[Reconstruction of the frontoorbital frame using split-thickness calvarial bone grafts].

BACKGROUND: There are many autogenous and allogenous grafts as well as alloplastic materials available for the reconstruction of craniofacia … ructed. The causes of the bone defects were trauma (n = 6), recurrent frontal sinusitis partly with osteomyelitis (n = 4), benigne tumors (n = 2) and malignancies (n = 3). Twelve patients have had multiple previous operations. In 3 patients the bone reconstruction was performed in the same operation as the tumor removal. Within the follow-up period between 2 an …

Disseminated infection due to Chrysosporium zonatum in a patient with chronic granulomatous disease and review of non-Aspergillus fungal infections in patients with this disease.

We report the first case of Chrysosporium zonatum infection in a 15-year-old male with chronic granulomatous disease who developed a lobar pneumonia and tibia osteomyelitis while on prophylaxis with gamma interferon. The fungus was isolated from sputum and affected bone, and hyphae were observed in the bone by histopathology. Therapy with amphotericin B eradicated the osteomyelitis and pneumonia, but pneumonia recurred in association with pericarditis and pleuritis during therapy with itraconazole. These manifestations subsided, and no recurrences occurred with liposomal amphotericin B therapy…

[Testing of cementless stepped-stem prosthetic devices replacing the hip shoulder joint of the dog. Comparison of two animal models (author's transl)].

In 6 dogs the hip joint was experimentally replaced using a ceramic stepped-stem prosthetic device implanted in the thigh bone. In six other dogs V4A steel stepped-stem prosthetic devices inserted in the arm bone was used for replacing shoulder joint. The six devices replacing the shoulder joint healed up without complications and proved to be mechanically stable. From six devices replacing the hip joint only one was successful; the remaining five implants did not heal up due to fractures of the stem, loosening in the thigh bone and osteomyelitis. We suggest that these different results are ba…

Vascularized fibula graft for the traumatically induced long-bone defect.

Long-bone defects resulting from trauma can be managed with a variety of techniques, including conventional bone grafting, distraction osteogenesis, bone graft substitutes, prosthetic devices, and vascularized bone grafting. Although there is an array of available methods for long-bone reconstruction of bony defects, vascularized bone transfer is particularly useful in large defects (> 6 cm) and in cases in which osteomyelitis and unstable soft-tissue or beds make conventional techniques difficult.

Surgical correction of craniosynostosis in malignant osteopetrosis.

In osteopetrosis, bone healing is complicated by progressive sclerosis. A 2-year-old blind boy with malignant osteopetrosis developed increased intracranial pressure secondary to craniosynostosis. Osteopetrosis had already been treated with bone marrow transplantation (BMT), and bone remodeling seemed to be restored. A craniotomy was performed. The postoperative course and bone healing were uneventful. After successful BMT, craniofacial procedures seem to be possible without a high risk for osteomyelitis.

[Bone scintigraphy in children from the surgical and orthopedic viewpoints].

Sixty children with Legg-Perthes disease (19), bone tumor (27), osteogenesis imperfecta (7), osteomyelitis (5) and transient synovitis (2) were studied using 99mTc labeled diphosphonate. A number of benign or malign bone diseases of children need early detection in order to institute the best form--the fine form--of treatment. We recommend the bone scintigraphy in the initial screening of children with signs and symptoms of bone pathology.

Fusobacterium necrophorum septicemia following oropharyngeal infection.

Fusobacterium necrophorum septicemia developed in five patients after an oropharyngeal infection. Four patients had sore throat or neck pain, and two had findings of jugular vein septic thrombophlebitis. Metastatic abscesses, including embolic pneumonia, empyema, septic arthritis, and osteomyelitis, also occurred. Four patients recovered and one died. Proper treatment requires recognition of the oropharyngeal source of the septicemia and its differentiation from endocarditis. Antibiotic therapy should be prolonged, and metastatic abscesses drained.

The "hot" patella.

Increased patellar uptake on bone scans is seen quite commonly but the possible or probable etiologies of this finding have not been previously well described. A review of 100 consecutive bone scans showed that the incidence of bilateral "hot" patellae is 15%. Identified etiologies include osteoarthritic degenerative disease (35%), fracture, possible metastatic disease, bursitis, Paget's disease, and osteomyelitis. The value of careful history, physical examination, and radiographs is stressed.

Osteomyelits of the pubic bone with vesicocutaneous - vesicovaginal fistula: A delayed complication of post-cervical cancer radiotherapy.

Vagino-vesico-cutaneous fistula is a rare condition characterised by continuous dribbling of urine and secondary infection of the involved areas with poor self-esteem. Osteomyelitis is delayed complication of radiotherapy treatment for cervical cancer. Treatment of these conditions is a challenging entity especially after previous surgery and irradiation. We present a case of vesicocutaneous - vesico vaginal fistula with osteomyelits of the right pubic bone which was a late complication of post-cervical cancer radiotherapy.

Severe cystic periventricular leukomalacia in a premature infant with capnocytophaga sepsis.

Capnocytophaga is an opportunistic gram-negative anaerobic bacillus found in the oropharyngeal cavity of mammals and is associated with periodontal disease in humans. Sepsis, osteomyelitis, lung abscess, endocarditis, and meningitis have been reported in humans following animal bites. Perinatal infection with Capnocytophaga is infrequent and is generally considered to have a low risk of morbidity to the mother and fetus. We report a case of neonatal Capnocytophaga sepsis associated with the development of severe cystic periventricular leukomalacia.

Caffey disease or infantile cortical hyperostosis: a case report.

Caffey disease or Infantile Cortical Hyperostosis (ICH) is a rare and mostly self limiting condition affecting young infants. It is characterized by acute inflammation of the periostium and the overlying soft tissue and is accompanied by systemic changes of irritability and fever. Diagnosis may be delayed as this disorder mimics a wide range of diseases including osteomyelitis, hypervitaminosis A, scurvy, bone tumors and child abuse. The emphasis here is to remind clinicians about the existence of the disease in this country.

Scalp abscess--a cautionary tale.

Transcranial extension of frontal sinus infection is a rare, but not eradicated entity. We present a 21-year-old male, in whom a persistent scalp abscess heralded the discovery of skull vault osteomyelitis and extradural abscesses secondary to frontal sinusitis. Patients with prolonged or unusual symptoms with a history of sinusitis or trauma warrant further investigation as they may have developed serious intracranial complications. Urgent management, both surgical and antimicrobial, is indicated in such scenarios.

Necrotizing otitis externa: a painless reminder.

Necrotizing otitis externa is a necrotizing osteomyelitis of the skull base that predominantly affects diabetic and immunocompromised patients. Pseudomonas aeruginosa is the most frequently implicated organism. Patients present with severe otalgia, otorrhoea with or without facial palsy, or rarely, facial pain as a symptomatic feature. We report a case of necrotizing otitis externa presenting with totally painless otorrhoea and highlight the need for a high index of suspicion when treating patients with otitis externa.

Lemierre's syndrome with spondylitis and pulmonary and gluteal abscesses associated with Mycoplasma pneumoniae pneumonia.

Lemierre's syndrome, a systemic anaerobic infection caused by Fusobacterium necrophorum, is characterized by an acute oropharyngeal infection, septic thrombophlebitis of the internal jugular veins, sepsis, and multiple metastatic infections. It commonly leads to pulmonary parenchymal abscesses and occasionally to septic arthritis, osteomyelitis, or liver or spleen abscesses. Reported here is a case of spondylitis and pulmonary and gluteal abscesses that occurred as part of a classic presentation of Lemierre's syndrome. Treatment with imipenem and clindamycin was successful.

[Torticollis--a harmless symptom?].

Acquired non-traumatic torticollis may be due to a variety of causes. Presumably it may arise from inflammatory diseases and tumours of the neck as well as the central nervous system. Three cases of acquired torticollis due to osteomyelitis, retropharyngeal carcinoma and subluxation of the spine are presented and the variety of possible underlying diseases is demonstrated. Persistent torticollis should provoke an aggressive search for a specific aetiology by all means, especially CT scans or even biopsy.

Persistent water diuresis in renal amyloidosis. A case report.

A 67-year-old male with previous history of osteomyelitis presented with persistent thirst and polyuria. His urine remained hyposmolar to plasma after overnight fasting and administration of pitressin. Biopsy and postmortem examination revealed renal amyloidosis with involvement of many glomeruli and smaller vessels. Amyloid was also found in the medulla, particularly around the collecting tubules. It is suggested that amyloid interfered directly with diffusion of water through the walls of the collecting tubules.

SAA amyloid protein levels in amyloid-prone chronic inflammatory disorders. Lack of association with amyloid disease.

Serum amyloid protein A (SAA) levels were evaluated by a sensitive radioimmunoassay technique in leprosy, juvenile rheumatoid arthritis and chronic osteomyelitis. SAA was elevated in all 3 groups of patients. The level in each of the patient groups did not reflect the presence of secondary amyloidosis. Our study suggests that serum SAA protein levels should not be considered a routine diagnostic test in patients with chronic inflammatory diseases prone to the development of amyloid disease.

[Compression osteosynthesis with percutaneous bone screws--clinical aspects and radiographic findings].

In a previous paper (Jacobs 1976) the development of an apparatus was described which achieved a compression osteosynthesis through the use of percutaneous bone screws without encroaching on the area of the fracture. As indication for this procedure we consider the condition which is described under the synonyms "post-traumatic osteomyelitis", "osteitis" and "fracture infection". Clinical experience is described with consideration of technical measurements - for the first time in vivo measurements of interfragmental compression in the patient and X-ray findings.
