Orbital Complication in the Setting of Cocaine Use: A Case Report and Review of Literature.

Orbital cellulitis is a common ophthalmologic consultation and has numerous risk factors; however, one that is seldomly encountered is chronic cocaine use. We describe a case of a 63-year-old man with a history of HIV and cocaine use who presented with OD pain, proptosis, and blurred vision. CT imaging revealed extensive erosions throughout the nasal septum, bilateral turbinates, ethmoid sinuses, and loss of the right medial orbital wall. The patient was treated empirically with broad-spectrum antibiotics, and a nasal biopsy and culture grew Staphylococcus aureus. After treatment with IV antib…

Modified Technique of Temporal Osteotomy in Fronto-Orbital Advancement in Anterior Plagiocephaly.

BACKGROUND: Nonsyndromic anterior plagiocephaly is one of the most common types of craniosynostosis. In recent years, various surgical techniques have been introduced to correct this deformity, which many surgeons believe have not been associated with adequate clinical and aesthetic results. The aim of the study was to introduce a new surgical technique (modified Z-technique of temporal region osteotomy) and to evaluate the aesthetic and anthropometric outcomes of frontal orbital correction for the management of patients with nonsyndromic anterior plagiocephaly. METHODS: In this cohort study,…

Orbital Apex Syndrome Secondary to Retained Foreign Body: A Case Report.

Orbital foreign bodies, especially organic materials, can cause severe eye and orbital damage. Orbital apex syndrome (OAS), a complex condition affecting multiple cranial nerves, is often caused by trauma, among other factors. The patient was a three-year-old boy who fell onto a tree stump three days prior. He presented to the emergency department with left-sided eyelid edema, ptosis, traumatic mydriasis, numbness, and ophthalmoplegia and was diagnosed with OAS. Despite treatment with intravenous methylprednisolone, analgesia, and antibiotics, his condition did not improve after the transnasal…

Orbital Abscess Secondary to Nasolacrimal Duct Obstruction in an Extremely Preterm Infant.

Orbital abscesses are rarely encountered in children younger than 1 year. The literature is limited to isolated case reports and a few case series. Most such cases are reported in infants born at term, with the earliest reported gestational birth age at 34 weeks. Children are more prone to orbital cellulitis compared with adults due to their underdeveloped sinuses and immature immune systems, and the origin is most commonly an ethmoid sinus infection. Orbital cellulitis secondary to dacryocystitis is even less common, with only a few isolated cases reported in infants and children. Herein, the…

Orbital tuberculosis presenting as vision loss and headache: early management is paramount.

Orbital tuberculosis is a rare form of extra pulmonary TB and may arise either by hematogenous route or spread directly from the paranasal sinus. We herein report two cases of orbital TB with a vision threatening complication. Case-1 is a 31-year-old female with a headache, a diminution of vision in the right eye, and pain in ocular movement. On examination, there was no proptosis with RAPD present in right eye and tenderness on palpation. CEMRI revealed a diffuse infiltrating lesion at the orbital apex, suggesting of inflammatory pathology. Case-2 is a 40-year-old male with similar complaints…

A Case of Supraorbital Hypoesthesia Following Double Orbital Sphere Volume Implant.

The patient is a 63-year-old female with past ocular history of end-stage glaucoma in her OD. To maintain symmetry and achieve cosmesis with her right orbit, she chose to proceed with enucleation of the eye with a donor sclera-wrapped 20-mm silicone sphere implant. After 1 year, she developed right enophthalmos due to volume loss. The patient opted for an additional sphere to be placed posterior to the original sphere. A 14-mm silicone sphere was placed through a lateral orbitotomy into the intraconal space. On postoperative day 1, the patient developed numbness along the V1 trigeminal nerve i…

Orbital reconstruction: titanium mesh implant after excision of orbitozygomaticomaxillary tumors.

Pathologic lesions of the orbitozygomaticomaxillary complex (OZMC) and caudal oral cavity can be a challenge in veterinary oromaxillofacial surgery. Neoplastic lesions that are in close proximity to or invading the orbit may result in significant loss of structural integrity after curative intent surgery. This in turn may alter the topography of the bulbous oculi (globe) with resultant enophthalmos, diplopia, and entropion. Historically, orbital exenterations have been deemed a suitable option to avoid these complications. However, lesions that do not include the globe in the surgical margin m…
