Type 1 diabetes mellitus in children: Patient reported outcomes.

The global prevalence of type 1 diabetes mellitus (T1DM) is increasing. Poor glycemic control in children and adolescents leads to both acute and chronic problems, reduced Health Related Quality of Life (HRQoL), and higher healthcare utilization. This study aimed to assess patient-reported outcomes related to self-management adherence, QoL, diabetes-related stigma, glycemic control, and other clinical outcomes along with their determinants, in insulin-treated pediatric T1DM patients in Jordan. A cross-sectional study was conducted from April to October 2023 at two health centers in Northern Jo…

Community-based participatory action research on rheumatic heart disease in an Australian Aboriginal homeland: Evaluation of the 'On track watch' project.

Strategies to date have been ineffective in reducing high rates of rheumatic heart disease (RHD) in Australian Aboriginal people; a disease caused by streptococcal infections. A remote Aboriginal community initiated a collaboration to work towards elimination of RHD. Based in 'both-way learning' (reciprocal knowledge co-creation), the aim of this study was to co-design, implement and evaluate community-based participatory action research (CBPAR) to achieve this vision. Activities related to understanding and addressing RHD social determinants were delivered through an accredited course adapted

Opposing Age-Related Trends in Absolute and Relative Risk of Adverse Health Outcomes Associated With Out-of-Office Blood Pressure.

Participant-level meta-analyses assessed the age-specific relevance of office blood pressure to cardiovascular complications, but this information is lacking for out-of-office blood pressure. At baseline, daytime ambulatory (n=12 624) or home (n=5297) blood pressure were measured in 17 921 participants (51.3% women; mean age, 54.2 years) from 17 population cohorts. Subsequently, mortality and cardiovascular events were recorded. Using multivariable Cox regression, floating absolute risk was computed across 4 age bands (≤60, 61-70, 71-80, and >80 years). Over 236 491 person-years, 3855 people d

Prediction of outliers in pain, analgesia requirement, and recovery of function after childbirth: a prospective observational cohort study.

BACKGROUND: Prediction models to identify parturients who experience protracted pain, prolonged opioid use, and delayed self-assessed functional recovery are currently inadequate. METHODS: For this study, 213 nulliparous women who planned vaginal delivery were enrolled and assessed daily until they completed three outcomes: (1) pain resolution; (2) opioid cessation; and (3) self-assessed functional recovery to predelivery level. The primary composite endpoint, 'pain and opioid-free functional recovery' was the time required to reach all three endpoints. The subjects were divided into two cate

Clinical observation of self-injurious behavior correlated with changes in scalp morphology in a child with congenital hydrocephalus.

We report the case of a 12-year-old girl born with cerebral dysgenesis and congenital hydrocephalus first shunted shortly after birth. She had severe tissue-damaging self-injurious behavior, profound mental retardation, quadriparesis, as well as multiple cranial anomalies including turricephaly. After stage 1 cranial remodeling, a bone window was left pending second stage remodeling. Episodic changes in fluctuation of the scalp overlying the bone window were easily observed. During the course of a behavioral assessment for her self-injury, it was observed that the overall frequency of occurren

Understanding and managing dentin hypersensitivity.

Dental hygienists frequently encounter clients who present with one or several sensitive teeth. These clients often rely on the dental hygienist to help determine the cause and to provide supportive treatment to relieve their discomfort. Therefore, it is essential for dental hygienists to fully understand the complexity of dentin sensitivity, be well informed about treatment approaches, and have an appreciation of the difficulties inherent in designing and conducting clinical hypersensitivity trials. Difficulties in designing hypersensitivity research have precluded a definitive answer concern

The Alumni program: redefining continuity of care in psychiatry.

The Alumni Program is a novel approach to the continuing care and preventive treatment of clients with "chronic" mental illness. This approach demands a different conceptualization of outpatient psychiatric intervention. The infectious disease model has been deliberately replaced with a chronic illness model that more accurately reflects the course and natural history of psychiatric illness. The Alumni Program is similar to the approach employed with other chronic medical conditions like arthritis, asthma, or diabetes. The program has complementary roles for both specialty psychiatric clinicia

Phenotyping the Transfusion Management Experience of Trainees Using Electronic Health Records.

BACKGROUND: Clinical experience is foundational to graduate medical education but is difficult to quantify. Self-reported case logs and scheduled rotations provide limited detail on residents' actual experiences. The analysis of electronic health record (EHR) data may provide a more comprehensive view of residents' actual clinical experiences. We aimed to quantify and characterize residents' direct clinical experiences with intraoperative transfusion management as a proof of concept for the utility of EHR data in anesthesiology graduate medical education. METHODS: This retrospective observati

Impact of Cognitive Impairment and Depression on Asthma in Older Patients.

Depression and cognitive decline in older adults with asthma are associated with poorer disease control, increased risks for exacerbations, higher health care utilization, and greater mortality risk. In this age group, depression and cognitive impairment also negatively impact self-management behaviors and alter perception of asthma symptoms. Conversely, data suggest that in some individuals, asthma can induce depression and cognitive deficits, increasing the risk of dementia, including Alzheimer's disease. Because there is a bidirectional communication between the lungs and brain, asthma, par…

Hard to Swallow: Tongue Avulsion Secondary to Hypocortisolism Posing a Difficult Airway.

Maxillofacial trauma involving tongue injuries poses a significant challenge for airway management due to active bleeding, anatomical distortion, and the risk of obstruction and aspiration. We report the case of a 37-year-old man who presented with a self-inflicted tongue avulsion, likely in the context of an epileptic seizure. The patient was orotracheally intubated and transferred by helicopter under sedation and mechanical ventilation to a tertiary center, where the surgical team requested nasotracheal intubation to facilitate intraoral access. However, active bleeding and avulsion of tongu

A Complex Presentation: Psychosis in a Patient Diagnosed With Lennox-Gastaut Syndrome.

Lennox-Gastaut syndrome (LGS) is a form of severe childhood epilepsy, with most children experiencing seizures before reaching the age of eight. Typically, patients have multiple types of seizures, making an accurate diagnosis challenging. While it can be secondary to other causes, often, it is idiopathic. Over time, children develop cognitive impairment, leading to intellectual disability. The mainstay of treatment and management is seizure control. However, management remains challenging due to the complexity of the syndrome, as it is associated with multiple seizure types, intellectual dete
