochanteric femoral fractures were followed for a minimum of 12 months. There were 29 subtrochanteric fractures and 10 intertrochanteric fractures (reverse obliquity pattern) for which the compression hip screw could not be used because of comminution of the trochanteric area and fracture extension to the lateral cortex. Intraoperative three-dimensional imaging in the treatment of acute unstable syndesmotic injuries.

BACKGROUND: Acute unstable syndesmotic ankle injuries are treated primarily by reduction and stabilization with a syndesmotic screw. Examination with fluoroscopy or standard radiographs may not provide reliable information about the quality of the reduction. There is evidence that intraoperative three-dimensional imaging can demonstrate a large proportion of malreductions. The aim of this study was to determine whether intraoperative three-dimensional imaging improves the detection of inadequate positioning of the distal aspect of the fibula in the tibiofibular incisura after syndesmotic screw

Minimally Invasive Plate Osteosynthesis with Mini-Open Technique and Supraclavicular Nerve Preservation Reduces Postoperative Numbness in Acute Displaced Midshaft Clavicle Fracture.

Background: Minimally invasive plate osteosynthesis (MIPO) for clavicular shaft fracture yields favorable functional outcomes and results in less surgery-related soft tissue injury than other techniques. Anterior chest and shoulder skin numbness, a common complication after open reduction and plate fixation, is related to injury to the supraclavicular nerves. We propose MIPO combined with a mini-open approach without fluoroscopy for nerve preservation to minimize the risk of postoperative numbness compared with traditional open plating without nerve preservation. Methods: A total of 59 patient

Case Report of Overlapping Pubic Symphysis Dislocation Managed Nonoperatively.

Overlapping pubic symphysis dislocation (OPSD) is a rare form of pelvic injury typically caused by high-energy lateral compression forces, resulting in the entrapment of the pubic body against or behind the contralateral pubic ramus. The majority of reported cases have required open reduction and internal fixation due to the mechanical complexity of the injury. However, less invasive management strategies may be feasible in select cases. We present a case of a 56-year-old male with diabetes who sustained an OPSD associated with a right sacral ala fracture, along with multiple nondisplaced rib

Successful return to work after surgical repair of fracture of the medial condyle of the distal femur in two working farm dogs.

CASE HISTORY: A 5-year-old male Heading dog (working Collie) and a 2-year-old female Huntaway each presented with non-weight-bearing lameness of a hindlimb after jumping or falling with the leg trapped in a gate or motorbike carrier. CLINICAL FINDINGS: Pain and swelling were localised to the stifle, and radiographs revealed a fracture of the medial condyle of the distal femur in each case. DIAGNOSIS: Medial unicondylar, intra-articular fractures of the distal femur. CLINICAL RELEVANCE: Open reduction and internal fixation provided an excellent outcome in each case. Open reduction was achiev

Surgical treatment of sagittal fracture of mandibular condyle using long-screw osteosynthesis.

PURPOSE: The retrospective study evaluated long-screw (bicortical screw) osteosynthesis used in the surgical treatment of sagittal fracture of the mandibular condyle and compared it with titanium plates and removal of the condylar fragment. PATIENTS AND METHODS: Ninety-five patients with sagittal fracture of the mandibular condyle received open surgical treatment from 1997 to 2008. Among these patients, the condylar fragments were fixed with long screws in 56 cases (group A), were fixed with titanium plates in 12 cases (group B), and were completely removed in 24 cases (group C). Hardware Removal in Craniomaxillofacial Trauma: A Systematic Review of the Literature and Management Algorithm.

BACKGROUND: Craniomaxillofacial (CMF) fractures are typically treated with open reduction and internal fixation. Open reduction and internal fixation can be complicated by hardware exposure or infection. The literature often does not differentiate between these 2 entities; so for this study, we have considered all hardware exposures as hardware infections. Approximately 5% of adults with CMF trauma are thought to develop hardware infections. Management consists of either removing the hardware versus leaving it in situ. The optimal approach has not been investigated. Thus, a systematic review o

Balloon acetabuloplasty for cartilage impaction reduction in traumatic acetabular fractures: a case series and technical note.

PURPOSE: Acetabular fractures with cartilage impaction are associated with a high risk of post-traumatic osteoarthritis, frequently requiring total hip arthroplasty (THA). Conventional open reduction and internal fixation (ORIF) often fail to restore joint surface congruity. This case series evaluates the radiological and clinical outcomes of a novel minimally invasive technique using balloon acetabuloplasty to reduce dome impaction in traumatic acetabular fractures. METHODS: This retrospective case series included patients with acetabular fractures involving cartilage impaction. Balloon acet

Temporising external fixation of calcaneus fractures prior to definitive plate fixation: a case series.

INTRODUCTION: Surgical management of calcaneus fractures is technically demanding and has a high risk of wound complications. These fractures are traditionally managed with splinting until swelling has subsided, which can take weeks and leaves the fracture fragments displaced. We describe a novel protocol for the management of displaced intraarticular calcaneus fractures that utilises a temporising external fixator and staged conversion to plate fixation through a sinus tarsi approach. The goal of this technique was to enable earlier treatment with open reduction and internal fixation, minimis

External fixation for displaced intra-articular calcaneal fractures: A case series.

Intra-articular calcaneus fractures (CF) remain challenging. Surgery aims to restore subtalar joint congruence and calcaneus width, height, shape, and alignment, thereby avoiding medial and lateral impingement and enabling patients to resume a normal lifestyle. We retrospectively evaluated 32 patients (35 fractures) with a minimum displacement of 2 mm of the posterior facet and disruption of the calcaneus length, width, and height. Treatment involved a minimally invasive sinus tarsi approach and definitive stabilization with an external fixator (EF). The mean time to partial weight-bearing was

Do we need standardized postoperative radiographs after locking plate fixation of distal radius fractures - A retrospective analysis of 664 patients.

INTRODUCTION: After surgical treatment of fractures of the distal radius, radiographs in 2 planes are routinely performed postoperatively as a standard procedure to verify anatomic reduction and implant positioning. However, the postoperative radiological examinations rarely has a consequence. Therefore, the purpose of this study was to evaluate the frequency of treatment plan changes based on standardized postoperative radiographs. Secondarily, abnormalities, already being present in the intraoperative radiographs, were examined. METHODS: Between 2015 and 2019 a total of 664 consecutive pati

ANALYSIS OF THE SEGMENTAL IMPACTION OF FEMORAL HEAD FOLLOWING AN ACETABULAR FRACTURE SURGICALLY MANAGED.

OBJECTIVE: Correlate the postoperative radiographic evaluation with variables accompanying acetabular fractures in order to determine the predictive factors for segmental impaction of femoral head. METHODS: Retrospective analysis of medial files of patients submitted to open reduction surgery with internal acetabular fixation. Within approximately 35 years, 596 patients were treated for acetabular fractures; 267 were followed up for at least two years. The others were excluded either because their follow up was shorter than the minimum time, or as a result of the lack of sufficient data repor

Arthrodesis versus ORIF for Lisfranc fractures.

The Lisfranc joints make up the bony structural support of the transverse arch in the midfoot and account for approximately 0.2% of all fractures. Early recognition and treatment of this injury are paramount to preserving normal foot biomechanics and function. Controversy exists regarding the optimal treatment of patients with Lisfranc injuries, particularly when the instability is entirely ligamentous. The authors performed a qualitative, systematic review of the literature to compare the 2 most common procedures for Lisfranc fractures: primary arthrodesis and open reduction and internal fixat

Contralateral Back Pain Following Trans-sacral Trans-iliac Screw Fixation: Resolution After Hardware Removal.

CASE: A 16-year-old girl presented to the emergency department after a 6-foot fall while skiing. The patient presented with left-sided pelvic pain and acute blood loss anemia. She was diagnosed with pelvic ring fracture (LC-2) and underwent open reduction internal fixation using a trans-sacral trans-iliac (TSTI) screw. She initially recovered but reported chronic right-sided back pain 4 years later. After hardware removal, she remained symptom-free for 1 year. CONCLUSION: Very few cases report contralateral pain associated with TSTI screws penetrating the opposite SI joint. This report highli
