Atrial fibrillation (AF) is the most common cardiac arrhythmia and shares many of the same risk factors as another common clinical condition, sleep apnea. The estimated prevalence of sleep apnea has increased over the past decade, and reflects a parallel increase in the most prominent risk factors of obesity and overweight. Both obstructive and central sleep apnea have been associated with AF in multiple studies, with the risk of AF increasing 2-4-fold compared to those without sleep breathing disorder. Continuous positive airway pressure (CPAP) has been shown to reduce the rate of AF recurren

Sleep disordered breathing causes repetitive episodes of nocturnal hypoxemia, sympathetic nervous activation, and cortical arousal, often associated with excessive daytime sleepiness. Sleep disordered breathing is common in people with, or at risk of, cardiovascular (CV) disease including those who are obese or have hypertension, coronary disease, heart failure, or atrial fibrillation. Current therapy of obstructive sleep apnea includes weight loss (if obese), exercise, and positive airway pressure (PAP) therapy. This improves daytime sleepiness. Obstructive sleep apnea is associated with incr

Sleep apnea is a common sleep disorder characterized by recurrent episodes of breathing cessation and resumption during sleep. The three main types are obstructive sleep apnea (OSA), central sleep apnea (CSA), and mixed sleep apnea (MSA), the latter of which is often used interchangeably with complex sleep apnea (CompSA). OSA, the most prevalent form, results from upper airway obstruction during sleep. Globally, nearly one billion adults aged 30-69 are affected by OSA, with approximately 425 million having moderate to severe disease, making sleep apnea a major global health burden affecting ro

Sleep disorders are classified in dyssomnias, parasomnias, sleep disorder associated with medical and psychiatric disorders and proposed sleep disorders. Only the parasomnias have been studied as such in the newborn period. The parasomnias that occur in this age group are infant sleep apnea, congenital central hypoventilation syndrome, sudden infant death syndrome, and benign neonatal sleep myoclonus. Infant sleep apnea includes three entities: (1) apnea of prematurity, (2), apparent life threatening episodes with apnea and (3) obstructive sleep apnea. Congenital central hypoventilation syndro

Polysomnograms were obtained from two groups of normal subjects (20 medical students and 20 elderly persons). The recordings included the usual sleep parameters (electroencephalograph, electrooculograph, and electromyograph of chin muscles) and respiratory data obtained by means of a pneumotachygraph (tidal volumes and respiratory frequency) and thoracic and abdominal strain gauges. In the older group, sleep was more disturbed and respiratory events (hypopneas and apneas) were more frequent both in non-rapid eye movement (NREM) and in REM sleep. The apneas were predominantly of the obstructive

BACKGROUND: Obstructive sleep apnea is common and associated with poor outcomes after stroke or transient ischemic attack (TIA). We sought to determine whether the intervention strategy improved sleep apnea detection, obstructive sleep apnea (OSA) treatment, and hypertension control among patients with chronic cerebrovascular disease and hypertension.
METHODS: In this randomized controlled strategy trial intervention, patients received unattended polysomnography at baseline, and patients with OSA (apnea-hypopnea index ≥5 events/h) received auto-titrating continuous positive airway pressure (C

OBJECTIVE: Measurement of esophageal pressure is considered to be the gold standard for assessing respiratory effort and neural respiratory drive but this requires placement of an esophageal catheter. We hypothesized that neural drive could be reflected by the diaphragm EMG recorded from chest wall surface electrodes.
METHODS: We simultaneously recorded esophageal pressure and the chest wall diaphragm EMG in 11 patients with suspected obstructive sleep apnea/hypopnea syndrome during full night polysomnography. We also recorded the diaphragm EMG from esophageal electrode in 5 of the 11 subject

Snoring, chronic rhinitis and fragmentation of sleep are common symptoms in pregnancy, especially in the third trimester. The prevalence of obstructive sleep apnea is also increased in pregnant women and can be associated with pregnancy induced hypertension, preeclampsia and diabetes mellitus. The fetus is as well exposed to a higher risk of prematurity and intra-uterine growth restriction. Given the lack of adequate screening tool, the diagnosis of obstructive sleep apnea is a challenge for the physician and a high degree of clinical suspicion is necessary in order to identify the patients at

Rapid eye movement sleep distribution changes during development, but little is known about rapid eye movement latency variation in childhood by age, sex, or pathologic sleep states. We hypothesized that: (1) rapid eye movement latency would differ in normal children by age, with a younger cohort (1-10 years) demonstrating shorter rapid eye movement latency than an older group (>10-18 years); (2) rapid eye movement latency in children would differ from typical adult rapid eye movement latency; and (3) intrinsic sleep disorders (narcolepsy, pediatric obstructive sleep apnea syndrome) would disr

INTRODUCTION: There is no consensus on a single classification system for the obstructive findings in drug-induced sleep endoscopy. Previous classification systems have neglected to address the upper retropalatal obstruction, the segmental division of the lateral pharyngeal wall and the primary or secondary nature of laryngeal collapse.
OBJECTIVE: To propose, illustrate and evaluate a more comprehensive and yet simple classification for drug-induced sleep endoscopy findings.
METHODS: Cross sectional study in a tertiary sleep surgery unit. A total of 30 patients with obstructive sleep apnea u

The benzodiazepines are sedative hypnotic drugs, i.e., central nervous system depressant drugs, that may adversely affect the control of ventilation during sleep. Prescription of these drugs may worsen sleep-related breathing disorders, especially in patients with chronic obstructive pulmonary disease or cardiac failure. The most frequent users of sedative hypnotics are the polymorbid elderly with a secondary complaint of insomnia. Although the benzodiazepines may reduce sleep fragmentation, their long-term use may also cause health problems, such as complete obstructive sleep apnea in heavy s

Nineteen diabetic patients, 12 type I (insulin-dependent) and 7 type II (late-onset, non-insulin-dependent), underwent nocturnal polygraphic monitoring after a daytime medical evaluation that included tests of vagal responses and, in 6 patients, pulmonary function and hypercapnic and hypoxic responses. Five lean type I patients had abnormal sleep-related breathing patterns with central or obstructive sleep apnea and brief breathing irregularities during stages 3 to 4 non-rapid-eye-movement sleep, compared with only 1 overweight type II diabetic with moderate obstructive sleep apnea. There was

Studying chemical control of ventilation implies evaluation of both chemoreceptor functions taken into account however the mechanical factors influencing the effector organs. The role of abnormal chemical drives has been demonstrated in COPD patients. More recently the role of abnormal chemical drives was studied during sleep. Absent or severely depressed drives may facilitate the development of central apneas and hypoventilation. High drives may lead to periodic breathing eventually with central apneas as well. Most intriguing therefore is the role of chemical drives in the pathogenesis of th

PURPOSE: To provide clinicians in primary care settings information on the effects of sleep-disordered breathing in patients with heart failure (HF). Assessment and screening tools, as well as management considerations, are presented.
DATA SOURCES: Review of the scientific literature of the past 10 years, along with classic studies and Internet sources.
CONCLUSIONS: HF is an increasingly prevalent problem with a high degree of associated sleep-disordered breathing. There are two broad categories of sleep-disordered breathing: obstructive sleep apnea and central sleep apnea/Cheyne-Stokes brea

Patients with sleep apnea syndrome (SAS) suffer considerable morbidity and increased mortality. The most common symptoms of SAS include excessive daytime sleepiness, nocturnal breath cessation, snoring and gasping sounds. We reviewed the characteristics of 20 Saudi patients with sleep apnea (15 males, five females) who were studied at our tertiary care center in 1992 and 1993. The ages ranged from 15 to 61 years. All were obese. Two each were acromegalic and hypothyroid. Hypertension was noted in seven (35%) patients and one had symptomatic bradycardia. Two (10%) patients presented with recurr

In this study, we collected and analyzed polysomnography (PSG) data to investigate the value of PSG in diagnosing sleep problems in children. The results of PSG studies of children (<18 years old) with sleep problems conducted from April 2015 to May 2017 at a children's hospital in Taiwan were collected and analyzed retrospectively. Data for 310 patients (209 males and 101 females) who underwent PSG were collected. The final diagnoses were as follows: obstructive sleep apnea in 159 (51.3%), snoring in 81 (26.4%), limb movement sleep disorder in 25 (8.1%), hypersomnias in 12 (3.9%), central apn

The purpose of this study was to determine the effect of acute nasal obstruction on sleep and breathing in eight normal persons. The subjects were randomized into two groups. One night the subject was studied with the nose open and a second night with the nose obstructed. The electroencephalogram, electrocardiogram, inspiratory effort, nasal and oral airflow, and oxygen saturation were monitored. Sleep proved to be both subjectively and objectively disturbed. The subjects with the nose obstructed awoke more often, had a greater number of changes in sleep stage, had a prolongation of rapid-eye-

Diagnosing sleep disordered breathing requires manual annotation of events from sleep studies, such as nocturnal polysomnography, a process that is time-intensive, costly, and prone to inter-rater variability. Automatic approaches exist but lack generalizability due to signal variability across centers. We develop an automatic apneic breathing event detector to localize and classify obstructive apneas, central apneas, hypopneas, and isolated respiratory events without arousals or desaturations. The model is trained on 5456 polysomnographies and tested on 1099 polysomnographies from six cohorts

Patients with complex sleep apnea syndrome (CompSAS) present with features of obstructive sleep apnea syndrome but demonstrate not only instability of upper airway tone (leading to classic obstructive apneas and hypopneas) but also unstable, chemosensitive ventilatory control leading to repetitive central apneas or periodic breathing during sleep. The central apneas often become most apparent after application of continuous positive airway pressure (CPAP) to alleviate upper airway obstruction; patients continue to have fragmented sleep and repetitive desaturations as a result of central apneas
