Allergic rhinitis: indicators of quality of life.

The objective of this review was to present evidence of the relationship between allergic rhinitis and impairment of quality of life. The data sources were original articles, reviews and consensus statements entered into the Medline and LILACS databases between 1997 and 2008. The following search terms were used: 'allergic rhinitis'; 'quality of life'; and 'sleep disorders'. Quality of life is often impaired in patients with allergic rhinitis, due to the classic symptoms of the disease (sneezing, pruritus, rhinorrhea and nasal obstruction). In addition, the pathophysiology of allergic rhinitis

Narcolepsy: clinical features, co-morbidities & treatment.

Narcolepsy is a neurologic illness that typically begins in the second and third decades of life. It is chronic in nature and negatively impacts the quality of life of affected patients. The classic presentation is a tetrad of excessive daytime sleepiness, cataplexy, sleep paralysis, and hypnagogic hallucinations. The exact cause remains unknown, but there is significant evidence that hypocretin deficiency plays an integral role. Some primary conditions that result in secondary narcolepsy include traumatic brain injury, congenital disorders, tumours, and strokes. Some medical and psychiatric d

Delayed onset of the diurnal melatonin rise in patients with Huntington's disease.

Sleep disturbances are very prevalent in Huntington's disease (HD) patients and can substantially impair their quality of life. Accumulating evidence suggests considerable dysfunction of the hypothalamic suprachiasmatic nucleus (SCN), the biological clock, in both HD patients and transgenic mouse models of the disease. As melatonin has a major role in the regulation of sleep and other cyclical bodily activities and its synthesis is directly regulated by the SCN, we postulated that disturbed SCN function is likely to give rise to abnormal melatonin secretion in HD. Therefore, we compared 24 h m

Approach to gastroesophageal reflux disease in primary care: Putting the Montreal definition into practice.

OBJECTIVE: To apply the recently published Montreal definition of gastroesophageal reflux disease (GERD) in primary care. SOURCES OF INFORMATION: The Montreal definition of GERD was developed by an international consensus group of experts in GERD and primary care physicians using rigorous evidence-based methods along with modern consensus development techniques and a patient-centred approach. MAIN MESSAGE: Gastroesophageal reflux disease can be diagnosed in primarycare based on symptoms alone without additional diagnostic testing. Symptoms reach a threshold where they constitute disease when

Pediatric allergic rhinitis: physical and mental complications.

Allergic rhinitis (AR) affects up to 40% of children in the United States and its prevalence continues to increase. Most AR develops during the pediatric years and it is the most common chronic allergic disorder seen in children. It is important to note that AR is more than just sneezing and a nuisance for the children. There are numerous complications that can lead to significant problems both physically and mentally in the child who suffers with AR. Under physical complications, otitis media with effusion, recurrent and/or chronic sinusitis, asthma, and snoring impact children with AR. Sleep

[Parkinson's disease and associated disorders].

Besides the core motor features of Parkinson's disease, other disorders such as gastro-intestinal dysfunction, postural hypotension, urinary, genital, sleep problems and pain contribute to the alteration of patient's quality of life. Drooling, swallowing difficulties and constipation are the more frequent digestive problems. Aspirations may be life-threatening. Sexual dysfunction as well as iatrogenic hypersexuality may be deleterious for the couple well-being. Symptomatic postural hypotension is the main manifestation of autonomic failure and needs a specific management. Pain is frequent in P

Sleep disturbances among methadone maintained patients.

We examined the relationship of sleep disturbance and demographic, mental health, drug use and other factors among 225 methadone-maintained individuals. The cohort was 78% Caucasian and 54% male with a mean age of 41 years. Sleep disturbance was measured using the Pittsburgh Sleep Quality Index (PSQI) with a score >5 indicating poor global sleep quality. Eighty-four percent of subjects had PSQI scores of six or higher. In multivariate analysis, depressive symptoms, anxiety symptoms, greater nicotine dependence, bodily pain, and unemployment were associated with poorer global sleep quality (p <

Sleep and anxieties in Brazilian children: the role of cultural and environmental factors in child sleep disturbance.

To test the contextual hypothesis of sleep disturbance, disadvantaged urban Brazilian children's self-reported fears were correlated with parental reports of children's sleep disturbance. Fears of death at the hands of "death squads," environment-related worries and other fears reflecting Brazil's political, religious, and collective culture all correlated with sleep disturbance. Results are contrasted with findings of earlier studies on sleep and anxieties in samples of learning-disabled adolescents and victims of lightning.

Sleep abnormalities in patients with brain stem lesions.

Seven patients with "locked-in" syndrome were studied by prolonged polygraphic recordings. Severe alterations in the sleep pattern were observed in five patients who had bilateral extensive pontine lesions resulting in tetraplegia, facial and pseudobulbar paralysis, and absence of conjugate horizontal gaze. Rapid eye movement (REM) sleep was entirely absent while non-rapid eye movement (NREM) sleep was absent, reduced, or altered. The remaining two patients, both of whom had relative sparing of horizontal gaze and apparently either no or minimal pontine tegmental involvement, showed both REM a

Circadian rhythms, endogenous glucocorticoids and skeletal health.

Circadian rhythms are a hallmark of life. They are present in numerous physiological functions, including behavioural patterns such as sleep-wake cycles or feeding-fasting behaviour, hormonal secretion, core body temperature, cardiovascular and brain activity, cell regeneration and metabolic processes. Most organisms have developed evolutionarily conserved biological clock mechanisms that effectively react to cyclic changes in the photic environment to synchronize behaviour and physiological processes accordingly. These "pacemaker" like clocks are present in almost every cell and are intrinsic

Management of Subsyndromal Delirium With Daridorexant and Quetiapine: A Case Report.

Subsyndromal delirium (SSD), characterized by subthreshold symptoms of delirium such as disturbances in attention and cognition, is prevalent in the intensive care unit (ICU) and is associated with prolonged hospitalization and delayed functional recovery. However, evidence-based treatment guidelines remain limited. Here, we present a postoperative patient with SSD successfully managed with daridorexant and quetiapine. A 76-year-old man with a history of pharyngeal cancer and tracheostomy developed acute insomnia in the ICU following surgery for esophageal cancer. Lemborexant was initiated on

Intermediate hypocretin-1 cerebrospinal fluid levels and typical cataplexy: their significance in the diagnosis of narcolepsy type 1.

STUDY OBJECTIVES: The diagnosis of narcolepsy type 1 (NT1) is based upon the presence of cataplexy and/or a cerebrospinal fluid (CSF) hypocretin-1/orexin-A level ≤ 110 pg/mL. We determined the clinical and diagnostic characteristics of patients with intermediate hypocretin-1 levels (111-200 pg/mL) and the diagnostic value of cataplexy characteristics in individuals with central disorders of hypersomnolence. METHODS: Retrospective cross-sectional study of 355 people with known CSF hypocretin-1 levels who visited specialized Sleep-Wake Centers in the Netherlands. For n = 271, we had full data o

Changes in the Cholinergic, Catecholaminergic, Orexinergic and Serotonergic Structures Forming Part of the Sleep Systems of Adult Mice Exposed to Intrauterine Alcohol.

We examined the effect of chronic prenatal alcohol exposure on certain neuronal systems involved with the sleep-wake cycle of C57BL/6J mice exposed to prenatal alcohol once they had reached 56 days post-natal. Pregnant mice were exposed to alcohol, through oral gavage, on gestational days 7-16, with recorded blood alcohol concentration (BAC)s averaging 1.84 mg/ml (chronic alcohol group, CA). Two control groups, an oral gavage sucrose control group (chronic alcohol control group, CAc) and a non-treated control group (NTc), were also examined. At 56 days post-natal, the pups from each group were

Narcolepsy type 1: an environment-influenced disease.

BACKGROUND AND PURPOSE: Narcolepsy type 1 (NT1) is a chronic and non-progressive neurological disorder that frequently arises in childhood. NT1 is characterized by loss of physiological boundaries between wakefulness and sleep, excessive daytime sleepiness, cataplexy, sleep paralysis, and hypnagogic or hypnopompic hallucinations. Current evidence supports a complex multifactorial model in which NT1 results from the interplay of immune-related genetic predisposition and environmental triggers, leading to immune-mediated destruction or dysfunction of hypocretin-producing neurons. Accordingly, we
